Biomarkers for predicting immunotherapy response and resistance in glioblastoma.

Glioblastoma is the most common and aggressive primary brain tumor in adults that fails to meet survival endpoint criteria in current immunotherapy Phase III clinical trials. As a small subset of immunotherapy-responsive patients has been identified, biomarkers that can nominate patients for immunotherapy and inform treatment combinations have been increasingly urgent. Mechanistically-driven single biomarkers (e.g. PD-1/PD-L1 enrichment, tumor mutational burden, and MGMT methylation) are early opportunities for immunotherapy response prediction but have had inconsistent results across studies.

Cluster of Differentiation 36-Mediated Inflammation and Lipid Metabolism in Cardiovascular Diseases: From Mechanisms to Novel Therapies.

Cardiovascular diseases (CVDs) continue to pose a key challenge to public health because of their high prevalence, mortality, and disability rates, together with a trend toward younger age at onset. Chronic inflammation and disordered lipid metabolism are key pathological mechanisms underlying the development and progression of CVDs. Although numerous pharmacological agents have been developed to intervene in CVDs, current therapies are still limited by adverse effects, suboptimal efficacy, and insufficient anti-inflammatory properties. Consequently, effective drug-based strategies for the pre

Preclinical Studies on the Photothermal Therapy of NIR-II AIEgens: Across Tumor Types.

Malignant tumors continue to pose a significant global health challenge, necessitating the development of precise and efficacious therapeutic strategies. Photothermal therapy (PTT), a minimally invasive modality, employs photothermal agents (PTAs) to convert light into localized heat, thereby elevating the local temperature of tumor tissues to induce hyperthermia-mediated cell death. Aggregation-induced emission luminogens (AIEgens) with second near-infrared (NIR-II) emission have garnered increasing attention owing to their high photothermal conversion efficiency, excellent photostability, an

[Research progress on lipid nanoparticle messenger RNA delivery system].

Messenger RNA (mRNA) therapeutics involve delivering in vitro transcribed mRNA into specific cells to produce target proteins for the treatment or prevention of diseases. However, the development of mRNA therapeutics relies largely on mRNA delivery systems. Lipid nanoparticles (LNPs) represent the most widely used mRNA carriers in clinical applications. Composed of ionizable lipids, zwitterionic phospholipids, cholesterol, and polyethylene glycol-lipids, LNPs can address critical challenges in mRNA drug development, such as poor in vivo stability and the difficulty in crossing biological barri

Prospect of Mesenchymal Stem-Cell-Conditioned Medium in the Treatment of Acute Pancreatitis: A Systematic Review.

Mesenchymal stem cells (MSCs) have demonstrated potential in both clinical and pre-clinical research for mitigating tissue damage and inflammation associated with acute pancreatitis (AP) via paracrine mechanisms. Hence, there has been a recent surge of interest among researchers in utilizing MSC cultured medium (CM) and its components for the treatment of AP, which is recognized as the primary cause of hospitalization for gastrointestinal disorders globally. A systematic review was conducted by searching the MEDLINE, EMBASE, and Web of Science databases. Studies that involve the administration

Practical Approach in Management of Extraosseous Ewing's Sarcoma of Head and Neck: A Case Series and Review of literature.

Extraosseous Ewing's Sarcoma (EES) is a high grade rare malignancy from Ewing's family tumors (EFTs) occurring in soft tissues. Diagnosis of EES relies on a constellation of features, including round cell morphology, characteristic immunohistochemistry (such as CD99, FLI-1 and NKX2.2 positivity), and pathognomic molecular abnormalities of t(11;22)(q24;q12). Multimodality treatment has improved the prognosis and clinical outcome in EFTs. Due to its rarity, the current recommendation to treat EES is based on Skeletal Ewing's Sarcoma (SES) guidelines. However, achieving clear surgical margins in

Mesenchymal stem cell-derived secretomes for therapeutic potential of premature infant diseases.

Preterm birth is a complex syndrome and remains a substantial public health problem globally. Its common complications include periventricular leukomalacia (PVL), bronchopulmonary dysplasia (BPD), necrotizing enterocolitis (NEC) and retinopathy of prematurity (ROP). Despite great advances in the comprehension of the pathogenesis and improvements in neonatal intensive care and associated medicine, preterm birth-related diseases remain essentially without adequate treatment and can lead to high morbidity and mortality. The therapeutic potential of mesenchymal stem/stromal cells (MSCs) appears pr

Oral Bifidobacterium longum expressing alpha-melanocyte-stimulating hormone to fight experimental colitis.

The oral delivery of peptides is a highly attractive treatment approach. However, the harsh environment of the gastrointestinal tract limits its application. Here, we utilize Bifidobacterium as a delivery system to orally deliver a potent anti-inflammatory but short duration peptide alpha-melanocyte-stimulating hormone (α-MSH) against experimental colitis. The aim of our study was to facilitate the efficient oral delivery of α-MSH. We designed a vector of pBDMSH and used it to construct a Bifidobacterium longum expressing α-MSH. We then determined the bioactivity of recombinant Bifidobacterium

The sodium/iodide symporter: state of the art of its molecular characterization.

The sodium/iodide symporter (NIS or SLC5A5) is an intrinsic membrane protein implicated in iodide uptake into thyroid follicular cells. It plays a crucial role in iodine metabolism and thyroid regulation and its function is widely exploited in the diagnosis and treatment of benign and malignant thyroid diseases. A great effort is currently being made to develop a NIS-based gene therapy also allowing the radiotreatment of nonthyroidal tumors. NIS is also expressed in other tissues, such as salivary gland, stomach and mammary gland during lactation, where its physiological role remains unclear.

Moving towards a cure: blocking pathogenic antibodies in systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is characterized by the presence of autoantibodies that can mediate tissue damage in multiple organs. The underlying aetiology of SLE autoantibodies remains unknown, and treatments aimed at eliminating B cells, or limiting their function, have demonstrated limited therapeutic benefit. Thus, the current therapies for SLE are based on the concept of nonspecific immunosuppression and consist of nonsteroidal anti-inflammatory drugs (NSAIDS), corticosteroids, anti-malarials and cytotoxic drugs, all of which have serious adverse side effects including organ damage.

Endophthalmitis as primary clinical manifestation of fatal fusariosis in an allogeneic stem cell recipient.

The occurrence of infections due to previously rare opportunistic pathogens is increasing despite the use of novel treatment strategies for immunocompromised patients. Here, we report the case of a patient presenting with fever, muscle pain, and bilateral endophthalmitis after allogeneic hematopoietic stem cell transplantation. Fusarium solani was isolated from peripheral blood samples and identified as the cause of gradual bilateral vision loss, despite appropriate antifungal prophylaxis, and therapy including vitrectomy and intraocular instillation of antifungal agents. The patient became co

Heat shock protein families 70 and 90 in Duchenne muscular dystrophy and inflammatory myopathy: balancing muscle protection and destruction.

Heat shock proteins are important factors in skeletal muscle physiology and stress response. We examined the effects of chronic inflammation on the distribution of heat shock protein families 70 and 90 using immunofluorescence and Western blotting, in muscle biopsies from 33 idiopathic inflammatory myopathy patients [aged 26-66 (dermatomyositis), 17-78 (polymyositis) and 57-80 (sporadic inclusion body myositis) years], and seven Duchenne muscular dystrophy patients (aged 3-19 years). Our results reveal the multifaceted role played by chaperones in inflammatory muscle tissue. On the one hand, r

Chronotherapy of colorectal cancer.

Chronotherapy consists of chemotherapy delivery according to circadian rhythms. These genetically based rhythms modulate cellular metabolism and cell proliferation in normal tissues. As a result, both the host tolerance and antitumor efficacy of 5-fluorouracil (5-FU) and oxaliplatin (L-OHP), like 30 other anticancer drugs, vary largely according to the dosing time in laboratory rodents. The transfer of this concept to the clinic is aimed primarily at increasing the dose-intensity of the therapy through adjustment of drug-delivery to 24h rhythms in host tolerance. [Pseudotumor focal xanthogranulomatous pyelonephritis in adults. Role of imaging technics in the diagnosis].

Based on 8 personal cases, pseudo-tumoral xanthogranulomatous pyelonephritis is reviewed with emphasis on diagnosis. The condition is a particular form of chronic renal suppuration of histologic definition (combined lesions of chronic pyelonephritis and xanthogranulomatous foam cells). Two forms are recognized: one diffuse, fairly frequent form corresponding to a pyonephrosis, and a pseudo tumoral focal form, the only type discussed in this report, which raises the problem of diagnosis of a renal mass that requires the application of all currently available exploration means to define its true

BI-TfR1 CapX rapidly transits the blood-brain barrier for efficient, low-dose gene delivery throughout the CNS.

Safe, efficient gene delivery throughout the CNS remains a central obstacle to treating genetic diseases of the brain and spinal cord. We describe BI-TfR1 CapX, a human transferrin receptor (TfR1)-binding AAV capsid that, after a low intravenous dose in adult humanized TFRC mice, transduced more than 80% of cortical and spinal neurons and ∼60% of cortical astrocytes, with reduced distribution and expression in several peripheral tissues relative to AAV9. Unlike other engineered BBB-crossing capsids, CapX bound the CNS vasculature within minutes and a large fraction of its genomes reached paren

Biomimetic catalase-engineered black phosphorus nanosheets enhance photo-/radio-therapy for hepatocellular carcinoma.

The hypoxic tumor microenvironment (TME) critically impairs the therapeutic efficacy of photo- and radiotherapy (RT) in hepatocellular carcinoma (HCC). Ameliorating tumor hypoxia thus presents a promising strategy to potentiate these treatment modalities. Herein, we developed a biomimetic nanoplatform by functionalizing black phosphorus nanosheets (BP) with catalase (CAT), forming a stable nanocomposite (BPCAT) via covalent bonding between carboxyl and hydroxyl groups. To enhance tumor-targeting capability and immune modulation, BPCATwas further camouflaged with macrophage membranes engineered

A Rare Case of Pulmonary Artery Intimal Sarcoma Diagnosed Antemortem by Endoscopic Ultrasound-Guided Fine-Needle Aspiration.

Primary pulmonary artery sarcoma (PPAS) is a rare malignancy often misdiagnosed as pulmonary thromboembolism. Antemortem diagnosis is often difficult because of the tumor's deep intravascular location and challenges in obtaining tissue samples. We report a 72-year-old man with pulmonary artery intimal sarcoma diagnosed antemortem using endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA). Contrast-enhanced computed tomography demonstrated a heterogeneously enhancing intraluminal pulmonary artery lesion, and positron emission tomography/computed tomography revealed intense fluorodeoxyg

Proptosis Revealing a Neurological Form of IgG4-Related Disease: A Case Report and Literature Review.

IgG4-related disease (IgG4-RD) is a chronic, immune-mediated fibro-inflammatory condition characterized by tissue infiltration of IgG4-positive plasma cells, with a continually expanding spectrum of clinical manifestations. Neurological involvement is uncommon, and imaging findings are often nonspecific, frequently delaying diagnosis. The condition generally carries a favorable prognosis due to its responsiveness to corticosteroid therapy. We report the case of a 30-year-old man presenting with a nine-month history of progressive, painless left-sided proptosis and eyelid swelling, without foca
