A review of association and linkage studies for genetical analyses of learning disorders.

Learning disorders (LD) commonly comprise of a heterogeneous group of disorders manifested by unexpected problems in some children's experiences in the academic performance arena. These problems especially comprise of a variety of disorders which may be subclassified to attention-deficit hyperactivity disorder (ADHD), reading disability (RD), specific language impairment (SLI), speech-sound disorder (SSD), and dyspraxia. The aim of this review is to summarize the current molecular studies and some of the most exciting recent developments in molecular genetic research on LD. The findings for th

The influence of color on transient system activity: implications for dyslexia research.

Metacontrast and apparent motion experiments designed to utilize transient system resources were adopted to investigate the proposal that transient system activity is differentially influenced by different colored stimuli. The results generally showed no effect of color on transient system activity in either adults or children. However, the predicted pattern of results was demonstrated when contrast rather than color was manipulated in a final metacontrast experiment. We discuss the tenuousness of the proposal that color differentially influences transient activity, exploring its physiological

Evaluation, treatment, and transport of pediatric patients with shock.

Early recognition and correct treatment of shock remain the most important keys to preventing the death and disability frequently caused by this condition in children. The pediatrician plays a vital role in this process and in referral of the patient for transport to tertiary care centers, where shock is best managed. The transport environment creates special challenges in initial stabilization and ongoing treatment of shock. Discussion centers on clinical clues to recognition, on simple measures available to increase tissue oxygenation, and on the issues of pretransport and transport treatmen

Fatty liver, encephalopathy, and sudden unexpected death in early childhood due to medium-chain acyl-coenzyme A dehydrogenase deficiency.

A case of sudden death associated with fatty liver and encephalopathy is described in a 4-year old white boy with medium-chain acyl-coenzyme A dehydrogenase (MCAD) deficiency. The death was caused by hypoglycemia triggered by fasting and vomiting associated with a minor viral infection. The differential diagnosis of the hepatoencephalopathy is discussed in relation to other conditions, especially Reye's syndrome. The forensic pathologist should be familiar with MCAD and other deficiencies of beta-oxidation of fatty acids as a cause of sudden unexpected death in children in order to advise pare

Tuberculous meningitis in children in the Western Cape. Epidemiology and outcome.

The incidence of tuberculous meningitis in children was determined using hospital records as well as local authority notifications. One hundred and eighty-five cases occurred over a 3-year period. The age-specific incidence in the 0-14-year-old group was 7,5/100 000. In only 28 cases was the disease at an early stage when treatment was commenced. Young age and late-stage disease at presentation were associated with a poor outcome. The associated morbidity and mortality rates were high; the mortality rate was 24%, and nearly 50% of patients were left with a residual disability.

Family abuse of the elderly.

In 12 cases of family abuse of disabled elderly persons, the majority of the abused were women. Caretakers included spouses, children, siblings or other relatives. Abuse was both physical and verbal/psychologic. Neglect, the most common form of physical abuse, sometimes resulted in decubitus ulcers and vermin infestation. Misuse of medical therapy and nutrition were other areas of physical abuse. Psychologic abuse included derogation, infantilization, and threats of institutionalization, abandonment, and homicide. Possible causes of abuse included alcoholism in the caretaker, financial concern

The floating knee: Ipsilateral fractures of the tibia and femur.

Fifty-one extremities with ipsilateral tibial and femoral fractures are reviewed. Some children are included in the study. Most of the 47 patients are young men who have multiple injuries with high velocity trauma. The involved extremities are classified according to the injury site: Type 1 fractures involve both shafts; type 2-A fractures involve the knee joint; type 2-B fractures involve hip or ankle joints. Complications are frequent. Delayed union or nonunion occurred in 44.6% of the total number of bones involved. Sixty to seventy percent of the adult patients showed significant permanent

RHEUMATOID ARTHRITIS. PHYSICAL MEASURES IN TREATMENT OF CHILDREN.

Prognosis in rheumatic arthritis in children is good, provided total care is given, deformity prevented and function maintained. Bed rest is desirable until active inflammation of the joints has subsided. During convalescence a balance between rest and exercise must be maintained to avoid recurrence of inflammation of the joints. When there is progressive deformity or disabling pain in the wrist, a molded leather wrist-cuff splint can control the deformity, decrease or abolish pain and lessen swelling. If there is valgus deformity of the knee or external rotation of the tibia, with no more tha

Interpretation of genome sequencing results in children with complex neuromotor phenotypes.

BACKGROUND: Although genetic diseases cause at least 30% of neuromotor disorders, genome sequencing is not routinely used for diagnosis in such patients. We assessed the diagnostic value in children with complex neuromotor phenotypes. METHODS: We performed trio genome sequencing in 108 children with complex neuromotor phenotypes in 100 families. Genetic disease was diagnosed by a multidisciplinary clinical research team on the basis of each child's complete medical history, clinical course, family history, physical examination, imaging studies, and all other laboratory test results, as well a

Pediatric Autoimmune Neurologic Disorders.

OBJECTIVE: This article discusses common principles in diagnosing and managing autoimmune neurologic conditions in children. LATEST DEVELOPMENTS: The key to improving outcomes in all patients with autoimmune neurologic diseases is making an early diagnosis, promptly initiating treatment, and identifying patients who will benefit from long-term maintenance treatment. Some neuroinflammatory syndromes can be diagnosed with an antibody biomarker (eg, aquaporin-4 antibodies, N-methyl-d-aspartate [NMDA] receptor antibodies), whereas others require clinical diagnostic criteria (eg, multiple sclerosi

Combining Unimanual and Bimanual Therapies for Children with Hemiparesis: Is There an Optimal Delivery Schedule?

Constraint-induced movement therapy (CIMT) and bimanual therapy (BT) are among the most effective hand therapies for children with unilateral cerebral palsy (uCP). Since they train different aspects of hand use, they likely have synergistic effects. The aim of this study was to examine the efficacy of different combinations of mCIMT and BT in an intensive occupational therapy program for children with uCP. Children (n = 35) participated in intensive modified CIMT (mCIMT) and BT, 6 weeks, 5 days/week, 6 h/day. During the first 2 weeks, children wore a mitt over the less-affected hand and engage

Is There Any Association Between Language Acquisition and Cognitive Development in Cochlear-Implanted Children?

OBJECTIVE: Different studies on normal children and children with a sensory or intellectual disability indicate a strong correlation between the child's vocabulary domain and his cognitive abilities. Based on this, the main focus of the present study was to investigate the cognitive performance of cochlear-implanted children after a cognition-based language intervention program. METHODS: In this experimental study, 60 cochlear-implanted children were selected and randomly allocated into case and control groups. The control group received auditory verbal therapy (AVT), while the intervention g

Oxidative Stress in Autism Spectrum Disorder.

According to the United States Centers for Disease Control and Prevention (CDC), as of July 11, 2016, the reported average incidence of children diagnosed with an autism spectrum disorder (ASD) was 1 in 68 (1.46%) among 8-year-old children born in 2004 and living within the 11 monitoring sites' surveillance areas in the United States of America (USA) in 2012. ASD is a multifaceted neurodevelopmental disorder that is also considered a hidden disability, as, for the most part; there are no apparent morphological differences between children with ASD and typically developing children. ASD is diag

Neuropsychological function in children with primary complex motor stereotypies.

AIM: Complex motor stereotypies (CMS) are patterned, repetitive, rhythmic, and involuntary movements that persist over time. They are divided into two subgroups dependent on the presence of other developmental problems: 'primary' (development is otherwise typical) or 'secondary' (associated with autism, intellectual disability, or sensory deficits). There are no currently published studies that examine neuropsychological function in children with primary CMS. This case-control study examines whether children with primary CMS manifest neurobehavioral deficits. METHOD: Fifty-seven children with

Systematic review of diarrhea duration and severity in children and adults in low- and middle-income countries.

BACKGROUND: Diarrhea is a leading cause of morbidity and mortality globally; yet the overall burden of diarrhea in terms of duration and severity has not been quantified. As improvements in treatment lead to decreases in diarrhea mortality, it is important to understand the substantial impact of diarrhea morbidity on disability among children and adults worldwide. METHODS: We conducted a systematic review to generate estimates of duration and severity outcomes for individuals 0-59 mos, 5-15 yrs, and ≥ 16 yrs, and for 3 severity indexes: mild, moderate, and severe. RESULTS: We estimate that a
