The symptomatic effect of pancreatic enzyme substitution therapy was examined in a 4-week double-blind crossover study. Twenty patients, 11 with and 9 without steatorrhoea, were examined. Pancreatic steatorrhoea was reduced from a median of 24 g/day to 10 g/day by the enzyme therapy (P less than 0.01). No significant pain reduction was found in either of the two groups, although there was a tendency to reduction in pain and analgetic consumption in the patients with steatorrhoea when treated with pancreatic enzymes. Fifty-one patients were treated in a dual-centre, double-blind comparison of acyclovir and adenine arabinoside in herpetic amoeboid (geographic) corneal ulceration. Anti-ischaemic effects and safety of nicardipine were assessed in 14 patients with vasospastic angina using a placebo comparison, cross-over design study for 8-13 weeks. The average daily dose of nicardipine for optimal angina prevention was 84 mg (range 40-160 mg). Nicardipine administration, as compared with placebo, significantly reduced anginal frequency and nitroglycerin consumption during the single- and double-blind phases of the study. Nicardipine appears to be effective in the prevention of vasospastic angina and not to cause major adverse effects.

A double-blind study was performed in 83 patients with generalized anxiety disorder comparing daily doses of 5 and 10 mg ritanserin (a selective antagonist of serotonine S2 receptors) versus placebo and 4 mg lorazepam as reference drug. A controlled clinical trial on the efficacy of a nortriptyline-fluphenazine combination was carried out in patients with painful diabetic polyneuropathy. A visual analog scale was used to evaluate the relief of pain or paresthesia. Significant relief of both pain and paresthesia was obtained with this combination. The differences were statistically significant. Side effects were frequent but not usually severe enough to lead to cessation of these medications.

A small, double-blind crossover study compared the efficacy of aminophylline and doxapram in ventilator weaning of eight premature infants recovering from respiratory distress syndrome (RDS). Although neither drug was significantly better than the other, four infants were weaned from mechanical ventilation after drug administration. It is suggested that drugs stimulating the respiratory center may aid in shortening the duration of mechanical ventilation in premature infants recovering from RDS.

Clonazepam has been reported to be of some value in the treatment of benign essential tremor in open trials. This study was designed to assess the effect of ranitidine on patients with symptomatic oesophageal reflux. In a double-blind comparative trial in 46 patients a twice daily dose of 150 mg ranitidine was compared with placebo. Relief of pain, endoscopic healing and histological improvement were significantly better in those treated with ranitidine. Thus, ranitidine is of value in the management of patients with reflux oesophagitis and may prevent the development of peptic stricture.
