Clinical manifestations of type IV ulna longitudinal dysplasia.

PURPOSE: Ulna longitudinal dysplasia is an uncommon congenital anomaly that demonstrates a wide variety of clinical manifestations. The clin … strated radiohumeral synostosis (RHS). Three of 14 patients with RHS had bilateral involvement. The elbows were fixed in 20 degrees to 90 degrees of flexion. No elbows were positioned in full extension. Eleven of the 17 involved limbs with RHS had digital anomalies. Nine of the 17 limbs had surgical reconstruction. The majority of these procedures were …

Surgical management of osteochondritis dissecans of the capitellum.

Ten cases of osteochondritis dissecans of the humeral capitellum which were treated surgically are reviewed. All 10 cases were males and involved the dominant side. The ages at surgery ranged from 13 to 17 years. Follow-up ranged from 1 to 7 years. All of the youths had competed in organized athletics, either baseball or football. By position there were three pitchers, two catchers, two infielders, and one outfielder; in addition there were one quarterback and one linebacker. Only one patient presented with locking of the elbow, whereas the others presented with pain and limitation of extensio…

Influence of Different Duct Configurations on Flow Field Uniformity and Aerosol Distribution of Airborne Effluents in Nuclear Facilities: Experimental and CFD Simulation Study.

To ensure the accuracy of single-point sampling for airborne effluents in nuclear facilities, it is critical to clarify how stack and duct c … bulence, resulting in a velocity COV >14% even at a length-to-hydraulic diameter ratio (L/D) of 20; elbows effectively reduced the velocity COV, with right angle transition elbows having a stronger turbulence effect than smooth-transition ones (e.g., L2 circular ducts reached a velocity COV <20% at L/D = 8, while L1 ducts required an L/D of 16 to achiev …

Management of Pediatric Humeral Supracondylar Fractures in a Referral Center From a Developing Country: A Comparison With American Academy of Orthopaedic Surgeons (AAOS) Guidelines.

Introduction Supracondylar fractures are the predominant type of pediatric elbow fractures. The usual mechanism of injury is falling over the hand with the elbow extended and the hand open. The management of these fractures encompasses a range of treatment options, and their goal is to recover the anatomy and achieve a stable contact area between them. There are some controversies on the management of these injuries mainly focused on those that present some degree of displacement. A review and analysis of the current treatment at our institution and a comparison with the guidelines suggested b…

Risk Factors for Revision Surgery Following Radial Head Arthroplasty without Cement for Unreconstructible Radial Head Fractures: Minimum 3-Year Follow-up.

BACKGROUND: Revision rates following radial head arthroplasty (RHA) for unreconstructible radial head fractures (RHFs) differ vastly in the … e most prevalent (80 [65%]). One or more associated osseous or ligamentous injuries were seen in 89 elbows (72.4%). The median time to surgery was 7 days (IQR, 3 to 11 days). Implanted prostheses were categorized as rigidly fixed (65 [52.8%]) or loosely fixed (58 [47.2%]). A total of 28 elbows (22.8%) underwent revision surgery at a median of 1.1 years …

Sex differences in kinematic adaptations to muscle fatigue induced by repetitive upper limb movements.

BACKGROUND: Muscle fatigue induced by repetitive movements contributes to the development of musculoskeletal disorders. Men and women respon … erformed repetitive pointing movements (one per second) between two targets while maintaining their elbow elevated at shoulder height. The task ended when participants rated a perceived level of fatigue of 8/10. Trunk, humerothoracic, and elbow angles were compared between the first and last 30 s of the experiment and between men and women. Linear posit …

Hybrid Fixation of Open Distal Humerus Fracture With Acute Loss of Lateral Articular Condyle.

Acute, major articular bone loss associated with an open fracture is an uncommon injury. These injuries typically occur after high-energy trauma and are challenging to treat. Various approaches to treatment include allografts, prosthetic composite structures, custom implants, and total joint arthroplasty. These treatment options provide a wide array of variability in outcomes, costs, and challenges, especially in young and active patients. Two patients presented to our institution with open elbow injuries that included structural bone loss of the lateral column including the entire capitellar …

Complex coronoid and proximal ulna fractures are we getting better at fixing these?

Technological advances and improved understanding of functional anatomy about the elbow have lead an evolution regarding operative reconstruction of complex proximal ulnar and coronoid fractures. When treating these complex and challenging fractures, goals of anatomic articular restoration along with balanced soft tissue stability can lead to early range of motion and thus, desired functional outcome. The purpose of this review is to outline and provide tips and pearls to achieve desired results, with a comprehensive update on the most recent literature to support the latest fixation methods a…

Isolated medial head of triceps rupture.

Triceps ruptures are less common injuries presenting to the orthopaedic or emergency department setting compared with other musculoskeletal … ding definitive treatment, ranging from 6 to 18 months. The triceps are the primary extensor of the elbow and are supplied with the radial nerve. Any injury to the triceps can adversely affect the functioning of the limb and influence the ability to work and return to employment. We share our experience of treating a patient with a triceps rupture, in w …

Supracondylar osteotomy for the treatment of cubitus varus in children: a systematic review.

Cubitus varus is the most frequent complication following the treatment of supracondylar humeral fractures in children. We investigated data from publications reporting on the surgical management of cubitus varus found in electronic searches of Ovid/MEDLINE and Cochrane Library databases. In 894 children from 40 included studies, the mean age at initial injury was 5.7 years (3 to 8.6) and 9.8 years (4 to 15.7) at the time of secondary correction. The four osteotomy techniques were classified as lateral closing wedge, dome, complex (multiplanar) and distraction osteogenesis. A mean angular corr

Management of lateral humeral condylar fracture in children.

Lateral condylar fractures constitute 12% to 20% of all pediatric distal humerus fractures. These fractures are easily missed and when not managed appropriately can displace. Missed fracture is a common cause of nonunion and deformity; thus, a high index of suspicion and adequate clinical and radiographic evaluation are required. Displaced fractures are associated with a high rate of nonunion. Nondisplaced fractures or those displaced ≤2 mm are managed with cast immobilization and frequent radiographic follow-up. Fractures displaced >2 mm are managed with surgical fixation. Successful outcomes

A case of Horner's syndrome following lateral sagittal infraclavicular block.

Horner's Syndrome results from paralysis of the ipsilateral sympathetic cervical chain (stellate ganglion) caused by surgery, drugs (mainly high concentrations of local anesthetics), local compression (hematoma or tumor), or inadequate perioperative positioning of the patient. It occurs in 100 % of the patients with an interscalene block of the brachial plexus and can also occur in patients with other types of supraclavicular blocks. Horner's syndrome may be described as an unpleasant side effect because it has no clinical consequences in itself. For this reason anesthesiologists should be awa

Overuse injuries in pediatric athletes.

PURPOSE OF REVIEW: The number of children participating in sports has risen steadily over the past few decades. As a result, the incidence of pediatric overuse injuries is rapidly increasing. Currently, primary care physicians are at the frontline in treating these injuries. It is important for these clinicians to be familiar with these types of injuries and their treatment options. RECENT FINDINGS: This article reviews overuse injuries on the basis of location. It discusses the most recent literature describing their presentations, their treatment options, and suggested criteria for return t

[Traumatic dislocation of the radial head associated with ipsilateral fracture of the radial shaft: a case report].

Ipsilateral radial head dislocation with radial shaft fracture is a rare injury. A few cases have been reported in the literature. We report the case of a 39-year-old man who presented with a radial shaft fracture and ipsilateral dislocation of the radial head caused by hyperpronation trauma of the forearm with hyperflexion of the elbow. The dislocation was reduced by manipulation after open reduction of the radial shaft fracture, which was then fixed with a plate. The clinical and radiological outcome was good with 2 years follow-up.

Osteochondral lesions of the capitellum in pediatric patients: role of magnetic resonance imaging.

The role of magnetic resonance imaging (MRI) in the evaluation of patients with osteochondral lesions of the capitellum is undefined. To define its role, the cases of nine consecutive children with 11 capitellar osteochondral lesions who underwent MRI were reviewed. Magnetic resonance imaging accurately delineated the size of the osteochondral lesions and identified capitellar loose bodies not seen on plain radiographs in two elbows. In patients without capitellar loose bodies, two distinct MRI patterns existed that were similar to those seen in femoral head osteonecrosis. Magnetic resonance i…
