Addressing the silent epidemic: a narrative review and evidence synthesis of digital health and telehealth interventions for loneliness in older adults.

BACKGROUND: Loneliness and social isolation represent major threats to the health and well-being of older adults, conferring elevated risks for depression, cognitive decline, cardiovascular disease, and premature mortality. The rapid proliferation of digital health technologies and telehealth services-accelerated by the COVID-19 pandemic-has opened new pathways for addressing these challenges at scale. OBJECTIVE: This narrative review synthesizes the current evidence on the effectiveness of digital health and telehealth interventions in reducing loneliness and social isolation among older adu

Editorial: Improving pandemic and epidemic responses - novel methods and lessons learned from previous infectious disease outbreaks.

Recent outbreaks, including the coronavirus disease 2019 (COVID-19) pandemic, have highlighted the importance of mathematical and statistical modelling in understanding the drivers of transmission and how to tailor responses. Improving future responses requires lessons from these previous outbreaks to be learned and the remaining challenges overcome. This requires developing appropriate mathematical, statistical and computational frameworks that accurately capture the studied mechanisms, leading to a better understanding of how different data can affect these formulations or the simulated inte

COVID-19 in nursing homes: What syndromic and virological surveillance revealed about the first two epidemic waves.

BACKGROUND: Nursing homes were affected by COVID-19, and limited testing capacity during the first wave highlighted the value of complementary surveillance approaches. OBJECTIVE: To characterize clinical presentations, virological findings, and mortality identified through syndromic and virological surveillance during two COVID-19 waves. MATERIALS AND METHODS: A retrospective study was conducted in 16 nursing homes in Northeastern France, including 1,493 residents monitored from February 2020 to February 2021. Residents were classified into four clinical sign patterns: combined general and r

Effectiveness of emergency vaccination in controlling a foot-and-mouth disease epidemic in a previously FMD-free without vaccination country.

Foot-and-mouth disease (FMD) vaccines are key tools for controlling and eradicating FMD virus (FMDV) and are often used as a control measure during epidemics. Emergency vaccination decisions in previously FMD-free countries should be supported by evidence on vaccine effectiveness (VE) against transmission. Mathematical models help assess vaccination strategies but require accurate effectiveness parameters. While experimental studies have shown that FMD vaccination, particularly in cattle, effectively controls transmission, there is limited information on its effectiveness during emergency vacc…

Aligning Licensure and Recommended Use to Expedite Vaccine Access in Response to the Ongoing Bundibugyo Ebolavirus Epidemic in Central Africa.

The ongoing Orthoebolavirus bundibugyoense-Bundibugyo virus (BDBV)-epidemic is the worst Ebolavirus outbreak in a decade and is rapidly expanding, with >4600 suspected cases in the Democratic Republic of Congo (DRC) alone. It has caused 2186 deaths and spread to neighboring countries. As BDBV has no approved vaccine, timely response will require adapted use of existing vaccines approved for other ebolaviruses and expedited development of novel vaccines targeted toward BDBV. Access to vaccines is fundamental to public health emergency response and generally depends on two expert bodies in each

The silent epidemic in sacred lands: knee osteoarthritis burden from high-frequency ritual kneeling, 1990-2021.

BACKGROUND: Knee osteoarthritis (KOA) disproportionately affects populations practicing high-frequency ritual kneeling during religious/cultural activities, inadequately explained by conventional risk models. METHODS: Using Global Burden of Disease 2021 data from 34 countries, we analyzed KOA prevalence, incidence, and disability-adjusted life years (DALYs). Age-standardized rates were computed via Bayesian meta-regression (DisMod-MR 2.1). Temporal trends (1990-2021) were assessed using estimated annual percentage change and Bayesian age-period-cohort modeling, linking biomechanical and cultu

Salvianolic acid A exerts antiviral effects by targeting the S protein, a virulence factor of porcine epidemic diarrhea virus.

UNLABELLED: Porcine epidemic diarrhea virus (PEDV), a member of the Coronaviridae family, infects the small intestinal epithelial cells of pigs, causing porcine epidemic diarrhea (PED), which is particularly severe in young piglets. Owing to its strong immunogenicity, the S1 subunit of the PEDV spike (S) protein mediates viral invasion by recognizing host cell receptors and inducing neutralizing antibodies. However, effective antiviral drugs against PEDV are lacking, and current control measures are limited. In this study, the truncated S1 protein was expressed and used in surface plasmon reso

Epidemic features and potential impact of vaccine campaigns on the 2025 chikungunya outbreak in southern China: a mathematical modelling study.

Chikungunya virus (CHIKV), an emerging arbovirus, is transmitted by Aedes mosquitoes. Climate change and increasing population mobility have driven recent outbreaks beyond traditional endemic regions. Since early July 2025, Guangdong province in southern China has faced an unprecedented outbreak of chikungunya fever. We aim to in-depth describe the epidemic features and theoretically assess the potential impact of vaccination campaigns. In total, the outbreak reported more than 25,000 cases. Foshan and Jiangmen successively emerged as epicenters of the outbreak. Through stringent public health

Coupled Information-Infection Dynamics Produce Information-Driven Shifts in Epidemic Regimes via Competing Vaccine Narratives.

Vaccine-related communication can be harnessed to curb infection; yet, in practice, it often undermines vaccine uptake and sustains transmission even when effective vaccines are available. Our goal is to understand how vaccine information dynamics may shape infection spread within a coupled information-infection modeling framework. We present a coupled information-infection modeling framework that links a standard SVIRS infection-spread model with an integrate-and-fire-inspired information-dissemination model. Vaccine-positive and vaccine-critical active groups disseminate competing informatio

RBM10 suppresses porcine epidemic diarrhea virus replication by degrading nonstructural protein 3 through selective autophagy.

Porcine Epidemic Diarrhea Virus (PEDV) causes highly contagious intestinal disease in swine and results in severe economic losses worldwide. PEDV Nsp3, a nonstructural protein produced in the early stage of viral replication, is responsible for cleaving polyproteins to generate nonstructural proteins and the replication-transcription complex, as well as inducing the formation of coronavirus replication organelles known as double-membrane vesicles (DMVs). Nsp3 is a key protein for viral replication; however, its specific mechanism of action remains unclear. To investigate the role of Nsp3 durin

Multifaceted effects of proline on intestinal injury, inflammation, and lipid metabolism in piglets induced by porcine epidemic diarrhea virus.

This study investigated the effects of proline on intestinal injury in piglets infected with porcine epidemic diarrhea virus (PEDV). Twenty-eight 7-day-old piglets were assigned to four groups: control (CON), proline (Pro), PEDV, and Pro + PEDV. Piglets in the Pro and Pro + PEDV groups received oral proline (500 mg/kg body weight) for 7 days. During the last 3 days, piglets in the PEDV and Pro + PEDV groups were inoculated with PEDV. Under PEDV infection, proline administration increased villus height and the villus height/crypt depth ratio in both the jejunum and ileum, and villus width and v

Genomic Surveillance Reveals Emergence and Spread of Macrolide-Resistant Mycoplasma pneumoniae in Australia During the 2023-2024 Epidemic.

BACKGROUND: The resurgence of Mycoplasma pneumoniae, first reported in China in 2023, was attributed to waning postpandemic immunity with increases in macrolide-resistant M. pneumoniae (MRMP) (> 80%). In Australia, infections peaked in early 2024, particularly among children under 15 years. While MRMP remains low in Europe, North America, and Australia (< 5%), limited routine testing restricts understanding of resistance dynamics. As macrolides are first-line therapy in many settings, MRMP surveillance is essential for guiding empirical treatment. METHODS: We applied a capture-based targeted

Ebola in the rear-view mirror: how epidemic pasts shaped Sierra Leone's preparedness and response to Covid-19.

This paper examines how past experience and legacies of epidemics shaped Sierra Leone's response to COVID-19 and how these influences evolved over time. COVID-19 unfolded in the wake of the West African Ebola epidemic (2013-2016), a crisis which was unprecedented in scale. Despite differing markedly in both transmission patterns and clinical outcomes, the Sierra Leonean government repeatedly invoked Ebola when responding to COVID-19, framing the new outbreak through the lens of the old. Drawing on ethnographic fieldwork and interviews with policymakers, response personnel, health workers, and …

Readiness of health public facilities to diagnose, manage, and prevent the Ebola epidemic along border districts in Southwestern Uganda.

BACKGROUND: Ebola Virus Disease (EVD) outbreaks typically start from a single case of a probable zoonotic transmission and evolve into an epidemic because of the local and international movement of people, causing a global health security threat. This study aimed to assess the readiness of health facilities in the border districts of Southwestern Uganda towards prevention, diagnosis and management of EVD. METHODS: This was a cross-sectional study conducted between July and September 2024. The study was conducted in the districts that share a border with the Democratic Republic of Congo, which

"Never More Than 15 Feet From the Respirator": Housestaff Research During the 1955 Boston Polio Epidemic.

In July 1955, a devastating polio epidemic struck Massachusetts only 3 months after the licensing of the Salk vaccine. One of the last major polio epidemics in the United States, it overwhelmed the local health care system. At Massachusetts General Hospital, 2 assistant residents-the equivalent of today's postgraduate year 2 trainees-were assigned to oversee a hastily organized polio ward and a rising second-year medical student was charged with setting up and running an adjoining laboratory. There, they and other residents conducted original patient-oriented research while providing round-the…

The Effectiveness of Contact Tracing to Reduce Transmission of Infectious Diseases During Epidemic or Pandemic Response: Rapid Systematic Review.

BACKGROUND: Contact tracing (CT), the process of identifying and managing contacts of infected cases, is one public health and social measure that may reduce the spread of infectious diseases. While previous systematic reviews of CT exist, a comprehensive review of both the effectiveness and potential unintended consequences has not been undertaken to our knowledge. Understanding effective CT strategies could help governments and health authorities prepare effectively for emergency epidemic or pandemic situations. OBJECTIVE: This study aims to systematically review the evidence on the effecti

Lessons From Building a Large, Public HIV-Related Database in Support of the Ending the HIV Epidemic in the US Initiative.

The HIV epidemic remains a national priority in the United States, and the Ending the HIV Epidemic initiative has renewed the call for expanded prevention and treatment strategies capable of reducing new HIV infections by 90% by 2030. Achieving this goal requires robust, integrated data for understanding HIV-related needs, barriers to care, and the effectiveness of interventions. However, despite the existence of numerous publicly available datasets, few integrate multiple domains such as HIV outcomes, social determinants of health, and community-level factors. The lack of unified data and dif

TadA-mediated A-to-I mRNA editing rewires redox metabolism to promote dominance of epidemic Klebsiella pneumoniae clones.

Dominant carbapenem-resistant Klebsiella pneumoniae (CRKP) clones have markedly intensified the burden of invasive infections while severely restricting treatment options. Although genomic determinants of these high-risk lineages are well characterized, adaptive regulatory mechanisms beyond fixed DNA variation remain poorly understood. Here we systematically map the adenosine-to-inosine (A-to-I) mRNA editing landscape in CRKP and reveal that clinically dominant lineages possess a constrained and clone-specific landscape of RNA editing. We identify PncR (a previously uncharacterized AraC/XylS f
