Physiological variations in the sagittal spine alignment in an asymptomatic elderly population.

BACKGROUND CONTEXT: Sagittal plane alignment is of crucial importance for the treatment of spinal deformities. When planning corrective strategies, spine surgeons need to refer to normative parameters' ranges which characterize the alignment in the asymptomatic population. Changes are known to occur with age. For the Caucasian population, the reference ranges are extensively documented for the young and middle-aged adults. In contrast, only a few studies have evaluated individuals in advanced age (>60 years) and in groups of limited numbers of subjects (less than 50). PURPOSE: To describe the

The power of peers: an effectiveness evaluation of a cluster-controlled trial of group antenatal care in rural Nepal.

BACKGROUND: Reducing the maternal mortality ratio to less than 70 per 100,000 live births globally is one of the Sustainable Development Goals. Approximately 830 women die from pregnancy- or childbirth-related complications every day. Almost 99% of these deaths occur in developing countries. Increasing antenatal care quality and completion, and institutional delivery are key strategies to reduce maternal mortality, however there are many implementation challenges in rural and resource-limited settings. In Nepal, 43% of deliveries do not take place in an institution and 31% of women have insuff

Increased versus stable doses of inhaled corticosteroids for exacerbations of chronic asthma in adults and children.

BACKGROUND: People with asthma may experience exacerbations or "attacks" during which their symptoms worsen and additional treatment is required. Written action plans may advocate doubling the dose of inhaled steroids in the early stages of an asthma exacerbation to reduce the severity of the attack and to prevent the need for oral steroids or hospital admission. OBJECTIVES: To compare the clinical effectiveness and safety of increased versus stable doses of inhaled corticosteroids (ICS) as part of a patient-initiated action plan for home management of exacerbations in children and adults wit

Micronized progesterone plus dydrogesterone versus micronized progesterone alone for luteal phase support in frozen-thawed cycles (MIDRONE): a prospective cohort study.

STUDY QUESTION: Does the addition of oral dydrogesterone to vaginal progesterone as luteal phase support improve pregnancy outcomes during frozen embryo transfer (FET) cycles compared with vaginal progesterone alone? SUMMARY ANSWER: Luteal phase support with oral dydrogesterone added to vaginal progesterone had a higher live birth rate and lower miscarriage rate compared with vaginal progesterone alone. WHAT IS KNOWN ALREADY: Progesterone is an important hormone that triggers secretory transformation of the endometrium to allow implantation of the embryo. During IVF, exogenous progesterone i

Web-Based Narrative Game Intervention for Initiating Advance Care Planning Among Adults: Exploratory Cross-Sectional Feasibility and Acceptability Study.

BACKGROUND: Advance care planning (ACP) is an ongoing dialogical process. Despite growing policy-level recognition, ACP engagement remains limited in Japan, and many individuals remain unfamiliar with the concept. Beyond knowledge deficits, insufficient psychological readiness may also impede engagement. Evidence on fully online narrative- and game-based approaches to supporting such readiness remains limited. OBJECTIVE: This study aimed to explore the feasibility and acceptability of a fully web-based ACP educational intervention integrating an explanatory video and an interactive game and t

Empowering Older Patients in Advance Care Planning: Evaluating MyCare™ as a Decision Support Tool.

BACKGROUND: Older adults with serious illness often face treatment decisions shaped by personal values, yet recognising and expressing those values in ways that guide decision-making can be challenging. This study evaluated the acceptability, usability and effectiveness of MyCare, a digital values clarification tool designed to help older adults identify and communicate their care preferences to substitute decision-makers, healthcare providers and support persons. METHODS: This convergent mixed-methods study explored older adults' experiences with MyCare. Acceptability was assessed using the

Advancing Discussions Using a Video-Based Support Tool About End-of-Life Care (ADVISE): Protocol for a Single-Center Randomized Controlled Trial.

BACKGROUND: More than 950,000 adults over 65 years of age are hospitalized each year after injury. Recommended care for this vulnerable group includes discussion of their goals for life-sustaining care ("code status") upon admission, but most older patients do not participate in adequate code status discussions. Randomized trials have demonstrated that video-based interventions are associated with better patient knowledge and improved alignment of patient preferences for life-sustaining treatment; however, there are limited data on the effectiveness of these interventions in acutely injured pa

Changing Cirrhosis Burden in the MENA Region, 1990-2021: From Premature Mortality to Disability (Global Burden of Disease).

BACKGROUND: Cirrhosis is a major contributor to disease burden in the Middle East and North Africa (MENA) region, with variations in outcomes across socioeconomic contexts and etiologies. The primary aim of this study was to assess the temporal trends in cirrhosis-related mortality and disability across the MENA region, with a focus on variations by aetiology and Socio-demographic Index (SDI). By identifying patterns and disparities in disease burden, the study seeks to support evidence-based policymaking and optimise regional liver health planning efforts. METHODS: We analysed age-standardis

The Relationship Between Obesity, Bariatric Surgery, and Infertility: A Systematic Review.

BACKGROUND: Obesity is a complicated, chronic condition that has a major impact on reproductive health, leading to infertility, anovulation, and poor pregnancy outcomes. It alters the hypothalamic-pituitary-ovarian (HPO) axis, promotes insulin resistance, and causes persistent low-grade inflammation, all of which result in hormonal abnormalities that compromise normal ovarian function. Because standard weight loss procedures frequently fail to provide significant and long-term reproductive benefits, bariatric surgery is becoming increasingly popular as a therapeutic option for obese women tryi

Surgical management of severe neglected adult idiopathic scoliosis: A review article on challenges and contemporary treatment strategies.

Neglected adult idiopathic scoliosis (NAdIS) is a spinal deformity characterized by severe rigidity and multiplanar curvature, significantly impairing pain levels, physical function, and health-related quality of life. Its complexity is heightened by longstanding vertebral malalignment, cardiopulmonary compromise, and age-related comorbidities, making surgical management particularly challenging. Despite progress in spinal deformity correction, consensus remains lacking on the optimal treatment strategy for these patients. This narrative review synthesizes contemporary surgical options for NAd

Estimating spatially disaggregated probability of severe COVID-19 and the impact of handwashing interventions: The case of Zimbabwe.

INTRODUCTION: The severity of COVID-19 disease varies substantially between individuals, with some infections being asymptomatic while others are fatal. Several risk factors have been identified that affect the progression of SARS-CoV-2 to severe COVID-19. They include age, smoking and presence of underlying comorbidities such as respiratory illness, HIV, anemia and obesity. Given that respiratory illness is one such comorbidity and is affected by hand hygiene, it is plausible that improving access to handwashing could lower the risk of severe COVID-19 among a population. In this paper, we est

Features of effective staff training programmes within school-based interventions targeting student activity behaviour: a systematic review and meta-analysis.

BACKGROUND: Evaluations of school-based activity behaviour interventions suggest limited effectiveness on students' device-measured outcomes. Teacher-led implementation is common but the training provided is poorly understood and may affect implementation and student outcomes. We systematically reviewed staff training delivered within interventions and explored if specific features are associated with intervention fidelity and student activity behaviour outcomes. METHODS: We searched seven databases (January 2015-May 2020) for randomised controlled trials of teacher-led school-based activity

Children of parents who have been hospitalised with psychiatric disorders are at risk of poor school readiness.

AIMS.: Children of parents with psychiatric disorders are at risk of poor outcomes. However, there is limited evidence regarding the relationship between parental psychiatric disorders and child school readiness, which is linked to later academic achievement. This study aims to investigate these relationships and broaden the evidence underlying the rationale for family-focused interventions for parental psychiatric disorders. METHOD.: This study used linked administrative data. Children's school readiness in multiple developmental domains (physical, social, emotional, communicative, cognitive

[Role of abortion and ectopic pregnancies in maternal mortality rate at three university hospitals in Yaoundé].

INTRODUCTION: The World Health Organization (WHO) estimates that 585.000 women die every year in the world as a result of complications related to pregnancy, delivery, postpartum period and abortion (the latter contributing to 13% of maternal deaths). Ectopic pregnancies are responsible for 10% of maternal mortality in the first quarter of pregnancy. Maternal mortality rate is high in Cameroon, estimated at 782 per 100.000 live births according to EDS-MICS 2011. AS the role of these two conditions in maternal mortality is little documented in our country, we conducted this study to assess the

Urogenital schistosomiasis transmission on Unguja Island, Zanzibar: characterisation of persistent hot-spots.

BACKGROUND: Elimination of urogenital schistosomiasis transmission is a priority for the Zanzibar Ministry of Health. Preventative chemotherapy together with additional control interventions have successfully alleviated much of the disease burden. However, a persistently high Schistosoma haematobium prevalence is found in certain areas. Our aim was to characterise and evaluate these persistent "hot-spots" of transmission and reinfection in comparison with low-prevalence areas, to support the intervention planning for schistosomiasis elimination in Zanzibar. METHODS: Prevalences of S. haematob

Surveillance of above- and below-ground mosquito breeding habitats in a rural midwestern community: baseline data for larvicidal control measures against West Nile Virus vectors.

BACKGROUND: Mosquitoes in the genus Culex are thought to play a major role as vectors in the transmission cycle of West Nile virus (WNV) and other arboviruses in the United States. Effective control of mosquitoes through larviciding and adulticiding is expensive for communities and should be guided by reliable surveillance data on the distribution of mosquito breeding habitats. However, few small to medium sized cities in rural areas of the midwestern United States have this type of baseline information available. OBJECTIVE: During the summer of 2004, we investigated the characteristics of Cu

Development of a core outcome set for the evaluation of shared decision-making interventions in healthcare: a study protocol.

INTRODUCTION: Shared decision-making (SDM) is a process whereby patients are supported to reach decisions about their healthcare in collaboration with healthcare professionals. Despite decades of SDM research, the impact of using SDM interventions (eg, patient decision aids, decision coaching, question prompt lists, training and feedback or service changes) on clinical and health service outcomes remains uncertain. This is partly because synthesis of the existing literature is hindered by substantial heterogeneity in the evaluation of interventions to facilitate SDM. A core outcome set (COS) i
