BACKGROUND: The plasma aldosterone-to-renin ratio (ARR) for the diagnosis of primary aldosteronism is normally calculated with plasma renin activity (PRA) as denominator. However, new direct renin assays that measure plasma renin concentration (PRC) are progressively replacing PRA because these are faster, simpler, and more reproducible.
OBJECTIVE: To assess whether the calculation of ARR with a direct assay (ARRD, ng/dl/mU/l) instead of PRA (ARRP, ng/dl/ng/ml/h) affects the rate of positive tests in patients on liberal antihypertensive treatment.

OBJECTIVE: In subjects at high risk for sleep apnea (SA), aldosterone concentrations correlate with severity of SA and primary aldosteronism (PA) is very often diagnosed. Aldosterone was in the past considered only as a prohypertensinogenic agent. It has recently become clear that apart from the classical endocrine action, i.e. causing blood pressure elevation as a result of salt retention, aldosterone has numerous blood-pressure-independent actions on nonepithelial tissue. Under conditions of high salt concentration, aldosterone is injurious to the kidney, heart and vasculature. Of particular interest are recent observations that aldosterone is a permissive factor for the effect of minor increases in plasma sodium concentration on endothelial cell dysfunction.

A patient who was thought to have testicular feminization syndrome and primary aldosteronism had an adrenal scan that suggested an adrenal adenoma. After later diagnosis of 17-alpha-hydroxylase deficiency, she was treated with glucocorticoids rather than surgery. Her clinical course and a repeat adrenal scan confirmed she did not have a tumor.
