Effects of bromocriptine on periodic limb movements in human narcolepsy.

We studied the effects of bromocriptine on periodic limb movements in sleep (PLMS) of narcoleptic patients. Bromocriptine significantly reduced the number and index of PLMS without improving the nocturnal sleep disruption in that disorder. These results support the hypothesis that dopaminergic mechanisms are involved in the physiopathology of PLMS, and suggest that PLMS do not play a major role in the nocturnal sleep disruption of narcoleptic patients.

Magnetic stimulation study in mirror movements.

A young man with congenital mirror movements was studied by non-invasive magnetic stimulation. Radiological examination showed no craniocervical or pituitary abnormality except for mild atrophy of the right hippocampus. Magnetic stimulation of the motor cortex caused large amplitudes in the ipsilateral hand muscles, indicating the possibility of functional disorder in the motor pathways from the motor cortex to muscles in the upper extremities.

Tourette-like disorder in Asperger's syndrome.

An eight-year-old boy with Asperger's syndrome was given haloperidol to control agitation and aggressive outbursts. Withdrawal of the drug after two years was followed by Tourette-like symptoms. Subsequently neither haloperidol nor a second antipsychotic drug altered the core features of Asperger's syndrome, despite suppressing the movement disorder. His first exposure to neuroleptics was in utero.

Heterocyclic antidepressants in nonpsychiatric disorders.

Heterocyclic antidepressants have been used successfully in the treatment of migraine, enuresis and encopresis, peptic ulcer disease, irritable bowel syndrome, chronic pain, narcolepsy, sleep apnea and attention deficit disorder. The mechanism of their therapeutic effects in these conditions is still unclear. Serotonergic, noradrenergic, anticholinergic and antihistaminic properties and rapid-eye-movement sleep suppression have been implicated.

Isolated Hemichorea-Hemiballismus as the Presenting Manifestation of Thalamic Intracerebral Hemorrhage: A Long-Term Treatment Course With Tetrabenazine and Aripiprazole.

Post-stroke movement disorders are uncommon, and hemichorea-hemiballismus is most often associated with lesions involving the basal ganglia or subthalamic region. Thalamic lesions are a recognized but less frequent cause. We report a 78-year-old right-handed woman who developed sudden involuntary jumping and dancing movements of the right foot, which later spread to the right arm. She had no weakness, sensory loss, dysarthria, visual symptoms, headache, or loss of consciousness. Examination showed continuous right-sided hemichorea-hemiballismus with preserved strength and sensation. Non-contra…

Measuring What Matters: Further Validation for the Tardive Dyskinesia Impact Scale, a Novel Patient-Reported Outcome Measure in Valbenazine Clinical Trials.

Importance: Tardive dyskinesia (TD) is a persistent, potentially disabling, medication-induced movement disorder that has been underrecognized. Involuntary movements in TD have a substantial impact beyond movement on individuals with TD. To quantify TD impact and burden, the Tardive Dyskinesia Impact Scale (TDIS), a new, TD-specific, fit-for-purpose patient-reported outcome (PRO) measure, was developed. Objectives were to examine how TDIS contributes to understanding of TD burden and use of clinician-reported outcomes (ClinROs) and other PROs in clinical trials assessing effects of vesicular m

Home-based video-oculography during spontaneous vertigo attacks: A proof-of-concept feasibility study in Ménière's disease and vestibular migraine.

BackgroundEpisodic vestibular disorders like Ménière's disease (MD) and vestibular migraine (VM) pose diagnostic challenges because patients are often assessed between attacks, when clinical signs are absent. Recording ictal eye movements could provide objective phenotypic data; however, the practicality of using unsupervised home-based video-oculography in everyday clinical settings remains poorly studied.MethodsWe conducted a prospective proof-of-concept study in patients with episodic vertigo. Participants were given portable, wireless video-oculography goggles for unsupervised home use and

Saccadometry Findings in Migraine Patients Without Aura.

INCLUDING: Migraine without aura is a common neurological disorder associated with structural and functional changes in the brain, which may lead to impairments in cognitive control and motor function. Functional magnetic resonance imaging (fMRI) has demonstrated changes in cortical gray matter volume associated with pain and ocular function in patients with migraine without aura. OBJECTIVE: This study aims to evaluate prosaccade and antisaccade eye movements in patients with migraine without aura (MWOA) and determine whether the saccadometry test is an effective tool for identifying cognitiv

Sleep-dependent motor sequence memory consolidation in individuals with periodic limb movements.

Periodic limb movements (PLMs) during sleep increase with age and are associated with striatal neurodegeneration and dopamine deficiency. Limb movements are often associated with disruptions to non-rapid eye movement (NREM) sleep. Motor skill memory consolidation recruits the striatum, and learning-dependent striatal activation is associated with NREM sleep. Therefore, we investigated whether de novo individuals who significantly experience elevated levels of PLMs but have not been formally diagnosed with periodic limb movement disorder had learning and sleep-related memory deficits and whethe

Performing the HINTS-exam using a mixed-reality head-mounted display in patients with acute vestibular syndrome: a feasibility study.

BACKGROUND: In patients with acute vestibular syndrome (AVS) differentiating between benign acute peripheral vestibular disorders and possible life-threatening central, causes such as stroke, can be challenging due to similar symptoms. AVS patients experience dizziness, vertigo, imbalance, nausea, vomiting, and abnormal eye movements. This research evaluates the feasibility of using the eye-tracking capability of a mixed reality optical-see-through head-mounted display (MR-OST-HMD) to detect pathological eye movement patterns in patients with AVS. METHODS: Conducted at University Hospital Ess

Beta burst characteristics predict instantaneous hand movements in Parkinson's disease.

Parkinson's disease (PD) is a neurodegenerative disorder characterized by bradykinesia, rigidity, and tremor. Amplification of beta-band oscillations in both cortical and subcortical areas have often been implicated in the pathoetiology of PD symptomatology. Although previous work has linked prolonged beta bursts to motor impairment, few studies have examined the moment-by-moment causal role of beta bursting in driving movement kinematics. We hypothesize that beta burst characteristics predict instantaneous hand movement dynamics and may, therefore, play a fundamental role in the pathophysiolo

The neurological phenotype of developmental motor patterns during early childhood.

INTRODUCTION: During early childhood, typical human motor behavior reveals a gradual transition from automatic motor patterns to acquired motor skills, by the continuous interplay between nature and nurture. During the wiring and shaping of the underlying motor networks, insight into the neurological phenotype of developmental motor patterns is incomplete. In healthy, typically developing children (0-3 years of age), we therefore aimed to investigate the neurological phenotype of developmental motor patterns. METHODS: In 32 healthy, typically developing children (0-3 years), we video-recorded

Tics and developmental stuttering.

BACKGROUND: Developmental stuttering affects 1% of the population but its cause remains unclear. Recent PET studies of metabolism in the central nervous system suggest that it may be related to dysfunction in the basal ganglia or its connections with regions of the cortex associated with speech and motor control. OBJECTIVE: To determine the presence and characteristics of involuntary movements (IMs) in people who stutter and to investigate the hypothesis that these movements may be of a very similar nature to the IMs seen in patients with movement disorders due to basal ganglia dysfunction. …

Survey on awareness of sleep medicine among members of the Japanese Society of Neurology.

BACKGROUND: Sleep-wake problems and sleep-related disorders, which are common attributes of neurological diseases, often remain under-recognized and under-treated. Insufficient education in sleep medicine might contribute to this gap. METHODS: The Sleep Medicine Section of the Japanese Society of Neurology conducted a nationwide, web-based survey of its full-member physicians (N = 9,703), assessing neurologists' education, clinical practice, and interest in sleep medicine. RESULTS: Of the 904 survey participants, only 10% considered their undergraduate education on sleep-wake problems and sl
