Temporal dermoid--an unusual presentation.

It is rare for a frontotemporal dermoid cyst to present as a discharging sinus, and even more rare for it to have intracranial extensions. Only a few cases of intraorbital extension have been reported. We report a 14-month-old girl who presented with all the aforementioned features. She had a temporal dermoid with three discharging sinuses over the temporal area, lower lid, and cheek. It also had an intraorbital extension through the lateral orbital wall and an intracranial extension through the temporal bone. Preoperative computed tomography was done as there was a history of "orbital celluli

A comparison of the accuracy of echo features during endoscopic ultrasound (EUS) and EUS-guided fine-needle aspiration for diagnosis of malignant lymph node invasion.

BACKGROUND: The purpose of this study was to re-evaluate echo features of lymph nodes during endoscopic ultrasound and assess the utility of these echo features and endoscopic ultrasound-guided fine-needle aspiration in predicting malignant lymph node invasion. METHODS: Thirty-five lymph nodes in 25 patients with lung, esophageal, and pancreatic cancer were evaluated by endoscopic ultrasound. Endoscopic ultrasound examinations were performed with a radial scanning echoendoscope. Confirmation of benign lymph nodes was obtained by surgical resection while malignant lymph nodes were confirmed by

[Therapy of preinvasive vulvar neoplasia--standardized or individual?].

The mostly diagnosed preinvasive vulvar neoplasia is of squamous origin called "vulvar intraepithelial neoplasia" (VIN). The knowledge about the biological character of these lesions especially grade 3 is limited. Progression to invasive cancer is expected in > 40 % of untreated patients who are mostly immunocompromised or elderly. After surgical treatment of VIN the risk of invasive cancer is about 4 %. There are no significant differences in later progression and relapse rates between different surgical approaches. Risk factors for relapse are: younger age, positive margins, grade 3, multice

[Upper influx distension in a patient with HIV; unusual localization of an HIV-associated lymphoma].

A 33 year old patient was admitted to the hospital because of deteriorated general condition, upper abdominal pain and progressive dyspnea. He had a positive HIV-serology associated with i.v. drug abuse. The CDC classification on admission was B1. There was no history of opportunistic infections, the patient had refused all prophylactic treatment. The physical examination showed an elevated central venous pressure, decreased breath-sound and percussible dullness, the liver was enlarged and a tumor was palpable on chest. The x-ray of the thorax confirmed a pleural effusion. Cytology of the effu

A proposal for treatment of invasive thymoma.

Eleven patients with invasive thymoma were treated with radiotherapy. All patients had gross anterior mediastinal tumors that invaded the adjacent structures, four of whom showed intrathoracic dissemination. Before radiotherapy, only one had macroscopic complete resection of the tumor, and another had partial resection; the other nine patients underwent biopsy only. All 11 patients achieved complete remission after radiotherapy, eight of whom have been free from relapse for 31 to 202 months. Three patients experienced recurrence within the thorax but outside the radiation fields. Even after wh

[Clinico-pathological features of intracranial germ cell tumors with dissemination or metastasis in childhood].

Recently reports of the cases with intracranial germ cell tumor associated with dissemination to the central nervous system are increased since the introduction of CT scan. The authors experienced 8 cases with the dissemination among 19 cases of intracranial germ cell tumor in childhood, and discussed their clinico-pathological features. Before the CT scan era, the authors experienced only one case with dissemination, who was a 11 years-old boy (Case 1) and revealed two cell pattern pinealoma disseminating on the subependymal layer of all ventricles and in the basal cisterns at autopsy. Since

The significance of extension and impaired mobility in cancer of the vocal cord.

The criteria for T2 glottic cancer staging in the UICC classification are extension to the supra- or subglottic region combined or not with impaired mobility of the vocal cord. The prognostic significance of these factors is examined in this study. Patients with T2A lesions (normal mobility, 33 patients) have an uncorrected actuarial 5 year survival of 54%, and a local control rate with radiotherapy alone of 62%. Patients with T2B lesions (impaired mobility, 28 patients) have a survival of 40% and a local control rate of 65% with radiation only. After rescue surgery, local control is obtained

Pathologic fractures of the mandible following invasive oral carcinomas.

Invasive and metastatic oral carcinomas can and often do result in pathologic fractures of the facial bones. Three cases of invasive oral carcinomas resulting in fractures of the mandible have been reported. It was only after multiple-view radiographs that pathologic fractures could be identified. It is mandatory that dental practitioners avail themselves of the radiographic views which assist in establishing a diagnosis. In addition, the role of the dental profession in diagnosing primary and secondary oral carcinomas cannot be overemphasized.

Prognostic impact of invasive features in completely resected NSCLC stratified by tumor diameter.

BACKGROUND: In node-negative non-small cell lung cancer (NSCLC), only Pleural invasion (PI) is reflected in the current TNM staging, whereas the prognostic roles of Lymphatic invasion (LI) and Vascular invasion (VI) remain poorly defined. Moreover, the variation in their prognostic impact across tumor size strata is not well understood. This study aimed to evaluate the prognostic impact of PI, LI, and VI on recurrence-free survival (RFS) in node-negative NSCLC, stratified by tumor size. METHODS: We retrospectively analyzed 692 patients with completely resected, node-negative NSCLC. Cases of a

Perineural invasion in head and neck squamous cell carcinoma: Neuro-immune mechanisms driving immunotherapy resistance and emerging therapeutic strategies.

Perineural invasion (PNI) is a frequent pathological feature of head and neck squamous cell carcinoma (HNSCC) and is increasingly recognized as more than a passive route of tumor dissemination. Emerging clinical and translational evidence suggests that PNI marks a specialized perineural microenvironment that may contribute to reduced sensitivity to immune checkpoint inhibitors (ICIs). Within this niche, tumor-nerve interactions initiate coordinated neural, immune, metabolic, and spatial adaptations that collectively constrain antitumor immunity. Sympathetic and sensory nerves release neurotran

[Innovative imaging techniques for urothelial carcinoma].

BACKGROUND: Imaging guides the staging of urothelial carcinoma and thus plays a central role in treatment decisions. Current guidelines reflect the increasing importance of multiparametric MRI (mpMRI) of the bladder with standardized reporting according to the Vesical Imaging Reporting and Data System (VI-RADS). MATERIALS AND METHODS: This work comprises a narrative, guideline-oriented update based on the European Association of Urology (EAU; 2025 update) and German S3 guidelines (long version 3.0, 2025); recent key studies on VI-RADS-/bpMRT- and PET-based methods; and early targeted imaging

Current Evidence on the Use of Cystoscopy in the Follow-up of Non-Muscle-Invasive Bladder Cancer: Are We Overusing It? Systematic Review of the Literature.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) accounts for the majority of bladder cancers. Its follow-up entails high costs and significant impact on quality of life. Cystoscopy is the standard method, although uncertainties remain regarding the optimal frequency and duration. The aim was to review the available evidence on surveillance schedules and their impact on recurrence and progression. METHODS: A systematic review was conducted following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta- Analyses). Searches in PubMed, Web of Science and Scopus included clinic

Lobectomy Versus Sub-lobectomy for Clinical Stage IA (≤2 cm) Lung Squamous Cell Carcinoma with Tumour Spread through Air Spaces: A Multi-Centre Retrospective Study.

OBJECTIVES: The efficacy of sub-lobectomy for clinical IA (tumours ≤2 cm) lung squamous cell carcinoma with tumour spread through air spaces (STAS) remains unknown. This study compares long-term survival outcomes between sub-lobectomy and lobectomy, aiming to offer pivotal evidence for optimizing resection strategies for clinical IA STAS-positive squamous cell carcinoma patients. METHODS: Consecutive clinical IA STAS-positive squamous cell carcinoma patients undergoing surgery between 2010 and 2020 at 7 high-volume institutions across 5 Chinese cities were retrospectively reviewed. The primar

Single-cell transcriptomics identify a novel macrophage population associated with bone invasion in pituitary neuroendocrine tumors.

BACKGROUND: Bone-invasive Pituitary Neuroendocrine Tumors (BI PitNETs) epitomize an aggressive subtype of pituitary tumors characterized by bone invasion, culminating in extensive skull base bone destruction and fragmentation. This infiltration poses a significant surgical risk due to potential damage to vital nerves and arteries. However, the mechanisms underlying bone invasion caused by PitNETs remain elusive, and effective interventions for PitNET-induced bone invasion are lacking in clinical practice. METHODS: In this study, we performed single-cell (n = 87,287) RNA sequencing on 10 cases

Analysis on clinicopathological characteristics and prognostic factors of adenoma-associated gallbladder carcinoma.

Gallbladder adenomas are relatively rare in clinical practice, which have tendency of malignant transformation. There might be differences in biological characteristics and prognosis between gallbladder carcinoma (GBC) associated and not associated adenoma. Few related literatures have been reported. All consecutive patients who underwent curative-intent resection for GBC at our institution between 2009 and 2021 were retrospectively analyzed. Clinicopathological characteristics and overall survival (OS) were compared between the two groups, including a propensity score-matched analysis. The cl

[mpMRI of the Urinary Bladder: Significance and Clinical Use].

The diagnosis and staging of bladder cancer is challenging. TUR-B and CT diagnostics bear limitations in detecting muscle invasion and perivesical fat invasion. Thus, second TUR-B is required to ensure more reliable pathological staging. The mpMRI of the urinary bladder is a new diagnostic tool in evaluation of tumor muscle invasion. Numerous retrospective and prospective studies indicate high diagnostic accuracy of the mpMRI in this setting. Current meta-analyses show sensitivities of up to 90% with a high specificity of >85%. The "Vesical Imaging-Reporting and Data System" (VI-RADS) is a sta

Combination Strategies to Enhance Bacillus Calmette-Guérin Efficacy for Nonmuscle-Invasive Bladder Cancer.

PURPOSE: Bacillus Calmette-Guérin (BCG) is the standard of care therapy for high-risk nonmuscle-invasive bladder cancer (NMIBC). However, up to half of patients will experience treatment failure, and many face considerable adverse effects, which can lead to dose reduction and intolerance. Novel bladder-sparing approaches with favorable risk-to-benefit ratio are thus critical in NMIBC. MATERIALS AND METHODS: A narrative review of the literature was performed of English-language MEDLINE indexed articles and abstracts of resistance mechanisms to BCG and combination-based strategies to enhance th

Activating Invasion and Metastasis in Small Cell Lung Cancer: Role of the Tumour Immune Microenvironment and Mechanisms of Vasculogenesis, Epithelial-Mesenchymal Transition, Cell Migration, and Organ Tropism.

BACKGROUND: Small cell lung cancer (SCLC) harbours the most aggressive phenotype of all lung cancers to correlate with its bleak prognosis. The aggression of SCLC is partially attributable to its strong metastatic tendencies. The biological processes facilitating the metastasis in SCLC are still poorly understood and garnering a deeper understanding of these processes may enable the exploration of additional targets against this cancer hallmark in the treatment of SCLC. RECENT FINDINGS: This narrative review will discuss the proposed molecular mechanisms by which the cancer hallmark of activa
