Trends of Concurrent Ankle Arthroscopy at the Time of Operative Treatment of Ankle Fracture: A National Database Review.

BACKGROUND: The purpose of this study was to report trends associated with concurrent ankle arthroscopy at the time of operative treatment of ankle fracture. METHOD: The current procedural terminology (CPT) billing codes were used to search the PearlDiver Patient Record Database and identify all patients who were treated for acute ankle fracture in the United States. The Medicare Standard Analytic Files were searchable between 2005 and 2011 and the United Healthcare Orthopedic Dataset from 2007 to 2011. Annual trends were expressed only between 2007 and 2011, as it was the common time period

Quality of life after osteosynthesis of fractures of the proximal humerus.

OBJECTIVE: Most clinical evaluations after fracture treatment focus on pure functional and radiological outcome. However, this may not appreciate the impact on quality of life (QoL). The aim of this study was to assess QoL and its improvement during the first year of postoperative recovery after fracture treatment. DESIGN: Prospective single-centre observational study. SETTING: Trauma unit. City hospital. PATIENTS: From March 2003 to June 2008 inclusion of 269 consecutive patients (72 years, 77% female) was possible. All were treated by open reduction and internal fixation (ORIF) with PHILO

Long-term treatment of osteoporosis in postmenopausal women: a review from the European Society for Clinical and Economic Aspects of Osteoporosis and Osteoarthritis (ESCEO) and the International Osteoporosis Foundation (IOF).

INTRODUCTION: Postmenopausal osteoporosis is a chronic disease requiring treatment that balances long-term fracture efficacy against risk. METHODS: We reviewed the efficacy and safety of calcium and vitamin D, the selective estrogen receptor modulators (SERMs), the bisphosphonates, denosumab, and strontium ranelate in studies of 3 years or longer. RESULTS: Six trials lasted for 5 years, and seven went beyond that. The evidence beyond 5 years is generally weak, mainly due to methodological issues (open-label design, small samples, or absence of placebo control). Although calcium and vitamin D

The cast brace and tibial plateau fractures.

Three hundred six tibial plateau fractures treated at the authors' institutions and in private practice were analyzed in relation to the use of a cast brace or fracture brace. One hundred forty-one of these patients had had a cast brace or fracture brace as part of their treatment program, either as the primary fracture treatment or after open reduction or traction. The aims of this study were (1) to determine if cast bracing could maintain alignment, fracture position, and range of motion and (2) to compare its results with those of other major long-term studies of similar fractures. There we

Screw characteristics and intramedullary fixation for Jones fractures and associated outcomes: A systematic review and meta-analysis.

OBJECTIVE: A Jones fracture is a common injury in foot and ankle care. Open reduction internal fixation (ORIF) versus conservative treatment has a strong literature background supporting ORIF for the active patients. Standard treatment for fixation of the Jones fracture is intramedullary screw fixation. However, there is limited research on how screw characteristics influence outcomes. This systematic review aimed to compare the outcomes based on screw characteristics in the treatment of Jones fractures. METHODS: A search of the Cochrane, EBSCO, Scopus, and PubMed databases from 2000 to 2023

Traumatic Case of Comminuted, Shattered Navicular Bone Fracture: A Rare Case of Midfoot Catastrophe.

INTRODUCTION: The navicular bone is a tarsal bone of the foot. It is an important bone that supports the medial arch of the foot. Navicular bone fractures are a rare injury. Fractures may occur as a result of stress or trauma. In both scenarios, it may be considered high risk due to poor blood supply to the bone. It is more common in athletes while kicking, sprinting, twisting, or falling. We report a case of a comminuted navicular fracture in an individual fixed with the Joshi external stabilization system (JESS). CASE REPORT: A 40-year-old male presented with injury to the right foot. The p

Rare Combination of Talar Body and Bimalleolar Fractures: A Case Report.

Background and Clinical Significance: Talar body fractures are very rare injuries, and their occurrence alongside ipsilateral fractures is even more uncommon. We present a case of a 40-year-old male who sustained a talar body fracture combined with an ipsilateral bimalleolar fracture after falling from a height, a combination previously described in only two cases. Case Presentation: Open reduction and internal fixation (ORIF) were performed using dual approaches for both the talus and malleolar fractures. Postoperatively, the rehabilitation protocol included a non-weight-bearing short leg cas

Are we staying up too late? Timing of surgery of displaced supracondylar fractures in children. Clinical audit in a paediatric tertiary UK trauma centre and literature review.

INTRODUCTION: The British Orthopaedic Association (BOA) guidelines in managing supracondylar humerus fractures in children, outline indications for urgent fixation of these fractures. We present our data from a regional paediatric trauma centre before and after implementing a change in practice as per these guidelines. MATERIALS AND METHODS: Retrospective clinical audit against BOA guidelines. Radiographs, admission clerking notes, operation notes, and clinic letters were all reviewed. We included all displaced supracondylar fractures of the extension type (Gartland Types 2b and 3). The first

Fibular nailing for bi-malleolar and tri-malleolar ankle fractures yields good functional outcomes - a retrospective single-centre study.

INTRODUCTION: The surgical treatment of elderly patients with unstable ankle fractures is challenging. Open reduction and internal fixation with conventional plating technique often yields a poor outcome. However, fracture management with isolated fibular nailing represents an alternative method. METHODS: We performed a retrospective study of patients treated with isolated fibular nailing at the North Zealand Hospital, Denmark, between 1 January 2012 and 31 December 2016. A total of 58 patients were included. The primary outcome was post-operative function (dependency of a walking aid). Secon

AO X-shaped midfoot locking plate to treat displaced isolated greater tuberosity fractures.

Although various implants exist for 3- and 4-part fractures, few implants are appropriate for isolated greater tuberosity fractures. This retrospective study evaluated the efficacy of AO X-shaped midfoot locking plates for greater tuberosity fractures. Between May 2008 and September 2009, nineteen patients with displaced greater tuberosity fractures were treated with open reduction and internal fixation using AO X-shaped midfoot locking plates. Postoperatively, radiographs, functional results, and complications were evaluated. All patients were followed up for a mean 33.2 months (range, 24-42

The AO/ASIF proximal femoral nail antirotation (PFNA): a new design for the treatment of unstable proximal femoral fractures.

INTRODUCTION: PFNA design compacts the cancellous bone to provide increased stability and has been bio-mechanically proven to retard rotation and varus collapse. METHODS: Between 2006 and 2007, 62 consecutive patients with unstable proximal femoral fractures were treated with the PFNA. RESULTS: Twenty males and 42 females with a mean age of 78 years (44-94) were reviewed. In 48 patients, the fracture resulted from a low energy injury. The majority of the fractures belonged to AO/ASIF types 31A2.3 (22) and 31A3.2 (29). Twelve patients required open reduction. The PFNA blade position was centr

Displacement of the common peroneal nerve associated with upper tibial fracture: implications for fine wire fixation.

The constant anatomic position of the common peroneal nerve is relied on when performing fine wire external fixation in the upper tibia. We report the case of a sixty-two-year-old woman with a Schatzker Type V fracture of her right tibial plateau and upper-third diaphyseal fracture associated with displacement and shortening of the upper tibia. She was treated by minimal internal fixation of the intraarticular fracture and application of a Sheffield Hybrid External Fixator. During percutaneous insertion of the reference wire in the fibular head, a distal muscle twitch alerted the surgeon, and

Penetrating Laryngeal Trauma: Management of Severe Injuries Without Open Reduction Internal Fixation.

OBJECTIVE: Thyroid and cricoid cartilage fractures that are displaced or have multiple fracture lines are traditionally managed with open reduction and internal fixation (ORIF). In practice, patients with penetrating laryngeal trauma may have small fragments of cartilage that are difficult to reduce and fixate. This study examines outcomes in patients managed with and without ORIF. METHODS: A retrospective review was conducted of patients aged 18 years and older with penetrating laryngeal trauma who were treated at a single Level I trauma center affiliated with a tertiary-level teaching hospi

Evaluation of Functional Outcome in Surgically Managed Tibial Pilon Fractures.

Background Pilon fractures are infrequent and among the most challenging to manage. One reason is the extensive soft tissue injury surrounding the distal tibia. Second, the articular surface of the distal tibia with a complex fracture pattern needs anatomic reduction. These fractures occur due to high energy impaction of the talus into the distal tibia. The fracture patterns and extent of soft tissue involvement vary based on the intensity of the impact's energy. The management needs to be patient-specific to prevent complications. Proper pre-operative planning with the help of computer tomogr

A structured three-step reduction strategy for locked posterior fracture-dislocation of the proximal humerus through a standard deltopectoral approach: A retrospective case series.

BACKGROUND: Locked posterior fracture-dislocation of the proximal humerus is a rare and technically demanding injury in which the humeral head remains impacted posterior to the glenoid. Although several reduction techniques have been described, the practical details of anterior-only reduction remain incompletely standardized. We evaluated a structured three-step anterior reduction strategy performed through a standard deltopectoral approach. METHODS: We retrospectively reviewed 16 consecutive patients with acute locked posterior fracture-dislocation of the proximal humerus treated between Jan

Malreduction of the tibiofibular syndesmosis in ankle fractures.

BACKGROUND: Diagnosis and reduction of syndesmosis injuries in ankle fractures can be challenging. Previous studies have demonstrated that standard radiographic measurements used to evaluate the integrity of the syndesmosis are inaccurate. The purpose of this study was to determine the adequacy of standard postoperative radiographic measurements in assessing syndesmotic reduction compared to CT and to determine the prevalence of postoperative syndesmotic malreduction in a patient cohort. METHODS: Twenty-five patients with ankle fractures and syndesmotic instability who had open reduction and

Risk factors for avascular necrosis in pediatric femoral neck fractures: a systematic review and meta-analysis.

BACKGROUND: Avascular necrosis (AVN) is a severe complication following pediatric femoral neck fractures (PFNFs). Identifying risk factors for AVN is critical for guiding timely treatment and follow-up. However, prior studies report inconsistent associations. This systematic review and meta-analysis aimed to identify risk factors associated with AVN after PFNFs. METHODS: A comprehensive literature search was conducted in PubMed, Web of Science, EMBASE, and the Cochrane Library through April 30, 2025. Risk ratios (RRs) with 95% confidence intervals (CIs) were calculated to assess the associati

Nonunion Fractures: Trends in Epidemiology and Treatment of Femur Fractures, 2017-2022.

BACKGROUND: The incidence of femur fractures has increased in recent years, along with the rate of surgery and subsequent nonunion following these fractures. Nonunion is a significant concern due to the burden it places on both patients and the healthcare system. This study aims to investigate the demographic factors associated with femoral shaft fracture nonunion by comparing two surgical management approaches: open reduction internal fixation (ORIF) with plating versus closed reduction and fixation using intramedullary nailing (IMN). METHODS: The TriNetX database was queried using current p

Surgical Management of Unstable U-Shaped Sacral Fractures and Tile C Pelvic Ring Disruptions: Institutional Experience in Light of a Narrative Literature Review.

Unstable U-shaped sacral fractures and vertical shear Tile C pelvic ring disruptions are characterized by rare lesions occurring in patients with severe trauma. Because the initial damage-control resuscitation primarily aims to stop life-threatening bleeding, emergency treatment often includes an anterior external pelvic fixator. Delayed surgery is mandatory to allow early mobilization, reduce mortality, and improve functional outcomes. Regarding U-shaped sacral fractures, although Roy-Camille type 1 U-shaped sacral fractures can be treated with iliosacral screws, types 2 (posteriorly displace
