[Anxiety patients before and after hospitalization for psychotherapy - from a psychophysiological standpoint].

Psychophysiological course studies are still the exception in AP hospitalized for psychotherapy. The aim of this study was to perform a more precise psychophysiological analysis of anxiety patients (AP) before and after hospital treatment. From a sample of 28 AP, 13 were psychophysiologically and psychodiagnostically examined and compared to controls (CG) at the beginning and end of in-patient psychotherapy and 4 months later. APs showed higher heart rate (HR) and electrodermal spontaneous fluctuation (SF) than control patients but did not differ in their resting systolic (SBP) and diastolic b

Early improvement predicts endpoint remission status in sertraline and placebo treatments of panic disorder.

The early identification of likely remitters and non-remitters to pharmacotherapy for panic disorder may have important implications for clinical treatment decisions. To address this question, combined data from two fixed-dose and two flexible dose placebo-controlled studies of sertraline treatment of panic disorder were examined. Patients (N=544) diagnosed with panic disorder, with or without agoraphobia, were treated with 50 mg of sertraline, 100 mg of sertraline, flexible dosages of sertraline, or placebo. Measures of early improvement included panic attack frequency (full + limited symptom

Patterns of psychopathology and dysfunction in high-risk children of parents with panic disorder and major depression.

OBJECTIVE: The purpose of the study was to evaluate 1) whether an underlying familial predisposition is shared by all anxiety disorders or whether specific risks are associated with specific disorders, and 2) whether panic disorder and major depression have a familial link. METHOD: The study compared four groups of children: 1) offspring of parents with panic disorder and comorbid major depression (N=179), 2) offspring of parents with panic disorder without comorbid major depression (N=29), 3) offspring of parents with major depression without comorbid panic disorder (N=59), and 4) offspring

Influence of panic-agoraphobic spectrum symptoms on treatment response in patients with recurrent major depression.

OBJECTIVE: The authors tested the hypothesis that a lifetime history of panic-agoraphobic spectrum symptoms predicts a poorer response to depression treatment. METHOD: A threshold for clinically meaningful panic-agoraphobic spectrum symptoms was defined by means of receiver operating characteristic curve analysis of total scores on the Structured Clinical Interview for Panic-Agoraphobic Spectrum in a group of 88 outpatients with and without panic disorder. This threshold was then applied to a group of 61 women with recurrent major depression, who completed a self-report version of the same in

[Psychiatric problems in bone marrow transplantation patients during isolation].

Psychological stress is extremely high in patients with oncological diseases. About 50% of all cancer patients show psychological signs and symptoms which are related to the multiple stress factors of their oncological illness. Although many psychological problems connected with cancer are well known, additional acute stress could result from new therapeutic strategies which require settings similar to intensive care, e.g. bone marrow transplantation. For 2 years the Department of Psychiatry in Innsbruck has provided a "liaison service" at the Department of Bone Marrow Transplantation. Within

Imipramine treatment of panic disorder with agoraphobia: dose ranging and plasma level-response relationships.

OBJECTIVE: The aim of this study was to characterize the specific effects of imipramine in the treatment of panic disorder with agoraphobia and to delineate dose-response and possibly plasma level-response relationships. METHOD: Eighty patients with panic disorder with agoraphobia were randomly assigned, for an 8-week, double-blind dose-ranging trial, to placebo or to a weight-adjusted dose of imipramine: (low) 0.5 mg/kg per day, (medium) 1.5 mg/kg per day, or (high) 3.0 mg/kg per day. Plasma levels of imipramine and N-methylimipramine, patients' and clinicians' ratings of panic and phobic sy

Physical and sexual abuse among orofacial pain patients: linkages with pain and psychologic distress.

This study examines the incidence of and the potential correlates of sexual and physical abuse among facial pain patients. An anonymous survey composed of standardized self-report measures of abuse, pain, and psychologic status was distributed to 120 adult facial pain patients following their initial evaluations. Forty-five questionnaires were returned by mail. In addition, 206 charts were randomly selected from a population of 520 new patients seen at the Orofacial Pain Center during the same time period that data from the anonymous survey were collected. Results of the anonymous survey indic

Effects of fluoxetine on mood and immune status in depressed patients with HIV illness.

BACKGROUND: When present, depression in the context of human immunodeficiency virus (HIV) illness not only detracts from quality of remaining life but may interfere with motivation to obtain good medical care as well, thus directly influencing life's duration. This study was undertaken to assess the efficacy of fluoxetine in treating depression occurring in the context of HIV illness and to assess effects, if any, of fluoxetine on immune status (T-cell subsets). METHOD: Patients had originally participated in a double-blind, placebo-controlled, 6-week study of imipramine. Imipramine nonrespon

Coping as an index of illness behavior in panic disorder.

Illness behavior in panic disorder was examined by comparing the coping strategies of female primary care patients (34 with panic disorder, 30 with simple panic, and 78 without panic.) Relationships of coping and distress were also examined within each group. The groups differed significantly on the Ways of Coping Checklist, anxiety (SCL-90 and Zung scales), depression (SCL-90 and Beck scales), and number of phobias. The panic disorder group used proportionately less problem-focused and more wishful thinking than the other groups. Within the panic disorder group, anxiety and depression were co

Abused to abuser: antecedents of socially deviant behaviors.

The authors interviewed 34 young people who had been sexually abused as children 6 or 8 years after the abuse had occurred and compared them with 34 control subjects who had not been abused. They also compared subjects who had been abused for less than 1 year with those who had been abused for more than 1 year. The findings suggest a link between childhood sexual abuse and later drug abuse, juvenile delinquency, and criminal behavior. The authors explore the effects of pretrauma factors of previous childhood physical abuse and parental modeling of aggression and the postdisclosure factors of s

IgG Glycosylation Patterns May Identify Distinct Clinical Trajectories in Fibromyalgia Patients: A Longitudinal Pilot Study.

BACKGROUND: Altered immunoglobulin G (IgG) glycosylation was implicated in various conditions, but its role in fibromyalgia (FM) remains unclear. This study investigated IgG glycosylation patterns in FM patients and associations between glycosylation changes and clinical profiles. METHODS: We recruited 30 FM patients and 30 healthy controls. Sleep quality (Pittsburgh Sleep Quality Index, PSQI), anxiety (Beck Anxiety Inventory, BAI), depression (Beck Depression Inventory, BDI), widespread pain (Widespread Pain Index), symptom severity (Symptom Severity Scale), functional impact (FM impact ques

Multimedia tablet or paper handout to supplement counseling during preterm birth hospitalization: a randomized controlled trial.

BACKGROUND: Parents of premature infants engage in shared decision-making regarding the care of their infant. The process of prenatal counseling typically involves a verbal conversation with a neonatal provider during hospitalization. Support people may not be available, and the pregnant person's memory is impaired by medications, pain, and stress. The American Academy of Pediatrics, American College of Obstetricians and Gynecologists, and the Eunice Kennedy Shriver National Institute of Child Health and Human Development have called for improvements to this process, including the development

Depression, anxiety, and clinical history in Spanish-speaking American patients with psychogenic nonepileptic seizures (PNES) compared with Spanish-speaking American patients with epilepsy.

OBJECTIVE: The objective of this study was to compare Spanish-speaking American patients with epilepsy to Spanish-speaking American patients with psychogenic nonepileptic seizures (PNES) on depression, anxiety, and other clinical variables. BACKGROUND: Research on Spanish-speaking American patients with epilepsy or PNES is relatively infrequent, with only a few studies on psychopathology in these two patient groups. Studies of English-speaking patients indicate that those with PNES present with greater depression and anxiety and report poorer quality of life (QOL) when compared with persons w

Validation of depression screening scales in patients with CKD.

BACKGROUND: Depressive symptoms, assessed by using self-report scales, are present at a striking rate of 45% in patients with chronic kidney disease (CKD) at dialysis therapy initiation. These scales may emphasize somatic symptoms of anorexia, sleep disturbance, and fatigue, which may coexist with chronic disease symptoms and lead to overestimation of depression diagnosis. No study has validated these scales in patients with CKD before dialysis therapy initiation. STUDY DESIGN: We conducted a diagnostic test study in participants with CKD to investigate the screening characteristics of 2 depr

The Role of Biofeedback in Improving Vertigo Management in Older Adults. A Controlled Clinical Study in Italy.

INTRODUCTION: Vertigo significantly impacts the psychosocial functioning in older persons, limiting their ability to carry out daily activities. In older adults, the effects of vertigo are even more severe than in younger populations, as it negatively impacts gait and balance, increasing the risk of falls. This, in turn, has a profound effect on their independence and overall quality of life. Despite the importance of providing integrated treatment, only a few psychological interventions have been proposed for older people. The aim of this study is to evaluate the effectiveness of biofeedback

Symptomatic and functional recovery in depression in later life.

OBJECTIVES: Functional limitations give an indication of the total impact of diseases, such as depression, on individuals health and recovery. This study examines the change in several domains of functioning over 2 years in older persons depressed at baseline (non-remitted group and remitted group after 2 years) and in a non-depressed comparison group. METHODS: Data were used from a cohort study (Netherlands Study of Depression in Older persons [NESDO]) consisting of depressed older persons ≥ 60 years (N = 378) and a non-depressed comparison group (N = 132) with 2 years of follow-up (attritio

Atypical sleep architecture and the autism phenotype.

A growing body of evidence indicates that people with autism frequently experience sleep disorders and exhibit atypical sleep architecture. In order to establish whether sleep disorders truly belong to the autism spectrum disorder (ASD) phenotype, we conducted a subjective and objective study of sleep in a group of high-functioning adults with ASD but without sleep complaints, psychiatric disorders or neurological comorbidity. We compared the subjective data of 27 ASD participants with those of 78 healthy controls matched for chronological age and gender. Subjective measures of sleep in the cl

Divergent Patterns of Speaker-Intention Discrimination and Emotional Valence Ratings in Self-Reported Autistic and Non-Autistic Adults.

Autistic adults often interpret verbal and non-verbal cues underlying non-literal language differently compared to non-autistic people, reflecting differences in communicative style and shared context. Research on non-literal language in autism in controlled settings has typically depended on static stimuli that lack ecological validity and primarily focused on children, leaving non-literal language processing in autistic adults relatively understudied. The present study examined how self-reported autistic and non-autistic adults infer speaker intentions and emotional valence in dynamic, natur
