Contemporary global epidemiology of cirrhosis.

Cirrhosis remains a major global cause of morbidity and mortality, with rapidly evolving etiologic drivers. This review summarizes the contemporary global epidemiology of cirrhosis, highlighting regional patterns and major transitions in underlying causes to inform prevention and policy. In 2021, cirrhosis and chronic liver disease accounted for ~1.4 million deaths worldwide. Although age-standardized mortality rates have declined in many regions, the absolute burden continues to rise due to population growth and aging. Reductions in hepatitis B and C-related cirrhosis, driven by vaccination,

Dengue and aging: challenges and opportunities in prevention and care. A narrative review.

Dengue infection is a global health issue with significantly increased incidence and overall burden, especially since 2024. Specifically, epidemiological trends show a rising median age of affected individuals over 65 years/old. Older individuals face increased risks of severe disease, extended hospital stays, healthcare-associated infections, and higher mortality rates, mainly due to a decline in immune function, and multimorbidity. Antibody-dependent enhancement, cytokine dysregulation, and endothelial dysfunction exacerbate disease severity. Moreover, in older patients, dengue diagnosis can

The Psychological Burden of Neuromuscular Diseases: A Narrative Review of Anxiety, Depression, Coping, and Quality of Life.

Neuromuscular diseases (NMDs) include a heterogeneous group of progressive chronic conditions that frequently lead to substantial physical disability and loss of autonomy. Although motor and functional impairments of NMDs are well documented, the psychological burden remains underexplored. This narrative review synthesizes current literature regarding four psychological domains in individuals with NMDs: (i) anxiety, (ii) depression, (iii) coping strategies, and (iv) quality of life. Evidence indicates that anxiety and depressive symptoms are highly prevalent in the spectrum of NMDs, influenced

From pixels to prognosis: radiomics and AI in Alzheimer's disease management.

Alzheimer's disease (AD), the leading cause of dementia, poses a growing global health challenge due to an aging population. Early and accurate diagnosis is essential for optimizing treatment and management, yet traditional diagnostic methods often fall short in addressing the complexity of AD pathology. Recent advancements in radiomics and artificial intelligence (AI) offer novel solutions by integrating quantitative imaging features and machine learning algorithms to enhance diagnostic and prognostic precision. This review explores the application of radiomics and AI in AD, focusing on key i

Unmet health care needs for patients with thyroid disease including thyroid cancer among Bangladeshi population.

We aimed to identify the unmet health care needs of and proposed potential solutions for patients with thyroid diseases, including those with thyroid cancer, among the Bangladeshi population. We performed a qualitative study using structured interviews with healthcare professionals involved in care management and surgery for thyroid diseases. The interviews were conducted in four hospitals across three large cities in Bangladesh, with participants gathered through convenience sampling. Key findings were categorised and summarised into themes by topic, such as challenges, barriers, unmet needs,

Typologies of women who have sexually offended: exploring psychosocial, developmental and motivational characteristics.

Typologies classify persons based on characteristics and motivation of offending and inform clinical treatment, and management of, people who commit acts of sexual harm. Research examining typologies of women who have sexually offended (WWSO) is limited. We address this gap by examining court sentencing decisions for WWSO against children and adults in New Zealand, Australia and Canada (N = 62) to formulate typologies. We consider a broad range of individual and developmental factors, including offender background and psychosocial factors (e.g. experience of sexual abuse, adverse childhood exp

Expert consensus on the clinical management of HIV late presentation.

With the advancement of combination antiretroviral therapy (cART), the global number of new human immunodeficiency virus (HIV) infections and HIV-related mortality has significantly declined. However, the high proportion of "late presentation" (defined as presenting to care with a low CD4 cell count or AIDS-defining events) among HIV-infected individuals remains a significant challenge in HIV prevention and treatment. HIV late presentation is associated with increased clinical risk, complex management, and higher risk of transmission, and represents a significant barrier to achieving the "95-9

The Risk of the Aggravation of Diabetic Foot According to Air Quality Factors in the Republic of Korea: A Nationwide Population-Based Study.

This study aims to examine the association between the occurrence of diabetic foot and air quality (SO2, CO, NO2, O3). Open data were collected to conduct a big data study. Patient information was gathered from the National Health Insurance Service, and the National Institute of Environmental Science's air quality data were used. A total study population of 347,543 cases were reviewed (case = 13,353, control = 334,190). The lag period from air quality changes to the actual amputation operation was calculated for each factor. The frequency of diabetic foot amputation in each region was identifi

Calciphylaxis and Intractable Pain in a Veteran with Psychological Trauma History.

Guidelines are lacking for patients with calciphylaxis on renal replacement therapy, often leading to difficulty optimally treating these patients. A 60-year-old male veteran receiving hemodialysis presented with calciphylaxis of the left lower extremity and intractable pain. His condition was complicated by chronic back pain, long-term opioid therapy, and psychological trauma history. He was ultimately transferred to a calciphylaxis treatment center but was unable to tolerate further treatments due to sepsis and hemodynamic instability. He was transitioned to comfort measures and died in the

Best practice guidelines for evaluating patients in custody in the emergency department.

Patients in custody due to arrest or incarceration are a vulnerable population that present a unique ethical and logistical challenge for emergency physicians (EPs). People incarcerated in the United States have a constitutional right to health care. When caring for these patients, EPs must balance their ethical obligations to the patient with security and safety concerns. They should refer to their institutional policy for guidance and their local, state, and federal laws, when applicable. Hospital legal counsel and risk management also can be helpful resources. EPs should communicate early a

Acromegaly and COVID-19, lessons, and new opportunities.

The COVID-19 pandemic created challenges in effective management of patients with acromegaly. Specifically, with regards to timely diagnosis, delays in surgeries, and disruption(s) to routine patient care. A transition to telemedicine did help to overcome safety restrictions that were placed on in-person care. Creation of surgical safety protocols in conjunction with widespread testing for COVID-19 has also helped with the resumption of pituitary surgery cases. However, acromegaly related comorbidities including cardiovascular disease, diabetes mellitus, sleep apnea and respiratory disease, ve

Challenges of HIV Management in an Aging Population.

PURPOSE OF REVIEW: Potent, well tolerated and simple to administer antiretroviral therapy (ART) has resulted in significant improvement in life expectancy for people with HIV. The increased lifespan does not necessarily equate to improved healthspan with increased rates of comorbidities, frailty and geriatric syndrome experienced by older people with HIV. This review explores the challenges in prevention and management of multimorbidity and geriatric syndrome with the ultimate goal of improving health and quality of life through holistic care. RECENT FINDINGS: Recent studies have drawn attent

Historical Assessment and Mapping of Human Plague, Kazakhstan, 1926-2003.

Understanding Kazakhstan's plague history is crucial for early warning and effective health disaster management. We used descriptive-analytical methods to analyze spatial data for human cases in natural plague foci in Kazakhstan during 1926-2003. The findings revealed 565 human cases across 82 outbreaks in Almaty (32.22%), Aktobe (1.59%), Atyrau (4.42%), Mangystau (21.24%), and Kyzylorda (40.53%) oblasts. Before antibiotic drugs were introduced in 1947-1948, major plague outbreaks occurred in 1926, 1929, 1945, 1947, and 1948, constituting 80.7% of human transmission. Plague spread through flea

Understanding cognitive frailty in aging adults: prevalence, risk factors, pathogenesis and non-pharmacological interventions.

The worldwide increase in the aged population raises health concerns for elderly individuals. Cognitive frailty of the elderly (apart from those suffering from Alzheimer´s disease or other type of dementia) is a complex construct associated with aging, which is composed of physical and cognitive components, while physical frailty and cognitive impairment mutually affect each other. Although the prevalence of cognitive frailty in community-dwelling older adults without neurodegenerative disease is low, it can rise dramatically in clinical settings. Early identification of this condition can con

Artificial intelligence in retinal disease: clinical application, challenges, and future directions.

Retinal diseases are a leading cause of blindness in developed countries, accounting for the largest share of visually impaired children, working-age adults (inherited retinal disease), and elderly individuals (age-related macular degeneration). These conditions need specialised clinicians to interpret multimodal retinal imaging, with diagnosis and intervention potentially delayed. With an increasing and ageing population, this is becoming a global health priority. One solution is the development of artificial intelligence (AI) software to facilitate rapid data processing. Herein, we review re

Sleep Disturbances in Neurological Disease: A Target for Intervention.

Sleep is a biological function required for neurological and general health, but a significant and under-recognized proportion of the population has disturbed sleep. Here, we briefly overview the biology of sleep, sleep requirements over the lifespan, and common sleep disorders. We then turn our attention to five neurological diseases that significantly contribute to global disease burden and neurology practice makeup: epilepsy, headache, ischemic stroke, Parkinson's disease, and Alzheimer's disease. For each disease, we review evidence that sleep disturbances contribute to disease risk and se

Updated Estimates and Mapping for Prevalence of Chagas Disease among Adults, United States.

We combined American Community Survey data with age-specific Trypanosoma cruzi prevalence derived from US surveys and World Health Organization reports to yield estimates of Chagas disease in the United States, which we mapped at the local level. In addition, we used blood donor data to estimate the relative prevalence of autochthonous T. cruzi infection. Our estimates indicate that 288,000 infected persons, including 57,000 Chagas cardiomyopathy patients and 43,000 infected reproductive-age women, currently live in the United States; 22-108 congenital infections occur annually. We estimated ≈

Predictors of varying levels of risks posed by fixated individuals to British public figures.

Concerning approaches and communications to the Royal Family and other British public figures are relatively numerous. This paper examines over 2000 such cases logged over a three-year period in the United Kingdom. Using police and health data, the paper conducts a series of bivariate and multivariate analyses to demonstrate the predictors of what types of risk are posed by an individual case (e.g., communicate only, approach, security breach). The results showed that (a) the rates of serious mental disorders are higher among this sample than the general population base rate, (b) approachers w
