Abnormalities in the awareness and control of action.

Much of the functioning of the motor system occurs without awareness. Nevertheless, we are aware of some aspects of the current state of the system and we can prepare and make movements in the imagination. These mental representations of the actual and possible states of the system are based on two sources: sensory signals from skin and muscles, and the stream of motor commands that have been issued to the system. Damage to the neural substrates of the motor system can lead to abnormalities in the awareness of action as well as defects in the control of action. We provide a framework for under

Cognitive-behavioural therapy for psychosis: a status report.

OBJECTIVE: To review the nature of cognitive-behavioural interventions for psychosis and to evaluate evidence of their effectiveness. METHOD: Electronic (Medline and Psychinfo) and bibliography-based searches were carried out to locate descriptions and evaluations of cognitive-behavioural interventions for psychosis. RESULTS: Various cognitive-behavioural interventions have been used for reducing psychosis. These have usually been applied to auditory hallucinations and/or delusions in otherwise treatment-resistant patients. Most evaluations comprise case studies or simple pre-post designs. C

Voice-overs.

Hearing voices is distressing and frightening, but cognitive psychology can provide a range of interventions that enable people to live with their auditory hallucinations. Liam Clarke looks at ways to empower this patient group.

Misidentified political figures: an underappreciated danger.

A series of twelve patients is presented in which each patient suffered from one or more misidentification syndromes and also misidentified one or more political figures. The fact that misidentification syndromes have been associated with physical violence and that the majority of the patients studied had a history of physical violence suggests that these individuals could pose a significant danger of physical harm to others, including political figures. Persons who threaten political figures should be evaluated for misidentification syndromes.

Psychiatric implications of altered limbic-hypothalamic-pituitary-adrenocortical activity.

Hormones of the limbic-hypothalamic-pituitary-adrenocortical (LHPA) system are much involved in central nervous system regulation. The major LHPA neuropeptides, corticotropin-releasing hormone (CRH), vasopressin (AVP) and corticotropin (ACTH) do not only coordinate the neuroendocrine response to stress, but also induce behavioral adaptation. Transcription and post-translational processing of these neuropeptides is regulated by corticosteroids secreted from the adrenal cortex after stimulation by ACTH and other proopiomelanocortin derived peptides. These steroids play a key role as regulators o

Atypical psychosis with disseminated subpial demyelination.

A 34-year-old woman experienced three episodes of an atypical psychosis, characterized by confusion, agitation, delusional thinking, paranoid ideation, and auditory hallucinations, during the 14 months prior to her death. Findings of gross examination of the brain and spinal cord were unremarkable. Histologic examination revealed scattered subpial foci of demyelination throughout the brain stem, with involvement of the hippocampal formation bilaterally. Although occasional active lesions at early stages of development were noted, most lesions were gliotic and therefore quiescent. This case and

Treatment of resistant lithium-induced nephrogenic diabetes insipidus and schizoaffective psychosis with carbamazepine.

A man with resistant lithium-induced nephrogenic diabetes insipidus and affective psychosis was successfully treated with carbamazepine. Carbamazepine may be appropriate for concurrently treating lithium-induced renal dysfunction and manic-depression in patients whose prognosis may be jeopardized by lithium discontinuation.

Using ECT to study hemispheric specialization for sequential processes.

Two studies are reported, citing the effects of unilateral versus bilateral electroconvulsive shock therapy on the performance of short cognitive tests. In Study 1, seven tets were administered before and after three treatment sessions. The most interesting finding was that performance improved significantly on the Knox Cube Imitation test following right unilateral shock. Study 2 replicated this unexpected finding. These results were interpreted as demonstrating that the left cerebral hemisphere is specialized for the processing of sequential time-dependent information.
