Comparison of postoperative bupivacaine with lidocaine on pain and analgesic use following periodontal surgery.

The purpose of this study was to compare postoperative administration of bupivacaine, a long-acting local anesthetic, with lidocaine, a short-acting local anesthetic, on pain perception and analgesic use following periodontal surgery. Ten male subjects were selected on the basis of having similar bilateral mandibular quadrants with moderate to severe periodontal disease requiring osseous surgery. The study was a matched-pair, double-blind design. Carpules of 2% xylocaine with 1:100,000 epinephrine and 0.5% bupivacaine with 1:200,000 epinephrine were wrapped in opaque tape and placed in separat

Knockdown of the Non-canonical Wnt Gene Prickle2 Leads to Cerebellar Purkinje Cell Abnormalities While Cerebellar-Mediated Behaviors Remain Intact.

Autism spectrum disorders (ASD) involve brain wide abnormalities that contribute to a constellation of symptoms including behavioral inflexibility, cognitive dysfunction, learning impairments, altered social interactions, and perceptive time difficulties. Although a single genetic variation does not cause ASD, genetic variations such as one involving a non-canonical Wnt signaling gene, Prickle2, has been found in individuals with ASD. Previous work looking into phenotypes of Prickle2 knock-out (Prickle2-/-) and heterozygous mice (Prickle2-/+) suggest patterns of behavior similar to individuals

Sleep and stress in the acute phase of concussion in youth.

This study sought to address the complex interplay between both biological and psychological perceptions of stress and sleep in the acute stages following a mild traumatic brain injury. A secondary goal was to identify potential targets for intervention. Eleven acutely injured youth (mean age 12 years) were studied at home with overnight actigraphy, salivary cortisol and melatonin assays, and subjective ratings of stress and fatigue (injured group). Nine matched control youth also were assessed (control group). Results suggested longer sleep latencies (time to fall asleep) and higher levels of

Perceptual-learning evidence for separate processing of asynchrony and order tasks.

Normal perception depends, in part, on accurate judgments of the temporal relationships between sensory events. Two such relative-timing skills are the ability to detect stimulus asynchrony and to discriminate stimulus order. Here we investigated the neural processes contributing to the performance of auditory asynchrony and order tasks in humans, using a perceptual-learning paradigm. In each of two parallel experiments, we tested listeners on a pretest and a posttest consisting of auditory relative-timing conditions. Between these two tests, we trained a subset of listeners approximately 1 h/

Knowledge of visual attributes in the right hemisphere.

Neurobiological theories of knowledge processing are biased toward the language-dominant (usually the left) hemisphere. Does the right hemisphere critically contribute to knowledge processing? J.A. is a left-hemisphere language-dominant individual who suffered a lesion confined to the right mid- and anterior fusiform gyrus. Although her language abilities are intact, she showed a partial loss of knowledge of the visual attributes of biological and nonbiological entities. This was observed regardless of the task performed: object discrimination, oral feature generation, forced-choice naming-to-

Longitudinal Mechanism-of-Disease Mapping to Promote Integration Across Disciplines in Preclinical Medical Education.

To improve integration of basic science learning and clinical reasoning in preclinical medical education, we developed a longitudinal, case-based activity spanning physiology, immunology, and respiratory pathophysiology. Student teams created mechanism-of-disease maps and illness scripts for respiratory disease cases with increasing complexity across courses. Integration was reinforced through iterative refinement and explicit linkage to previously learned concepts. The activity was feasible to implement across courses and did not negatively impact student performance on traditional assessment

Addressing therapist concerns about religion in cognitive behavioural therapy: why, when, and how?

Despite religion being an important part of some patients' identity and value systems, therapists report finding it challenging to discuss religion in therapy. Avoiding religion for these patients can have detrimental effects on the therapeutic alliance, treatment outcomes, and parity of access for minoritised groups. Therefore, this clinical guidance paper aims to support therapists to bring religion into the therapy room by addressing six key concerns: (1) difficulties raising the topic, (2) 'getting it wrong' when discussing religion, (3) therapist and patient differences, (4) managing nega

Power, Ideology, and Institutional Betrayal: The Intensive Care Unit Experiences of Bereaved Families during COVID-19.

In this qualitative study conducted in Israel, the authors explored the experiences of family members (N = 19) who lost a loved one to COVID-19 following hospitalization in intensive care units (ICUs). In particular, the authors examined how bereaved individuals perceived their interactions with ICU staff, institutional policies, and power dynamics within healthcare settings. Semistructured interviews with the 19 participants revealed recurring themes of struggle, perceived injustice in medical decision making, enforced patient isolation, and a resulting erosion of trust in the healthcare syst

Patient demographic differences in patient-reported outcomes of breast reconstruction: A comparative analysis of BREAST-Q.

Advances in breast cancer care have improved survival, making it vital to assess long-term patient perceptions and well-being post-breast reconstruction. Although evaluating patient-reported outcomes (PRO's) has become increasingly crucial to breast reconstruction care, differences among racial groups have not been studied extensively. A prospective cohort study including BREAST-Q survey results of 234 patient after breast reconstruction found significant differences between racial groups for distinct outcome categories of "Confidence," "Sensitivity," and "Sexual Well-being" across various tim

Effective communications to support risk-adapted cervical screening in human papillomavirus-vaccinated women: lessons learned and ways forward.

As decisions are made worldwide about the best way to adapt cervical screening to cohorts and individuals who have been offered human papillomavirus (HPV) vaccination, we consider the issues this raises for communication. This article reviews the relevant literature on knowledge and awareness of HPV, lessons we can learn from emerging evidence on attitudes to risk-adapted cancer screening, and what the psychological literature tells us about the complex relationships between risk perceptions and behavior. We highlight the need to ensure communication strategies do not create or widen social in

Mapping brain neural networks in stress brain connectivity.

Stress can cause severe damage to the CNS and contribute to an increased risk of neurological and psychiatric disorders. Gaining more insight into the neurobiology of stress is essential to treating neurological disorders associated with stress, which account for a high percentage of the world's disease burden. However, because of complicated variations in stressor types, stress perception, and preceding exposure to stressors, studying the impacts of stress is challenging. Gender, age, and timing are other crucial variables that can influence the stress response. Behavioral, physiological, gen

Endogenous endophthalmitis caused by Rothia dentocariosa.

We report a case of a late adolescent female who presented to us with complaints of acute, painful loss of vision in her left eye. Her visual acuity (VA) had reduced to perception of hand movements within a week. Slit lamp examination and ultrasonography revealed that the patient had endophthalmitis. She was treated with intravitreal antibiotics, pars plana vitrectomy, lensectomy and a 5000 centistoke silicone oil injection. The vitreous biopsy showed significant growth of Rothia dentocariosa on blood and chocolate agars. An infected lower mandibular tooth was identified as the infective focus

The Spatial Organization of Ascending Auditory Pathway Microstructural Maturation From Infancy Through Adolescence Using a Novel Fiber Tracking Approach.

Auditory perception is established through experience-dependent stimuli exposure during sensitive developmental periods; however, little is known regarding the structural development of the central auditory pathway in humans. The present study characterized the regional developmental trajectories of the ascending auditory pathway from the brainstem to the auditory cortex from infancy through adolescence using a novel diffusion MRI-based tractography approach and along-tract analyses. We used diffusion tensor imaging (DTI) and neurite orientation dispersion and density imaging (NODDI) to quanti
