Schisis contributes to necrosis in experimental HSV-1 retinitis.

Injection of the KOS strain of herpes simplex virus type 1 into the anterior chamber of one eye of a BALB/c mouse produces bilateral retinal inflammation which progresses in the contralateral eye to total retinal necrosis by day 10 post-inoculation. Although both immune and direct viral cytopathic effects have been proposed as causes of the initial infiltration, the precise mechanism(s) of the contralateral retinal necrosis remains uncertain. In this study, histopathological examination of the uninoculated eyes of HSV-1 injected BALB/c mice revealed the presence of vitreous traction against th…

The physical and surgical aspects of silicone oil in the vitreous cavity.

Silicone oil inside the vitreous cavity exerts forces on to the retina as a result of buoyancy, volume displacement, and surface tension. Surface tension rather than viscosity is the key to understanding why the oil seals retinal breaks effectively. The physics of the tamponade was studied quantitatively. Retinal traction can be counteracted by the oil up to a calculated threshold value, depending on the size and shape of the tear, the strength of the surface tension and, most importantly, the distance between the retina and choroid. For a nearly flat retinal hole, the tamponade is very effect…

The posterior surface of the lumbar vertebral bodies. Part I.

The degree of concavity (ie, the scalloping) on the posterior surface of the lumbar vertebral bodies has been evaluated quantitatively by me … c. This pressure will, at the edges of the superior and inferior end-plates, be counteracted by the tractional stresses of the fibers of the discal annulus fibrosus that are inserted at the vertebral margins. This effect contributes to the final shape of the vertebral bodies. The posterior vertebral surface constitutes part of the anterior wall of spinal c …

Is the meniscus of the knee joint a fibrocartilage?

A histological analysis of the structure of intact knee joint menisci was carried out in adult dogs. By means of specific histochemical meth … iscus is accounted for by its functional duality manifested in offering resistance to the forces of traction and pressure, the latter ones favoring the process of evolution of tissue from connective, through chondroid, to cartilaginous.

Interfascicular nerve grafting.

Interfascicular nerve grafting is a useful method to repair nerves with gaps. Good motor recovery can be obtained, and return of some degree of sensibility is possible. The procedure can be used to overcome small gaps that result from neglected sharp lacerations or after failure of primary repair, or large gaps that result from loss of nerve substance or traction lesions. Primary nerve repair should be done for acute lacerations. However, in old lacerations (more than three weeks old) and in nerve gaps of more than 2 cm in length, a functional recovery can still be expected after nerve graftin…

Congenital dislocation of the hip. Review at maturity of 147 hips treated by excision of the limbus and derotation osteotomy.

In 1957 Somerville and Scott presented their principles of management for the older child with congenital hip dislocation. They advocated preliminary traction followed, in those hips which remained dislocated, by excision of the limbus and subsequent derotation varus osteotomy of the femur. As alternative regimes are advocated it becomes increasingly important to subject each method to detailed long-term review. One hundred and forty-seven hips in 121 patients aged between 12 months and three years and treated by the standard Somerville and Scott regime have been reviewed. The age at review ra…

Nuclear cataract as a cause of senile glaucoma.

When radial cortical opacities occurred in the lenses of glaucomatous eyes, ocular hypertension soon became normal. Glaucoma operations were not performed on patients over 50 years of age, and medical control or simple lens extraction was adopted. In 86% of the lenses extracted from glaucomatous eyes no medication was necessary to keep normal ocular tension for two years postoperatively. The water content of extracted lens from glaucomatous eyes was always in the lower half compared to water content of nonglaucomatous cataractous lenses. Thus the absence of radial cortical opacities is a cause…

Penetrating eye injuries: a histopathological review.

This study reports the histological findings in human eyes after severe penetrating trauma. The findings confirm the high incidence of retinal detachment in eyes with severe penetrating injuries. The retina was detached in 32 out of the 34 eyes examined, with 27 having evidence of traction on the retina. These eyes were characterized histologically by intraocular cellular proliferation producing cyclitic, epiretinal, and retroretinal membranes. Intraocular cellular proliferation was discernible or established within 1 week of injury and typically resulted in a cyclitic membrane at about 6 week…

Progression in idiopathic scoliosis: A preliminary report of a possible mechanism.

Recent surveys have shown that idiopathic structural scoliosis of mild degree is generally not progressive. We will propose a mechanism which may be responsible for deterioration in the few. It has been observed that the spinal cord, although displaced towards the concavity, does not rotate in company with the vertebrae, thus exposing the emerging nerve roots to the effects of traction and possibly of entrapment. We suggest that progression occurs when the neuraxis is unable to adjust to the change in the anatomy of vertebral column. Our proposition is based upon our findings in a complete spi…

Palliative treatment of complete transposition of the great vessels: First clinical results of closed atrial septectomy with new septectome.

Our initial method of closed atrial septectomy, used in the palliative therapy of transposition of the great arteries (TGA), failed to produce a technical success in 1 of 5 patients. Subsequently, we modified the technique by securing the septum on traction sutures and simplifying the introduction of the septectome. With the new method, we achieved technical success in all 13 patients so treated. In our total of 18 patients treated by closed atrial septectomy, 7 had TGA with no other cardiac anomaly, and none of the 7 died. Of the remaining 11 patients who had TGA and other cardiac anomalies a…
