A case of meningitis treated with intraventricular vancomycin in an infant.

There is limited evidence for vancomycin (VCM) ventriculoperitoneal (VP) administration. We report a 3-month-old female with congenital hydrocephalus who developed a fever and was admitted to the hospital with a suspected VP shunt infection; she was being managed as an outpatient following VP shunt placement. Methicillin-resistant Staphylococcus aureus was detected on the removed catheter tip, and VCM treatment was initiated. Intravenous administration was ineffective, so intraventricular administration was initiated. Therapeutic drug monitoring (TDM) of cerebrospinal fluid (CSF) drug concentr

Actinotignum schaalii infection: Challenges in diagnosis and treatment.

Actinotignum schaalii affects elderly people and is associated with individuals with urological-related predispositions, but can be found in a variety of locations, such as cutaneous, intraabdominal, genitourinary and surgical infections. Disseminated infections occur less frequently and are by and large related to urinary tract colonisation. This pathogen is often neglected due to growth requirements, especially in urinary tract infections. We present 107 Actinotignum schaalii isolated from genitourinary samples (80.4%), from skin and soft tissue infections (13.1%), from bone and deep tissue

Tigecycline Containing Polymethylmethacrylate Cement Against MRSA, VRE, and ESBL-In Vitro Mechanical and Microbiological Investigations.

BACKGROUND: The use of antibiotic-loaded bone cements (ALBCs) in arthroplasty has been well established for the prevention and treatment of infections. Tigecycline (Tig), a broad-spectrum antibiotic, has shown efficacy against various pathogens, including vancomycin-resistant strains. METHOD: ISO and DIN mechanical and microbiological inhibition zone tests were performed on PMMA cement with manually added Tigecycline. RESULTS: Manually adding 0.5 and 1.0 g Tigecycline to PMMA always meets the mechanical requirements of ISO and DIN standards. Mixtures containing 0.5 g were microbiologically e

Vancomycin-Induced Neutropenia With Subsequent Perioperative Readministration: A Case Report.

Vancomycin (VCM), an essential antibiotic for antimicrobial-resistant Gram-positive cocci, can lead to complications such as neutropenia. Here, we present a case of a 25-year-old male with noncommunicating hydrocephalus due to an intraventricular tumor who developed neutropenia during VCM therapy. Despite the suspected VCM-induced neutropenia, short-term readministration was deemed necessary for perioperative infection prophylaxis. This patient was readministered without neutropenia. A review of the literature revealed an earlier onset of VCM-induced neutropenia than that previously reported,

Daptomycin and ceftaroline combination for the treatment of persistent methicillin-resistant Staphylococcus aureus bloodstream infections: a case series and literature review.

Methicillin-resistant Staphylococcus aureus (MRSA) is challenging to treat due to a lack of guidance for clinicians. The daptomycin and ceftaroline combination is promising for treating persistent MRSA bloodstream infections (BSIs). In this report, we present a case series of 7 patients who failed vancomycin and then were treated with daptomycin and ceftaroline for persistent MRSA BSIs. The median age (IQR) of the included patients was 59 (48-67), with 5 male and 2 female patients. Six patients (85.7%) had a clinical cure for their persistent BSIs. The median time (IQR) for sterilization of MR

Multi-Drug-Resistant Elizabethkingia meningoseptica: A Rare Cause of Late-Onset Sepsis in a Preterm Neonate.

Elizabethkingia meningoseptica is a gram-negative bacillus and is a rare cause of opportunistic infections. Literature shows that this gram-negative bacillus may cause early-onset sepsis in neonates and immunocompromised adults; however, it is a rare cause of late-onset sepsis or meningitis in neonates. We hereby delineate a case of a preterm neonate, born at 35 weeks of gestation, presenting to us on the eleventh day after birth, with fever, tachycardia, and delayed reflexes. The neonate was managed in the neonatal intensive care unit (NICU). Initial laboratory tests, blood, and cerebrospinal

Isolation and identification of Streptococcus agalactiae in cage-cultured green chromide Etroplus suratensis in Kerala, India.

Streptococcus agalactiae is one of the main aetiological agents in large-scale mortalities of tilapia, having caused major economic losses to the aquaculture industry in recent years. This study describes the isolation and identification of the bacteria from cage-cultured Etroplus suratensis that experienced moderate to severe mortalities in Kerala, India. Gram-positive, catalase-negative S. agalactiae was identified from brain, eye and liver of the fish by antigen grouping and 16S rDNA sequencing. Multiplex PCR confirmed that the isolate belonged to capsular serotype Ia. Antibiotic susceptibi

Daptomycin in combination with rosuvastatin induced blood creatine phosphokinase elevation.

We present the case of a 73-year-old male patient who was hospitalised with infective endocarditis, and report an elevation in his blood creatine phosphokinase (CPK) after receiving daptomycin and rosuvastatin therapy concomitantly. His previous home-scheduled medications included apixaban, ivabradine, metformin, rosuvastatin 20 mg, ginkgo biloba and trimetazidine, and he continued to receive these medications at the hospital. After three sets of blood cultures were taken, empirical treatment was started with vancomycin and gentamicin. On the eighth day of treatment, daptomycin and ampicillin-

Forty years of peritoneal dialysis Listeria peritonitis: Case and review.

Infections with Listeria monocytogenes (LM) are very uncommon and severe especially in immunocompromised people. We report a continuous cycling peritoneal dialysis (CCPD) patient who presented initially disseminated listeriosis with peritonitis. He was successfully treated with intraperitoneal and intravenous ampicillin but died unfortunately from a cardiorespiratory arrest due to food inhalation. It is the 20th case of such peritonitis mentioned among PD patients and the first reported in Belgium. This case illustrates the importance of a systematic approach to get quick diagnosis and effecti

Combined bacteriophages and antibiotics as an efficient therapy against VRE Enterococcus faecalis in a mouse model.

Clinical applications of bacteriophage therapy have been recently gathering significant attention worldwide, used mostly as rescue therapy in cases of near-fatal antibiotic failure. Thus, clinically relevant in-vivo models presenting both short- and long-term implications of phage therapy given as rescue treatment for fulminant infections are of highest importance. In this study, a cocktail consisting of two lytic bacteriophages was used to evaluate the therapeutic efficacy of phage therapy as a rescue treatment for severe septic peritonitis in a mouse model. We established that a single injec

Evaluation of rapid mecA gene detection versus standard culture in staphylococcal chronic prosthetic joint infections.

In case of periprosthetic joint infections, the antibiotic treatment administered intraoperatively entails consequences on bacterial ecology with potential secondary effects. This study evaluates the rapid detection of methicillin-resistant staphylococci (MRS) by Xpert® technology directly on intraoperative samples. Xpert® technology was compared to conventional culture for 104 clinical specimens performed on 30 patients. The performance of the test expressed in terms of sensitivity, specificity, positive predictive value, and negative predictive value was, respectively, 87.1%, 100%, 100%, and

Prostatic abscess due to community-acquired methicillin-resistant Staphylococcus aureus.

We report what we believe to be the second case of a prostatic abscess due to community-acquired methicillin-resistant Staphylococcus aureus (MRSA). A previously healthy diabetic man presented with dysuria, fatigue, weight loss, a tender prostate, and leukocytosis. Computerized tomography of the abdomen and pelvis demonstrated a large prostatic abscess at the base of the bladder. Blood, urine, and pus obtained by percutaneous aspiration grew MRSA. Percutaneous drainage and prolonged therapy with intravenous vancomycin resulted in cure. Prostatic abscess is most often caused by Gram-negative or

Clinical experience with linezolid for the treatment of central nervous system infections.

Linezolid, an oxazolidinone, exhibits bacteriostatic activity against virtually all Gram-positive bacteria and even covers atypical organisms like mycobacteria and Nocardia. However, little is known about its effectiveness for central nervous system (CNS) infections. We report on our good experience with linezolid for the treatment of CNS infections in 10 patients, amongst whom three were caused by mycobacteria. While six of our patients clinically improved during linezolid therapy even after failure of various antibiotics, it was unsuccessful in one case. Side-effects were only mild gastroint

[Intravenous vegetation of methicillin-resistant Staphylococcus aureus induced by central venous catheter in a patient with implantable cardioverter-defibrillator: a case report].

A 54-year-old woman with an implantable cardioverter-defibrillator was admitted because of convulsion. Previously, she had suffered methicillin-resistant Staphylococcus aureus (MRSA) sepsis induced by a central venous catheter. Despite removal of the central venous catheter and administration of vancomycin, fever and increased C-reactive protein level persisted. MRSA had probably infected the implantable cardioverter-defibrillator system. Gallium scintigraphy showed accumulation at the right subclaviculan area, although local inflammatory signs were absent. Intravascular ultrasonography demons

Emerging infectious diseases: a challenge to all.

Emerging infections are defined as diseases of infectious origin with an incidence that has increased within the past two decades or threatens to increase in the near future. Some of these diseases are associated with newly discovered infectious agents; others are well-known conditions rapidly increasing in incidence. Five emerging infections are reviewed in this article: ehrlichiosis, a tick-borne infection caused by obligate intraleukocytic bacteria; infections caused by vancomycin-resistant enterococci, which have become a serious nosocomial problem; hantavirus pulmonary syndrome, a Sin Nom

Prosthetic valve endocarditis.

The diagnosis, therapy, and complications of prosthetic valve endocarditis (PVE) in 48 patients seen between 1962 and 1978 are reviewed. Staphylococcus epidermidis and diphtheroids were the most common causes of both early and late PVE. These microorganisms were frequently resistant to the penicillins and cephalosporins but were uniformly sensitive to vancomycin. The mortality rate in this series was 69%, with 20% of the deaths attributed to central nervous system emboli and the remainder to cardiac causes. The mortality rate exceeded 75% in patients with any of the following findings: aortic

Repurposing Azeliragon as a novel antibacterial agent against methicillin-resistant Staphylococcus aureus via combined membrane phospholipids and cell wall targeting.

Methicillin-resistant Staphylococcus aureus (MRSA) poses a persistent clinical threat due to limited therapeutic options and the rapid emergence of resistance. Azeliragon, an orally bioavailable Receptor for Advanced Glycation End-products (RAGE) inhibitor previously tested in human trials, was identified as a potential antibacterial agent against MRSA through high-throughput screening. It exhibited potent antibacterial activity against clinical S. aureus isolates (MIC50 = 6.25 μM/3.3 μg/mL) and significantly inhibited biofilm formation. Metabolomics and proteomics suggested perturbation of li

Occurrence and antimicrobial resistance profile of Listeria monocytogenes isolated from chilled processed meat products in the Metropolitan Region of Recife, Brazil.

The presence of Listeria monocytogenes in food represents a major challenge to food safety, especially in ready-to-eat products stored under refrigeration. This risk is associated with the ability of the pathogen to survive and multiply during prolonged refrigerated storage, making these foods potential sources of infection and a relevant public health concern. In this context, the aim of this study was to investigate the presence of Listeria monocytogenes in chilled processed meat products sold in bulk in the Metropolitan Region of Recife, PE, Brazil, as well as to evaluate the antimicrobial
