A French team investigated hypersensitivity reactions that occurred during locoregional or general anaesthesia over an 8-year period. They estimated that the incidence of anaphylactic reactions was about 1 per 10 000 anaesthetic procedures. Among the 1816 reports of anaphylactic reactions, the most commonly implicated drugs were neuromuscular blocking agents (1067 cases), latex (361 cases), and antibiotics (236 cases). Some anaphylactic reactions to neuromuscular blocking agents occurred in patients who had never previously been anaesthetised, suggesting cross-reactivity with other, poorly kno

Shock is a common reason for medical intensive care unit admission, with septic and cardiogenic accounting for most of the etiologies. However, the potential severity of adverse side effects of drugs indicates that any medication should be carefully scrutinized for potential pharmacokinetic and pharmacodynamic interactions that may result. We herein report the case of a life-threatening shock mimicking successively anaphylactic, cardiogenic, and septic shock, which was finally related to disulfiram ethanol reaction. Indeed, disulfiram ethanol reaction is known to provoke unpleasant symptoms th

Two consecutive hip surgery procedures in the same patient were aborted following a severe anaphylactic reaction of the patient. After the second procedure, the patient was diagnosed to be allergic to chlorhexidine, a component of the gel used to insert a urinary catheter. Chlorhexidine allergy is rare, and the delayed presentation may make it easy to overlook. In hip arthroplasty surgery, this may result in an abandoned procedure and the need for re-operation.

Ecstasy (3,4 methylenedioxymethamphetamine, or MDMA) is a recreational drug widely used among young people in discos or rave parties (1,2). MDMA is taken because it gives a feeling of euphoria, enhances energy and sociability, and heightens sensations and sexual arousal. However, several side effects have been described: headache, nausea, anorexia, xerostomia, insomnia, myalgia, trismus, and bruxism (2,3). More serious complications have also been reported, sometimes even leading to death: hyperthermia, disseminated intravascular coagulopathy, rhabdomyolysis, acute renal failure, liver failure

The pediatrician and scientist, Clemens Freiherr von Pirquet (1874-1929) was the first to coin the term "allergy". His experiments with Behring's serum, which he administered to sick children, led him to an awareness of an interaction between the human organism and pathogens. In his article published in the Müncher Medizinischen Wochenschrift in 1906, he laid the foundation stone for modern allergology.

We describe the case of a 24-year-old healthy volunteer who underwent a dextran-induced anaphylactic/anaphylactoid reaction (DIAR) type III after administration of 10 ml 6% Dextran60 (0.6 g) during a preliminary examination. There were no specific incidents in the medical history or any infusions of any colloids. In contrary to other DIAR case reports of anaphylactic reactions, in this case we observed a latency period after intravenous application of Dextran60 to the first clinical symptoms of anaphylactic shock of about 5 min. The initial decrease of systolic blood pressure to less than 90 m

Ficus benjamina (weeping fig) is a widespread indoor ornamental plant. Allergens of Ficus benjamina are a well-known cause of IgE-mediated respiratory diseases. We treated a 32-year-old female who for 10 years had suffered from perennial rhinoconjunctivitis. When dusting her 2 meter high Ficus benjamina, she developed an anaphylactic reaction which resolved without sequelae. Skin prick testing revealed a strong immediate type reaction to a Ficus extract, the serum concentration of specific IgE-antibodies to Ficus was > 100 kU/I (CAP class 6). In view of these strong test reactions and the conc

Anaphylactic shock is a life-threatening allergic reaction with cardiovascular collapse. The cardiovascular collapse may occur suddenly without warning signs or may be preceded by symptoms such as pruritus, wheezing, dyspnea, urticaria, pallor, digestive symptoms, and weakness. Food allergens, injected drugs and hymenoptera stings are the main etiologies. Anaphylactic shock requires an emergency treatment with immediate intramuscular or subcutaneous epinephrine injection. Subsequent avoidance of the inciting allergens is mandatory together with the availability of a first aid kit including rea

A 41-year-old-man without previous ischemic heart disease, developed a severe anaphylactic reaction. After administration of epinephrine (0.5 mg) the patient complained of chest pain. The electrocardiogram showed an elevation of ST segment in inferior leads. Myocardial necrosis was ruled out. Coronary arteriography disclosed normal coronary arteries. Eight months later, the patient developed severe chest pain during physical activity. ST elevation was again seen in inferior leads. ECG changes disappeared, when sublingual nitroglycerin was administered. A diagnosis of vasospastic angina was mad

BACKGROUND: Children with spina bifida (SB) are exposed to latex soon after birth during bladder catheterization, rectal disimpaction, and multiple surgical procedures. IgE-mediated latex-allergic reactions have been reported recently in these children. Our study was designed to assess the prevalence of allergic reactions to latex products in a group of Saudi Arabian children with SB in a tertiary care hospital.
METHODS: Fifty-nine patients, aged 1-20 years, with SB were evaluated by a questionnaire on type of latex reactions; family and personal history of other allergic disorders, such as a

The effect of a herbal protein, CI-1, purified from the leaves of Cajanus indicus was evaluated as a probable immunomodulator on immune response in control and stressed mice. In mice, sensitized with sheep red blood cells (SRBC), CI-1 enhanced IgG level by 28% over control (P < 0.05). Primary and secondary antibody response to SRBC and bovine serum albumin (BSA) were significantly increased in CI-1 treated group. Furthermore CI-1 facilitated the delayed type of hypersensitivity (DTH) response to SRBC in sensitized mice and also enhanced leucocyte and macrophage migration inhibition response in

This paper presents the case of a non-smoking and non-atopic male, exposed to iridium chloride at work, who developed respiratory tract symptoms and contact urticaria. Application of iridium salt to normal skin caused contact urticaria. An iridium chloride prick test showed a positive reaction and a scratch test produced anaphylactoid reactions. Platinum salt allergy was excluded through prick testing with hexachloroplatinate solution. There is no previous report describing a case where an individual has a positive prick test reaction to iridium salts and simultaneously a negative one to plati

The examination of 11 urticaria pigmentosa (UP) patients included allergological history, skin prick, scarification tests and intracutaneous tests with noninfectious and bee poison allergens, total and specific serum IgE measurements, in vitro reaction of histamine release from peripheral blood basophils induced by bee poison. The response of mastocytosis patients to insect sting was characterized by a rapid (within 5 min) development of severe systemic reactions or shock. The skin reactions and serum antibodies to bee poison were not registered in 9 of 11 patients. They also had a negative re

Of a total of 131 patients suffering from chronic urticaria, 15 were cautiously re-exposed to ASA after an initial provocative exposure during an urticaria test programme 2-11 years before. Only 1 of these patients, who had undergone the initial provocative test 7 years earlier, reacted at the same intensity; 1 other patient reacted with much less intense symptoms 4 years after the original test. Among 3 other patients, who merely reacted to ASA intake with urticarial eruptions and did not suffer from chronic urticaria, only 1 presented 4 years after the initial exposure with oedema of the ski

Systemic allergic reactions to hymenoptera stings are most often mediated by venom-specific IgE antibodies. The most frequent symptoms are urticaria, angioedema, asthma and anaphylactic shock. Rarely, cerebral or myocardial ischemia with permanent damage may result from severe anaphylactic shock. Each year several individuals die in Switzerland from allergic reactions following hymenoptera stings. The most valuable diagnostic tests are skin tests and estimation of venom-specific serum-IgE antibodies. The most important drugs for emergency treatment are adrenaline and antihistamines. During ana

A 30 year old woman bitten by a large Sri Lankan krait (Bungarus ceylonicus) developed progressive paralytic symptoms within one hour of the bite. After seeking traditional treatment her condition deteriorated and when she arrived at hospital 6 h after the bite she was drowsy, with bilateral ptosis and signs of aspiration pneumonia (a complication of traditional treatment). She developed an anaphylactic reaction after antivenom treatment and, despite treatment, had a cardio-respiratory arrest. She was resuscitated and mechanically ventilated, but remained deeply unconscious until her death 90

Anaphylactic shock as a result of trauma is very rare. We describe a 20-year-old Druze soldier who presented with anaphylactic shock due to rupture of a splenic echinococcal cyst induced by blunt trauma to the left chest wall and upper abdomen. The main clinical manifestations, which developed within minutes of the trauma, were high fever, pruritus, edema of the lips and eyelids, dyspnea, stridor and rhinorrhea. Eosinophilia was not present on admission but appeared 4 days later. Surgery revealed an intact echinococcal cyst in the left lobe of the liver and another in the spleen. The splenic c

Until now, immunoassays for detection of anti-muscle relaxant IgE in serum have been performed with the drug coupled to epoxy-activated Sepharose or to RAST papers dics. In the present work we have used a quaternary ammonium-Sepharose in which the quaternary ammonium reactive group (choline chloride) was directly coupled to Sepharose via an ether linkage. 50 microliters of the quaternary ammonium solid phase (QAS) was incubated with 50 microliters of serum for 3 h, washed, incubated 18 h with 125I-anti-IgE and washed again. The results were expressed as the percentage of 125I-anti-IgE absorbed
