Plasma ropivacaine levels following scalp block for awake craniotomy.

The plasma levels of ropivacaine HCl with 5 mcg/mL epinephrine were measured in 10 patients following scalp blockade for awake craniotomy. A mean dose of 260 mg (3.6 mg/kg) resulted in peak plasma concentrations of 1.5 +/- 0.6 mcg/mL, with a median time to peak plasma concentration of 15 minutes. The pattern of rise of plasma level was similar in all patients and rapid compared with other regional blocks (epidural, intercostal, and axillary brachial plexus block). Despite this rapid rise of plasma level, no signs of cardiovascular or central nervous system toxicity were observed. In this group

Evaluation and management of chronic pelvic pain.

Evaluating patients with chronic pelvic pain is complex. A detailed medical history should be ideally supplemented by psychologic evaluation and assessment of the woman's social background. At the time of physical examination, the location and intensity of the pain should be mapped. Assessment of pain relief with the administration of a local anesthetic to trigger points or selected nerves may be useful in predicting the potential efficacy of surgical interventions such as uterosacral nerve ablation. Appropriate tests include pelvic ultrasound and magnetic resonance imaging. Ultimately, laparo

Esophagogastroduodenoscopy for the family physician.

Esophagogastroduodenoscopy is most commonly performed for evaluation of epigastric pain, severe heartburn, chronic nausea and other dyspepsia syndromes that are not relieved by medical therapy. It should not be performed as an in-office procedure in patients with unstable angina, respiratory failure, active upper gastrointestinal bleeding or hemodynamic instability. Although office esophagogastroduodenoscopy is safe for low-risk patients, the risks associated with anesthesia and the need for postprocedure observation may require other patients to undergo the procedure in a hospital setting. Myoclonic spasms following intrathecal diamorphine.

The use of intrathecal diamorphine via an implanted portal system is described for pain control in a patient suffering from vertebral metastatic disease. The complication of myoclonic spasms affecting the lower half of the body occurred after 14 days, when increasing the bolus dose to 40 mg. The spasms lasted for 3 hr and then gradually subsided. Diamorphine was subsequently restarted at a lower dose of 15 mg twice daily. On increasing the dose to 20 mg diamorphine 10 days later, severe distressing myoclonic spasms recurred 20 min postinjection. Myoclonus could only be controlled by institutin

High-resolution physical mapping of four microsatellite repeat markers near the RYR1 locus on chromosome 19q13.1 and apparent exclusion of the MHS locus from this region in two malignant hyperthermia susceptible families.

Malignant hyperthermia susceptibility (MHS) is a potentially lethal, hereditary disorder of skeletal muscle that may be triggered by inhalation anesthetics and depolarizing muscle relaxants. Defects in the gene encoding the ryanodine receptor (RYR1) localized on human chromosome 19q13.1 have been proposed to be responsible for MHS. Using a chromosome 19-specific human/hamster somatic cell hybrid mapping panel, we were able to determine that four closely linked microsatellite repeat markers bracket RYR1 with the order 19cen-D19S75-D19S191-RYR1-(D19S47, D19S190)-19ter. Application of the four ma

Advances in the understanding of anterior instability of the shoulder.

Anterior instability involves a spectrum of disease ranging from the obvious acute first-time dislocation to the athlete presenting with shoulder pain and no history suggestive of instability. It is important to recognize the pathophysiology and how it relates to this spectrum of disease. The arthroscope has helped to identify the underlying pathology in both acute and chronic situations. Diagnostically, a history of a painful shoulder, especially in the athlete, should suggest anterior instability. Tests of translation, apprehension, and the use of local anesthetic can be useful. Arthroscopy

Unusual complication of strabismus and lid surgery.

A patient with longstanding dysthyroid eye disease developed a broad adhesion between the globe and the upper eyelid following simultaneous surgery to recess the levator palpebrae superioris and lateral rectus muscles. This unusual complication may have developed as a consequence of failure to suture the conjuctiva of the upper fornix to the tarsus and also due to the prolonged static contact between the resulting adjacent raw areas due to postoperative eyelid traction and pressure dressings. Periods of such contact may have been further prolonged by the action of diazepam, which reduces the p

Esophageal atresia.

Esophageal anomalies present a challenge to the pediatric surgeon and demand close attention and care from the nursing staff as well as the surgical staff. We have encountered a survival rate of 96 per cent for all infants undergoing operation for esophageal atresia. An emergency gastrostomy is recommended as an immediate procedure when the diagnosis is established. In many instances, it can be performed with local anesthesia if the infant is particularly small or has developed aspiration pneumonia. The definitive operation is delayed until the infant is in optimum condition to permit a genera

[Postoperative cervical spondylitis. Apropos of 5 cases].

The authors reported 5 cases of post-operative cervicalgia of spondylodiscitic nature. Authors deem that this severe complication is to be attributed to the three following factors: microtraumatisms of cervical rachis with consequent local microcirculatory troubles; bacteremia provoked by anesthesiological manipulation (intubation, tracheal or oropharyngeal aspiration) or by the surgical operation and decrease in the immune response caused by operation and by used anesthetic drugs. Severe radiological alterations of the structure of cervical vertebrae and of the intersomatic space appear aroun

Evaluation of a surgical method for immobilizing the eye of an alert monkey.

One eye of a macaque monkey was immobilized by severing the third, fourth, and sixth cranial nerves. The residual movements of the operated eye were measured by repeatedly mapping the position of a visual receptive field. Movements of several degrees were caused by contraction of the orbital musculature during the closure blink. The amount of movement was dramatically reduced by paralysis of the superficial orbital muscles with a local anesthetic. Side effects of surgical immobilization include increased risk of corneal clouding and eye infection. In one monkey intraocular pressure was lowered

Denervation of the wrist joint.

A collective review was made of the results of denervation of the wrist joint for painful restrictiorn of motion done in 313 patients and follow-up studies on 195 (average 4.1 years, ranging from 9 months to 14 years). Complete denervation was done in only 30, partial denervation in the others being done after testing with local anesthetic blocks. Sixty-nine of the patients retained a moble wrist without pain or with slight pain with heavy work. No evidence of Charcot-like joints was seen. Poorest results followed when the operation was done for sequelae of intra-articular fracture of the radi

Perioperative Anesthetic Management of Kleine-Levin Syndrome: Current Evidence, Mechanistic Rationale, and a Proposed Practice Framework.

Kleine-Levin syndrome is a rare disorder of central hypersomnolence characterized by relapsing-remitting severe sleepiness associated with cognitive impairment, derealization, apathy, behavioral change, and hyperphagia. Direct perioperative evidence remains limited and is insufficient to establish comparative safety, recurrence risk, or a preferred anesthetic technique. Reports with direct perioperative evidence described favorable immediate perioperative courses, but follow-up was heterogeneous, and the uniformly favorable published outcomes are susceptible to substantial publication bias. Anesthesia management of a child with Melnick-Needles syndrome associated with retrognathia and significant tracheomalacia.

BACKGROUND: Melnick-Needles syndrome (MNS) is a rare X-linked genetic disorder caused by mutations in the FLNA gene, characterized by distinctive craniofacial and skeletal dysmorphisms, hearing loss, and urological anomalies. The associated upper airway abnormalities, including retrognathia and tracheomalacia, present a high-risk scenario for general anesthesia. CASE PRESENTATION: We report the case of an 8-year-old girl with genetically confirmed MNS scheduled for urgent transurethral stenting due to obstructive hydronephrosis and recurrent sepsis. Her clinical features included severe retrog

Macula-Involving Serous Choroidal Detachment following Multiple Short-Interval Trans-Tenon's Capsule Retrobulbar Anesthesia Injections during Vitreoretinal Surgery: A Case Report.

INTRODUCTION: Compared with sub-Tenon's anesthesia (STA), trans-Tenon's capsule retrobulbar anesthesia (TTRBA) provides rapid and complete akinesia. However, excessive anesthetic volume may cause complications such as choroidal detachment (CD). CASE PRESENTATION: A 59-year-old man who had previously undergone cataract surgery presented with rapidly progressive visual field constriction in the right eye. The best corrected visual acuity (BCVA) was 20/16; intraocular pressure, 13 mm Hg; and axial length, 29.99 mm. Multiple retinal breaks were detected in the superior and nasal retina, with reti

Unilateral Anterior Spinal Artery Syndrome following Spinal Anesthesia for Cesarian Section: A Case Report.

INTRODUCTION: Spinal cord infarction is a rare but serious neurologic complication of spinal anesthesia. Direct vessel injury, intra-arterial anesthetic injection, and anesthetic-induced local hypotension are potential mechanisms of infarction during this procedure. The proximity of the artery of Adamkiewicz to the spinal levels used for spinal anesthesia may also play a role. This case of unilateral anterior spinal artery syndrome highlights the potential for an atypical pattern of injury and deficits due to the complexity of the spinal cord's anterior circulation. CASE PRESENTATION: We pres

Neuraxial Anesthesia in Patients With Aortic Stenosis: A Systematic Review.

Neuraxial anesthesia (NA) has been contraindicated in patients with aortic stenosis (AS) due to concerns of sympathetic blockade and hemodynamic instability. These considerations are based on precautionary expert recommendations, supported by expected physiologic effects, but in the absence of any published scientific evidence. In light of the increasing elderly population and the prevalence of AS, this systematic review compiles available literature on NA in patients with AS to address the understanding of the anesthetic practice and safety in this population. Using a systematic approach, Pub

Regional anesthesia and sickle cell crisis in pediatric patients: An educational-focused review.

INTRODUCTION: Sickle cell disease (SCD) is the most common inherited hemoglobinopathy, affecting approximately 100 000 patients in United States and millions worldwide. Although the mainstay of pain management for VOC remains systemic opioids, given the potential for adverse effects including respiratory depression and hypoxemia, there remains interest in the use of regional anesthetic techniques (neuraxial or peripheral nerve blockade). METHODS: A systematic search of pubMed, Scopus, and Google Scholar was conducted using the terms sickle cell disease, sickle cell crisis, pain crisis, vaso-o

Using ketamine in a patient with a near-occlusion tracheal tumor undergoing tracheal resection and reconstruction: A case report.

BACKGROUND: Tracheal tumors may cause airway obstruction and pose a significant risk to ventilation and oxygenation. Due to its rarity, there is currently no established protocol or guideline for anesthetic management of resection of upper tracheal tumors, therefore individualized strategies are necessary. There are limited number of reports regarding the anesthesthetic management of upper tracheal resection and reconstruction (TRR) in the literature. We successfully used intravenous ketamine to manage a patient with a near-occlusion upper tracheal tumor undergoing TRR. CASE SUMMARY: A 25-yea

A standardized approach to airway management during Abbé flap reconstruction.

BACKGROUND: The Abbé flap is a two-staged procedure to address upper lip tightness, creating a surgically closed mouth during the first stage. Airway manipulation and management in the setting of a surgically closed mouth presents a challenge from an anesthetic standpoint. AIMS: This study aims to describe the authors' standardized approach to airway management in cleft lip patients undergoing Abbé flap reconstruction. METHODS: A retrospective review was performed including consecutive patients who underwent Abbe flap reconstruction at a single institution from 2019 to 2021. Five patients we

Study of pain response in neonates during venipuncture with a view to analyse utility of topical anaesthetic agent for alleviating pain.

BACKGROUND: Neonates being nonverbal are unable to express their pain leading to underestimation of their pain and hence insufficient pain relief. Neonatal pain is assessed by pain scales based on the behavioural and physiological changes that occur in response to painful stimuli. This cross sectional study was conducted at a tertiary care centre using Premature Infant Pain Profile (PIPP) score with 4% lidocaine as local anaesthetic agent to produce surface anaesthesia of skin prior to intravenous cannulation. METHODS: Sample size was collected by simple randomisation method. Our study groups
