A Battery-Powered Ankle Exoskeleton Improves Gait Mechanics in a Feasibility Study of Individuals with Cerebral Palsy.

Neuromuscular impairment associated with cerebral palsy (CP) often leads to life-long walking deficits. Our goal was to evaluate the ability of a novel untethered wearable ankle exoskeleton to reduce the severity of gait pathology from CP. In this clinical feasibility study of five individuals with CP, we used instrumented gait analysis to quantify how powered plantar-flexor assistance affected gait mechanics following multi-visit acclimation. Compared to how each participant walked normally, walking with untethered exoskeleton assistance resulted in improved ankle plantar-flexion and knee ext…

Poor prognosis for infectious complications of surgery for ankle and hindfoot fracture and dislocation. A 34-case series.

INTRODUCTION: Ankle and hindfoot fractures are associated with high rates of complications, and of infection in particular, for which rates of 1% to 48% are reported. HYPOTHESIS: Treatment of bone and joint infection (BJI) secondary to surgery for ankle or hindfoot fracture is at high risk of failure. We analyzed results of treatment of BJI in this context. MATERIAL AND METHODS: 33 patients (34 cases) were treated for ankle or hindfoot BJI between 2010 and 2015. Cure was defined by absence of fistula and of local or general inflammatory signs and by normal C-reactive protein level, at a mini…

Dimensional Changes of the Tibial Nerve and Tarsal Tunnel in Different Ankle Joint Positions in Asymptomatic Subjects.

The tarsal tunnel is a clinically important fibrous osseous conduit for the tibial nerve and associated tendons. It is mechanically dynamic, and normal ankle movements appear to change the tunnel shape, potentially having an impact on the tibial nerve. The objective of this study was to measure changes in the tibial nerve and tarsal tunnel dimensions in plantarflexion and dorsiflexion of the ankle joint in healthy subjects. A cross-sectional study with 13 volunteer subjects and a total of 18 records was designed. The cross-sectional area, anterior-posterior distance, transverse distance, and f…

Carotid intima media thickness and ankle brachial index are inversely associated in subjects with and without diabetes.

Carotid intima-media thickness (IMT) and ankle brachial index (ABI) are non-invasive indicators of generalised atherosclerosis. The aim was to determine the association between carotid IMT and ABI in subjects with and without diabetes mellitus (DM), and to analyse specific age change-points. We included 2744 subjects from the Copenhagen City Heart Study (mean age (SD) 56.6 (17.2) years, 56.8% women and body mass index (BMI) 25.4 (4.1) kg/m2). Carotid IMT and ABI measurements were performed during the fifth examination. Of the 2744 subjects, 125 subjects (4.6%) had DM. Average carotid IMT was 0…

The Effect of Insurance Type on Patient Access to Ankle Fracture Care Under the Affordable Care Act.

The purpose of this study is to assess the effect of insurance type (Medicaid, Medicare, private insurance) on the ability for patients with operative ankle fractures to access orthopedic traumatologists. The research team called 245 board-certified orthopedic surgeons specializing in orthopedic trauma within 8 representative states. The caller requested an appointment for their fictitious mother in order to be evaluated for an ankle fracture which was previously evaluated by her primary care physician and believed to require surgery. Each office was called 3 times to assess the response for e…

The Impact of Perioperative Ankle-Brachial Index and Clinical Status on Outcomes following Lower Extremity Bypass.

BACKGROUND: Ankle-brachial index (ABI) is a common method of graft surveillance after infrainguinal lower extremity bypass (LEB) surgery and is recommended by the Society for Vascular Surgery. Several studies failed to show the benefit of ABI surveillance but were limited by sample size, and the practice remains variable. METHODS: We identified all patients who underwent LEB for occlusive disease from the Vascular Study Group of New England Registry (VSGNE) between 2003 and 2016. Postoperative changes were defined as follows: improvement for ABI >0.15 at discharge or clinical status improved …

Effects of Dangguixu-san on acute lateral ankle sprain: study protocol for a randomized controlled trial.

BACKGROUND: Ankle sprain is a common musculoskeletal injury. In Korean medicine, blood stasis is thought to be the main cause of pain and swelling in patients with ankle sprain. Dangguixu-san (DS), a herbal extract, is widely used in Korean medicine for the treatment of traumatic ecchymosis and pain by promoting blood circulation and relieving blood stasis. However, the effects of DS on ankle sprain have not been evaluated in a randomized clinical trial. Here, we describe the protocol for a randomized controlled trial that will evaluate the efficacy and safety of DS for the treatment of ankle …

Maximum-Entropy-Rate Selection of Features for Classifying Changes in Knee and Ankle Dynamics During Running.

This paper investigates deteriorations in knee and ankle dynamics during running. Changes in lower limb accelerations are analyzed by a wearable musculoskeletal monitoring system. The system employs a machine-learning technique to classify joint stiffness. A maximum-entropy-rate method is developed to select the most relevant features. Experimental results demonstrate that distance travelled and energy expended can be estimated from observed changes in knee and ankle motions during 5-km runs.

Lower Extremity Biomechanics During a Drop-Vertical Jump in Participants With or Without Chronic Ankle Instability.

CONTEXT: Chronic ankle instability (CAI) is a condition characterized by range-of-motion, neuromuscular, and postural-control deficits and subjective disability, reinjury, and posttraumatic osteoarthritis. Differences have been reported in kinematics, kinetics, surface electromyography (EMG), and ground reaction forces during functional tasks performed by those with CAI. These measures are often collected independently, and the research on collecting measures simultaneously during a movement task is limited. OBJECTIVE: To assess the kinematics and kinetics of the lower extremity, vertical gro…
