Analysis of HRV based on routine 24-hour Holter recordings provides a sensitive, noninvasive measurement of autonomic input to the heart. HRV can be measured in the time or frequency domain. Each frequency domain variable correlates at least r = 0.85 with a time domain variable. Autonomic dysreflexia is the most important specific complication of high level spinal cord injury both in tetraplegic and in paraplegic patients above the midthoracic neural segment. It is a life threatening emergency that may lead to apoplexy. We present a case of fatal cerebral hemorrhage due to autonomic dysreflexia in order to demonstrate the gravity of this particular syndrome.

The present study was conducted to determine the relationship between autonomic nervous function and cerebral autoregulation in 17 patients with vertigo or dizziness. Autonomic nervous function was examined by measuring systolic blood pressure response and dynamic electrocardiogram R-peak to R-peak response upon standing up. Regional cerebral autoregulation was examined by comparing cerebral blood flow in both supine and standing positions with single photon emission computed tomography. Moreover, the relationship of each of these three measures to postural provocation of symptoms was studied.

Tetanus is often accompained by autonomic instability, rendering hemodynamic management difficult. Death is frequently secondary to an inability to control this instability. A variety of modalities have been used to stabilize the cardiovascular system, but all are not ideal. Esmolol offers theoretical advantages over other modalities. We report a case of severe tetanus in which a continuous infusion of esmolol was effective in controlling the autonomic instability.

Acute autonomic neuropathy is an uncommon syndrome, usually affecting healthy young people. Presentation is often dramatic and initial misdiagnosis is common.
