OBJECTIVE: Growth modulation in operative treatment of juvenile scoliosis can be done by USS paediatric instrumentation to control spinal growth in patients of small stature with juvenile scoliosis. The double-rod system has to be distracted every 4-6 months. The system is used in young patients too tall for VEPTR (vertical expandable prosthetic titanium rib) instrumentation. The system with a very low profile allows reduced soft-tissue pressure saving soft tissue from atrophy or the development of pseudocysts above the screws. With this procedure controlled growth with growth modulation of th

Natural teeth have a periodontal ligament with viscoelastic properties, while implants are connected to the bone with a strong connection and osseointegration; therefore, the stress on the adjacent bone of the implant and its prosthetic components is more than that of natural teeth. This study examines the connection (Morse different tapers) to find the most suitable length and Morse angle of the taper and the angle of applying the force on the tooth to create the least stress using finite element analysis. Geometrical and 3D models of the mandible bone, implant, and its prosthetic components

STUDY DESIGN: The bone union rate after lumbar interbody fusion (LIF) using titanium (Ti) or polyetheretherketone (PEEK) cages was investigated retrospectively.
OBJECTIVE: To assess whether the PEEK cage is superior to the Ti cage in terms of bone union after LIF.
SUMMARY OF BACKGROUND DATA: We previously reported that the formation of vertebral endplate cysts is useful for predicting nonunion after LIF using Ti cages.
METHODS: We examined 144 levels in 117 patients treated from March 2005 to July 2012 with transforaminal LIF using Ti (93 levels in 77 patients) or PEEK cages (51 levels in 4

BACKGROUND: Minimally invasive percutaneous screw fixation for pelvic ring and acetabular fractures has become increasingly popular due to its numerous benefits. However, the precise placement of the screw remains a critical challenge, necessitating a modification of the current techniques. This paper introduces a refined technique employing a modified guidewire to enhance the precision and efficiency of percutaneous fixation in pelvic and acetabular fractures.
METHODS: This study details the surgical techniques implemented for correcting guidewire misdirection in percutaneous screw fixation

BACKGROUND: A dorsal wrist-spanning plate (DWSP) can be affixed to the second or third metacarpal, depending on the morphology of the distal radial fracture (DRF) and on surgeon preference, but comparative biomechanical analyses between these methods are limited. This biomechanical study compared fixation to the second metacarpal versus the third metacarpal as measured by lunate facet fragment displacement in a cadaveric model of an axially loaded intra-articular DRF. The construct rigidity with the addition of an intrafragmentary lunate facet locking screw was also evaluated.
METHODS: A samp

Stress fractures of the proximal fifth metatarsal are common injuries in elite athletes. Fixation using an intramedullary screw represents the most popular surgery performed for treating these injuries, with excellent results in most cases. However, multiple reports in the literature highlight the possibility of painful hardware, usually related to the presence of the screw head, following intramedullary fixation In this case report, we outline 4 cases of professional athletes who developed lateral-based foot symptoms following complete healing of their surgically treated proximal fifth metata

BACKGROUND: It is unclear whether lunate fracture types in Kienböck disease correlate with the degree of fracture healing. This study aimed to review the clinical results of stage 3 Kienböck disease treated using partial capitate shortening (PCS) and to evaluate the healing rates of lunate fractures based on the fracture types.
METHODS: Twenty-three patients were clinically and radiographically followed up for >2 years after PCS. The preoperative location and postoperative healing of the lunate fracture were evaluated using plain radiography, computed tomography, and/or magnetic resonance ima

BACKGROUND: Intra-articular Pilon fractures remain therapeutically challenging due to osteochondral fracturing and comminution, marginal impaction, and insult to the soft tissue envelope. The purpose of this study was to compare the efficacy of anterolateral distal tibial locking plates in capturing main fracture fragments in tibial plafond fractures.
METHODS: From May 2011 to Dec 2015, 169 OTA C-type pilon fractures met inclusion and exclusion criteria with computed tomographic (CT) scans performed prior to definitive fixation. For each patient, the fracture lines were mapped, digitized, and

OBJECTIVES: The purpose of this study was to examine the screw trajectory of ten commercially available distal tibia plates and compare them to common fracture patterns seen in OTA C type pilon fractures to determine their ability to stabilize the three most common fracture fragments while buttressing anterolateral zones of comminution.
HYPOTHESIS: We hypothesized that a single plate for the distal tibia would fail to adequately stabilize all three main fracture fragments and zones of comminution in complex pilon fractures.
METHODS: Ten synthetic distal tibia sawbones models were used in con

PURPOSE: The purpose of this study was to evaluate the biomechanical stability and compressive forces across the sacroiliac (SI) joint of an anterior internal fixator compared to the femoral distractor and external fixator for vertically unstable pelvic fractures.
METHODS: Five composite pelvises with a simulated APC type III injury fixed with a femoral distractor, external fixator, or anterior internal fixator were tested. A pressure-sensitive film (Tekscan) was placed in the disrupted SI joint recording the magnitude of force. Then, in a single-leg stance model (Instron machine), a load was

Displaced fractures of the lateral malleolus are typically treated with plate osteosynthesis with or without the use of lag screws, and immobilisation in a plaster cast for up to 6 weeks. Fixation through a smaller incision with less metal, such as lag screw only fixation, would theoretically lead to decreased infection rates and less irritation caused by hardware. The purpose of this study was to evaluate the benefits and success of lag screw only fixation of the lateral malleolus in non-comminuted oblique fractures of the lateral malleolus. A total of 25 patients who had non-comminuted unsta

Supracondylar fractures of the humerus in childhood frequently result in cubitus varus deformity. Pain, functional impairment and cosmetic problems can request surgical correction. This is a report on an adult and on a mature child with such deformities. The boy had sustained three injuries with following bilateral cubitus varus and lengthening of one lower leg. Planning, surgical technique and outcome of the osteotomies are described. We should recommend the resection of a full wedge with two oblique osteotomies and internal fixation with two plates on either side of the humerus.

Over the years, depression of the malar eminence has been one of the most persistent complications of zygoma fractures, regardless of the fixation technique used. The use of a sublabial incision for direct visualization and alignment of the fracture lines at the infraorbital rim and lateral maxillary buttress is described. When coupled with single miniplate fixation of the zygoma across the frontozygomatic suture, the sublabial approach results in improved reduction of the fractures with rigid stabilization. Our experience with this approach, as well as with two-point wire fixation and with mi

Chronic atraumatic subtalar dislocation is an exceptionally rare and severe manifestation of adult acquired flatfoot deformity (AAFD) secondary to posterior tibial tendon insufficiency (PTTI). Early recognition is critical, as progressive deformity leads to rigid valgus malalignment and functional decline. A 65-year-old male presented with a seven-year history of worsening left foot pain and deformity. Examination showed rigid hindfoot valgus, midfoot collapse, forefoot abduction, and taut skin over the medial talar head. Radiographs revealed chronic lateral subtalar dislocation with talonavic

BACKGROUND: Medial femoral condylar fractures are uncommon and present diagnostic and management challenges. Medial buttress plating should be utilized for treatment. However, there are currently no available anatomical plates that fit the femoral medial condyle, posing management challenges and necessitating the use of alternative implants.
CASE PRESENTATION: We present a case of a 65-year-old male with poliomyelitis who experienced a ground-level fall resulting in a comminuted medial condylar impaction fracture with a posterior Hoffa fragment. Reduction was achieved through a medial approac

An 80-year-old fit and active gentleman presented with left hip pain and a new central scotoma in his right eye after falling from an electric bicycle. He was able to mobilise independently and presented to ophthalmic services where cotton wool spots were found in both eyes and a presumptive diagnosis of Purtscher's retinopathy (PuR) was made. The patient was referred to the emergency department for assessment of a potential fracture. The subcapital femoral neck fracture was subtle on X-ray and only detected the following day with radiological and orthopaedic review. Two screws were placed to

Intra-articular fractures of the distal humerus are complex injuries that can considerably limit elbow function if not treated appropriately. Surgical management is indicated for most intra-articular distal humerus fractures with the goal of restoring elbow range of motion and function. Open reduction and internal fixation (ORIF) with plates and screws has been the preferred surgical option. Double plating is recommended for bicolumnar fractures and plates can be applied either parallel or orthogonal to each other. Surgical approach for ORIF of the distal humerus can be performed through an ol
