Direct healthcare costs associated with sickle cell disease complications: a retrospective cohort study using routinely collected healthcare data in England.

OBJECTIVES: Due to the multisystemic nature of sickle cell disease (SCD), complications can occur together and thus discerning costs associated with individual complications requires a methodology that can estimate the costs of a given complication while accounting for the presence of other complications. In this study, we aimed to estimate period-based incremental costs associated with specific chronic complications in patients with SCD in England while accounting for multimorbidity. DESIGN/SETTING: All-cause primary and secondary care healthcare resource utilisation (HCRU) was obtained for

The Effects of Glucagon-Like Peptide-1 Receptor Agonists on Mitochondrial Function Within Skeletal Muscle: A Systematic Review.

BACKGROUND: Obesity is a chronic disease associated with increased risk of multiple metabolic and mental health-related comorbidities. Recent advances in obesity pharmacotherapy, particularly with glucagon-like peptide-1 (GLP-1) receptor agonists (RAs), have the potential to transform obesity and type 2 diabetes mellitus (T2DM) care by promoting marked weight loss, improving glycaemic control and addressing multiple obesity-related comorbidities, with added cardio-renal benefits. Dual agonists combining GLP-1 with other enteropancreatic hormones such as glucose-dependent insulinotropic polypep

Distinct virologic trajectories in chronic hepatitis B identify heterogeneity in response to nucleos(t)ide analogue therapy.

BACKGROUND & AIMS: The dynamics of HBV viral load (VL) in patients with chronic hepatitis B (CHB) on nucleos(t)ide analogue (NA) treatment and its relationship with liver disease are poorly understood. We aimed to study longitudinal VL patterns and their associations with CHB clinical outcomes. METHODS: Utilising large scale, routinely collected electronic health records from six centres in England, collated by the National Institute for Health and Care Research Health Informatics Collaborative (NIHR HIC), we applied latent class mixed models to investigate VL trajectory patterns in adults re

A Systematic Review and Meta-Analysis of Postoperative Opioid Consumption, Pain and Complications Following Intraoperative Autonomic Nervous System-Based Nociception Monitoring.

BACKGROUND: Autonomic nervous system-based nociception monitors have been proposed to optimise intraoperative opioid administration. While they reduce intraoperative opioid use, evidence on postoperative outcomes remains limited. We conducted a systematic review and meta-analysis to evaluate their effects on postoperative pain, opioid consumption and complications. METHODS: MEDLINE, EMBASE, LILACS, Google Scholar, Cochrane and ClinicalTrials.gov were searched for randomised controlled trials comparing intraoperative nociception monitoring with standard care and reporting postoperative outcome

Global epidemiology of hip fractures in adults aged 70 years and older from 1990 to 2021: a cross-sectional analysis from Global Burden of Disease Study 2021.

PURPOSE: Driven by population aging, osteoporosis, and the frequent coexistence of multiple chronic conditions, hip fractures mainly occur in adults aged ≥70 years. Nevertheless, the specific burden attributable to this population remains insufficiently understood. This study sought to address key gaps in the literature by analyzing the Global Burden of Disease 2021 dataset to investigate hip fracture patterns across age, sex, and sociodemographic index (SDI). METHODS: We extracted hip fracture incidence, prevalence, years lived with disability (YLD), and age-standardized rates from the Globa

Preventing Parkinson's disease in the context of movement disorders: a narrative review of current evidence and future directions.

Parkinson's disease (PD) is the fastest growing neurological disorder worldwide and, together with other movement disorders, belongs to a group of chronic neurological conditions associated with a substantial burden for affected individuals, caregivers, and healthcare systems. Despite significant advances in symptomatic treatment, disease-modifying therapies remain unavailable, shifting increasing attention toward prevention as a key therapeutic strategy. In this narrative we primarily focus on PD, as the epidemiologically most challenging condition and the one for which the most comprehensive

Evaluating postoperative pneumatic lymphatic drainage following lymphaticovenous anastomosis for secondary lower extremity lymphoedema in Japan (LVA-PLD trial): protocol for a multicentre randomised controlled trial.

INTRODUCTION: Secondary lower extremity lymphoedema is a chronic progressive condition that frequently develops after cancer treatment and results in persistent swelling, recurrent cellulitis and impaired quality of life. Lymphaticovenous anastomosis (LVA) is an established physiological microsurgical treatment; however, postoperative outcomes vary and evidence-based adjunctive postoperative management remains limited. A novel pneumatic lymphatic drainage (PLD) system has been developed to deliver anatomically directed, pathway-aligned mechanical stimulation that mimics manual lymphatic draina

Differentiating network structures and sex differences of pain-related outcomes, analgesic opioid dosages, and psychosocial factors for postoperative management: a study of PAIN OUT registry in seven Asian regions.

BACKGROUND: Despite advancements in global healthcare, Asia lacks a systematic framework for postoperative pain-related symptoms, particularly in addressing pain identification/prevention and reporting sex differences. This study used network analysis to unravel the complex interplay of pain-related symptoms, analgesic opioid dosages, and psychosocial factors, with a particular focus on sex differences in the Asian population. METHODS: Utilizing anonymized data from 5,093 adult patients across seven Asian regions between 2018 and 2021 from the PAIN OUT registry, this study applied network ana

Bone health in ambulatory male patients with chronic obstructive airway disease - A case control study from India.

OBJECTIVE: Chronic obstructive airway disease (COPD) is characterized by airflow limitation due to airway and/or alveolar abnormalities with significant extra-pulmonary manifestations. Bone health impairment is an extra-pulmonary complication of COPD which is less well studied in India. Moreover, it can contribute to significant morbidity and mortality. Hence, we aim to estimate the prevalence of osteoporosis and metabolic parameters of adverse bone health in patients with COPD. METHODS: In this case control study, male subjects aged 40-70 years with COPD attending the respiratory outpatient

The L-shaped correlation between systolic blood pressure and short-term and long-term mortality in patients with cerebral hemorrhage.

BACKGROUND: A large amount of evidence has shown the necessity of lowering blood pressure (BP) in patients with acute cerebral hemorrhage, but whether reducing BP contributes to lower short-term and long-term mortality in these patients remains uncertain. AIMS: We aimed to explore the association between BP, including systolic and diastolic BP, during intensive care unit (ICU) admission and 1-month and 1-year mortality after discharge of patients with cerebral hemorrhage. METHODS: A total of 1085 patients with cerebral hemorrhage were obtained from the Medical Information Mart for Intensive

The integrated treatment of eating disorders, posttraumatic stress disorder, and psychiatric comorbidity: a commentary on the evolution of principles and guidelines.

Psychiatric comorbidity is the norm in the assessment and treatment of eating disorders (EDs), and traumatic events and lifetime PTSD are often major drivers of these challenging complexities. Given that trauma, PTSD, and psychiatric comorbidity significantly influence ED outcomes, it is imperative that these problems be appropriately addressed in ED practice guidelines. The presence of associated psychiatric comorbidity is noted in some but not all sets of existing guidelines, but they mostly do little to address the problem other than referring to independent guidelines for other disorders.

Home and community care services: a major opportunity for preventive health care.

BACKGROUND: In Australia, the Home and Community Care (HACC) program provides services in the community to frail elderly living at home and their carers. Surprisingly little is known about the health of people who use these services. In this study we sought to describe health-related factors associated with use of HACC services, and to identify potential opportunities for targeting preventive services to those at high risk. METHODS: We obtained questionnaire data from the 45 and Up Study for 103,041 men and women aged 45 years and over, sampled from the general population of New South Wales,

Improvements in Air Quality and Health Outcomes Among California Medicaid Enrollees Due to Goods Movement Actions.

INTRODUCTION: In 2006, the California Air Resources Board (CARB) and local air quality management districts implemented an Emission Reduction Plan for Ports and Goods Movement program (referred to hereinafter as GM policy actions) (CARB 2006). The GM policy actions comprise approximately 200 actions with an estimated investment value of $6 to $10 billion. These actions targeted the major sources and polluters related to goods movements, such as highways; ports and railyard trucks; ship fuel and shore power; cargo equipment; and locomotives. These actions aimed to reduce total statewide domesti
