Is Cytoreductive surgery for unresectable colorectal liver metastases able to improve patient outcome? Results of a pilot prospective study.

BACKGROUND: Curative R0-R1 resection is not always feasible in patients with extensive bilobar colorectal liver metastases (CRLM). When unresectatility is confirmed, the standard of care is palliative chemotherapy or more recently liver transplantation (LT) with very restrictive eligibility criteria. This exploratory study evaluated cytoreductive surgery as an alternative to chemotherapy alone in patients with initially unresectable CRLM responding well to systemic chemotherapy but ineligible to LT. METHODS: This prospective study (January 2017-January 2024) included patients with permanently

Public preferences for robotic surgery across two cancer scenarios: a latent class discrete choice study.

Public preferences for robotic surgery may reflect different responses to the technology, physician recommendation, cost, and clinical context. This study characterized latent preference profiles across colorectal and thyroid cancer scenarios using a joint model with shared class membership and scenario-specific utilities. A discrete choice experiment was conducted among 1,012 Korean adults. Colorectal cancer choices compared robotic and laparoscopic surgery, whereas thyroid cancer choices included robotic, endoscopic, and open surgery. Attributes included out-of-pocket cost, physician recomme

FOLFOXIRI plus panitumumab for left-sided RAS-wild-type metastatic colorectal cancer.

BACKGROUND: Colorectal cancer (CRC) can be effectively treated with fluoropyrimidine chemotherapy doublets. The triplet chemotherapy program, FOLFOXIRI, is more effective than a doublet in randomized trials. We combined this triplet with the anti-epidermal growth factor receptor (EGFR) antibody, panitumumab, as first-line therapy in the appropriately targeted population: left-sided, RAS wild-type metastatic CRC. METHODS: A multicenter trial was conducted with the AGICC (Academic GI Cancer Consortium) group. Patients with stage IV or unresectable, left-sided, Rat Sarcoma Virus oncogene (RAS) w

Engineering design of oxaliplatin nanodelivery systems: Overcoming biological barriers toward precision therapy of colorectal cancer.

Colorectal cancer (CRC) remains one of the leading causes of cancer-related mortality worldwide, and the clinical efficacy of oxaliplatin (OXA), a first-line chemotherapeutic agent for CRC, is significantly limited by insufficient tumor targeting, limited intratumoral drug accumulation, and acquired drug resistance. Recent advances in nanomedicine have enabled the rational design of OXA nanodelivery systems to improve pharmacokinetics, enhance tumor accumulation, and achieve controlled drug release. This review summarizes the molecular mechanisms underlying OXA resistance and tumor biological

Enhancing preoperative planning for colorectal liver metastases: the role of open source 3D reconstruction across different levels of surgical expertise.

BACKGROUND: This prospective study evaluated open-source 3D reconstruction's impact on the preoperative assessment of colorectal liver metastases (CRLM) compared to standard 2D computed tomography (CT), focusing on its role as an "educational equalizer." METHODS: Thirty physicians, stratified by expertise, evaluated 10 cases of varying complexity. Performance was measured via a standardized scoring system (0-8), execution time, and satisfaction surveys. RESULTS: 3D modeling significantly improved diagnostic accuracy, with median scores increasing from 3.0 to 6.0 (p < 0.001). Mean evaluation

Integrated transcriptomic profiling reveals immune and molecular stage-specific signatures of colorectal cancer in hispanics living in Puerto Rico.

INTRODUCTION: Colorectal cancer (CRC) remains one of the most diagnosed malignancies worldwide and continues to rank among the leading causes of cancer-related death. Cancer progression involves dynamic transcriptional and immunological changes, and although many key molecular alterations driving CRC progression have been described, stage-specific differences remain poorly understood. This study aimed to characterize transcriptomic changes in Hispanics living in Puerto Rico (PRH) to identify relevant pathways, target genes, and stage-specific immune and molecular signatures. METHODS: We condu

Natural Orifice Extraction Techniques (Natural Orifice Specimen Extraction and Natural Orifice Transluminal Endoscopic Surgery) for Left-Sided Colorectal Cancer: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Purpose: Minimally invasive surgery is the standard approach for colorectal cancers and requires an abdominal incision for specimen removal. Natural orifice specimen extraction (NOSE) may improve outcomes, reducing trauma, and speeding postoperative recovery. This study compares both techniques regarding postoperative complications, operative outcomes, and recurrence. Methods: We searched PubMed, Scopus, and Cochrane Central Register of Clinical Trials for studies published up to November 2024. Odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (CIs) were pooled using a

Prognosis in stage II colorectal cancer: the effect of the primary tumor location and biomarkers-a retrospective cohort study.

BACKGROUND: The prognostic impact of primary tumor location (right- vs left-sided) is well-established in stage III/IV colorectal cancer (CRC), with right-sided tumors associated with poorer outcomes. However, its prognostic role in stage II CRC remains unclear. This study investigates whether the worse outcomes observed in advanced right-sided cancers also apply to early-stage CRC, or if they may be influenced by other factors such as patient demographics and molecular biomarkers. Clarifying this could help improve risk stratification in clinical practice. METHODOLOGY: For this study, we ana

Oral delivery of aptamer-decorated SICTERS Raman probes for colonoscopy-guided resection and photothermal immunization of microtumors.

Endoscopic resection of colon cancer represents an advanced technique in early intervention, but faces the challenges in identifying tumor margins and removing residual lesions. Here, we developed orally administered Raman probes (i.e., BBT-Apt@CS NPs) that enable precise tumor imaging and photothermal ablation. Protected by an enteric coating for colon-targeted release, the inner nanoparticles use a stacking-induced charge transfer-enhanced Raman scattering (SICTERS) mechanism and are functionalized with tumor-targeting aptamers. In a proof-of-concept study using the endoscopic optical fiber,

Simple definition of biologically borderline resectable colorectal liver metastases based on early surgical failure.

BACKGROUND: The benefit of neoadjuvant chemotherapy (NAC) in patients with resectable colorectal liver metastasis (CRLM) is debatable. This study aimed to establish a definition of biologically borderline resectable CRLM based on early surgical failure. METHODS: One hundred forty-two patients who underwent upfront surgery for resectable CRLM were examined. Potential predictors of early surgical failure were investigated to establish a definition of biologically borderline resectable CRLM. The impact of NAC on overall survival (OS) in patients with borderline resectable CRLM was examined, as w

Colonization of stenotrophomonas and its associated microbiome between paired primary colorectal cancers and their lung metastatic tumors (experimental studies).

The role of gut microbiome in colorectal cancer (CRC) lung metastases is still unclear. Herein, we collected primary colorectal tumors, matched lung metastasis, and adjacent normal colon from seven patients for 16S rRNA sequencing to evaluate microbiome signature in distant site metastasis and analyze prognosis. Fecal samples from 19 non-metastases CRC patients and 11 CRC lung metastases patients were analyzed for validation. Our efforts led to a discovery of comprehensive microbiome in CRC lung metastatic tumors, characterized by a decreased microbial diversity and an increase bacterial inter

Explainable machine learning for predicting lung metastasis of colorectal cancer.

Patients with lung metastasis of colorectal cancer typically have a poor prognosis. Therefore, establishing an effective screening and diagnosis model is paramount. Our study seeks to construct and verify a predictive model utilizing machine learning (ML) that can evaluate the risk of lung metastasis with newly diagnosed colorectal cancer (CRC) using Shapley Additive exPlanations (SHAP). Using the Surveillance, Epidemiology, and End Results database, 39,674 were extracted for model development, all of whom had been pathologically diagnosed with CRC. The data spans from 2010 to 2015. Our study

Temporal and Spatial Metabolic Shifts Revealing the Transition from Ulcerative Colitis to Colitis-Associated Colorectal Cancer.

Patients with ulcerative colitis (UC) have a higher risk of developing colorectal cancer (CRC), however, the metabolic shifts during the UC-to-CRC transition remain elusive. In this study, an AOM-DSS-induced three-stage colitis-associated colorectal cancer (CAC) model is constructed and targeted metabolomics analysis and pathway enrichment are performed, uncovering the metabolic changes in this transition. Spatial metabolic trajectories in the "normal-to-normal adjacent tissue (NAT)-to-tumor" transition, and temporal metabolic trajectories in the "colitis-to-dysplasia-to-carcinoma" transition

A Community-Engaged Approach to Enhancing Participation in Genomic Research in Rare and Understudied Cancer Populations.

UNLABELLED: Few studies provide insights on how to incorporate community members' perspectives of genomic research during the early phases of study development. Engaging with community members early and consistently throughout the research lifecycle could help identify and mitigate barriers to genomic research participation, particularly among groups with rare and understudied cancers. METHODS: The Washington University Participant Engagement and Cancer Genome Sequencing (WU-PE-CGS) study formed a Participant Engagement Advisory Board (PEAB) consisting of patients, patient advocates, and pati

The macrophage galactose-type C-type lectin 1 receptor plays a major role in mediating colitis-associated colorectal cancer malignancy.

Cancer-associated aberrant glycosylation can be detected by the macrophage galactose-type C-type lectin (MGL) receptor; however, whether this interaction enhances or deadens cancer development along with the associated immune response has not been well established. To determine the role of mouse MGL1 in colitis-associated colon cancer (CAC), azoxymethane (AOM)/dextran sodium sulfate (DSS)-induced tumor development was compared between Mgl1 knockout (Mgl1-/-) mice and their wild-type (WT) littermates. At 75 days post-CAC induction, colon tumor tissue contained more highly glycosylated proteins,

CAN-Scan: A multi-omic phenotype-driven precision oncology platform identifies prognostic biomarkers of therapy response for colorectal cancer.

Application of machine learning (ML) on cancer-specific pharmacogenomic datasets shows immense promise for identifying predictive response biomarkers to enable personalized treatment. We introduce CAN-Scan, a precision oncology platform, which applies ML on next-generation pharmacogenomic datasets generated from a freeze-viable biobank of patient-derived primary cell lines (PDCs). These PDCs are screened against 84 Food and Drug Administration (FDA)-approved drugs at clinically relevant doses (Cmax), focusing on colorectal cancer (CRC) as a model system. CAN-Scan uncovers prognostic biomarkers
