Dysphagia Prevention Strategies for Community-Dwelling Older Adults: A Scoping Review.

BACKGROUND: Swallowing difficulty, or dysphagia, is a common issue in aging populations, including those living in the community, due to age-related changes in sensory and motor functions. Effective prevention strategies are essential to avoid complications and improve well-being in this population. OBJECTIVE: The objectives of this scoping review were to identify and summarise interventions for preventing dysphagia in community-dwelling older adults. DESIGN: Following Arksey and O'Malley's framework, studies were retrieved from six databases: Embase, Ovid-Medline, Cochrane Library, CINAHL,

Communicating anaphylaxis risk in pediatric allergy consultations: A conversation analytic study.

UNLABELLED: Allergy in children is increasing. Reactions vary, with severe cases involving potentially fatal anaphylaxis which is treated with adrenaline. At-risk patients are prescribed adrenaline auto-injectors. However, due to limited specialist UK allergy clinics, risk discussions often take place with non-specialists (e.g., General Practitioners in primary care and general paediatricians in secondary care) who may lack confidence discussing risk and determining treatment suitability. Systematic examination of strategies for discussing risk is needed. OBJECTIVES: To identify, for the firs

iPhone Pregnancies: Self-Testing as Surveillance and Care in the Trying to Conceive Community.

People trying to conceive (TTC) often rely on accessible technologies and associated apps to track aspects of menstrual cycles. We explore this growing phenomenon from the perspective of self-testing as surveillance-care for the TTC individual or couple and their current and future fertility and pregnancy. Through an analysis of anonymous fora posts, we argue that surveillance-care provides those TTC with a sense of community, as well as agency and control over inexact bodily processes. Here, surveillance-care enacted on the self is about care for the hoped-for future pregnancy, and resulting

Exploring the Ecological Impacts of Herbicides on Antibiotic Resistance Genes and Microbial Communities.

The widespread application of herbicides has profound ecological consequences, particularly regarding the distribution of antibiotic resistance genes (ARGs) and microbial communities. In this study, we analyzed herbicide-related metagenomic data to assess the impact of herbicide exposure on ARGs and microbial populations. Our results demonstrate that herbicide application significantly increased the abundance of ARGs, particularly those associated with multidrug resistance, sulfonamides, and bacitracin, with notable increases in subtypes such as bacA and sul1. Microbial community analyses reve

Assessing communicative-pragmatic ability in telehealth: e-ABaCo in autistic individuals.

INTRODUCTION: Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder characterized by difficulties in social communication. Autistic individuals who are verbally fluent, often show difficulties in pragmatic ability, i.e. the capacity to use language and other expressive means, as gestures or the tone of the voice, to convey meaning in a given context. During the COVID-19 pandemic, the use of various technologies such as videoconferencing and internet for the delivery of healthcare services, i.e. telehealth, has proven to be effective, accessible and safe tools for remote healthcare. Nursing personnel and speech-language therapists communication practices in dysphagia management.

BACKGROUND: Communication between speech-language therapists (SLTs) and nursing personnel is essential for safe dysphagia management, as unclear exchanges can compromise patient outcomes and care continuity. In South Africa, high patient loads, limited dysphagia training, and linguistic diversity create additional challenges, making effective interprofessional communication crucial. OBJECTIVES: This study aimed to identify communication practices related to dysphagia in exchanges between SLTs and nursing personnel, focusing on mutual understanding, openness in interactions and frustration exp

Community-acquired pneumonia caused by Kluyvera intermedia: A rare case report.

BACKGROUND: Kluyvera intermedia is a rare Gram-negative bacillus that predominantly functions as an opportunistic pathogen. Commonly found in environmental sources such as freshwater and soil, it has been infrequently reported in human infections, often leading to underrecognition in clinical practice. Because it can resemble more prevalent pathogens, accurate identification is crucial for effective management of infections. CASE REPORT: We report a case of pneumonia caused by Kluyvera intermedia in a 55-year-old male with a 10-year history of well-controlled hypertension. The patient initial

Isolated Ruptured Pulmonary Hydatid Cyst Mimicking Community-Acquired Pneumonia.

Hydatid disease, most commonly caused by Echinococcus granulosus, usually affects the liver, and isolated pulmonary involvement is rare. Ruptured lung cysts can mimic pneumonia, often delaying diagnosis. Although surgery is the standard treatment, medical therapy alone has occasionally been successful. We report a 42-year-old healthy Lebanese man who presented with fever and elevated inflammatory markers suggestive of pneumonia that did not respond to broad-spectrum antibiotics. Imaging revealed right lower lobe consolidation accompanied by bilateral patchy ground-glass opacities. Additionally

Neurostimulation improves reading and alters communication within reading networks in dyslexia.

The left temporo-parietal cortex (TPC) is critical for phonological decoding during reading and appears hypoactive in dyslexia. Therefore, a promising approach to alleviating phonological deficits in dyslexia is to modulate left TPC functioning. However, it is unclear how neurostimulation alters activity and network interactions in dyslexia. To address this gap, we combined facilitatory transcranial magnetic stimulation (TMS) to the left TPC in adults with dyslexia with an overt word and pseudoword reading task during functional neuroimaging. We found TMS-induced improvements in pseudoword rea

Explanatory capacity of measures of community context for paediatric injury hospitalisations in the USA.

OBJECTIVE: Community context influences children's risk for injury. We aimed to measure the explanatory capacity of two ZIP code-level measures-the Child Opportunity Index V.3.0 (COI) and median household income (MHHI)-for rates of paediatric injury hospitalisations. METHODS: This was a retrospective cross-sectional population-based study of children living in 19 US states in 2017. We examined injury hospitalisation rates for three categories: physical abuse among children <5 years, injuries suspicious for abuse among infants <12 months and unintentional injuries among children <18 years. Community Disadvantage Is Associated With More Severe Motor Symptoms in Parkinson Disease.

BACKGROUND AND OBJECTIVES: The relationship between socioeconomic factors and Parkinson disease (PD) is unclear. Previous literature suggests a potential disconnect between the effect of socioeconomic status (SES) on PD risk and severity. A recent study found that people with PD in the United States were more likely to come from well-resourced communities. Multiple possible explanations were proposed, including that lower SES could be protective against PD risk. Other studies have found worsened PD symptoms and outcomes associated with lower individual SES. If environmental factors associated

Antibiotics for Paediatric Community-Acquired Pneumonia: What is the Optimal Course Duration?

Despite significant global reductions in cases of pneumonia during the last 3 decades, pneumonia remains the leading cause of post-neonatal mortality in children aged <5 years. Beyond the immediate disease burden it imposes, pneumonia contributes to long-term morbidity, including lung function deficits and bronchiectasis. Viruses are the most common cause of childhood pneumonia, but bacteria also play a crucial role. However, the optimal duration of antibiotic therapy for bacterial pneumonia remains uncertain in both low- and middle-income countries and in high-income countries. Knowing the op

Mechanistic prediction of community composition across resource conditions and species richness.

Predicting species coexistence and community assembly is a central goal in ecology. Traditional methods, based on the effect of one species on another (e.g., Lotka-Volterra competition model), are sensitive to environmental context. This is because they ignore the fundamental processes that can be applied across environments. While mechanistic approaches offer promise, empirical tests remain rare. Here, we integrate a mechanistic consumer-resource model with the growth of 12 phytoplankton species in monoculture over a range of nitrate, ammonium or phosphorous concentrations. We find that the m

Agentic and Communal Collective Narcissism: Their Differential Impact on Foreign Policy Preferences.

As a personality trait, collective narcissism refers to unrealistic beliefs about ingroup exceptionality. Previous literature has primarily focused on the link between agentic collective narcissism and foreign policy preferences, while largely neglecting the role of communal collective narcissism. Building on the theoretical distinction between agentic and communal collective narcissism, this study is the first to clarify their distinct associations with three forms of foreign policy preferences: offensive realism, defensive realism, and liberalism. We recruited 698 Chinese participants aged 1
