Anterior stabilization for unstable traumatic thoracolumbar spine burst fractures.

BACKGROUND: Traumatic injuries to the thoracolumbar spine result in a high incidence of unstable fractures. The goal of the surgical management is to achieve an adequate decompression and stabilization. We have analyzed operative and postoperative features of anterior surgical approaches. METHODS: We retrospectively analyzed the medical records of 45 patients that presented with traumatic unstable thoracolumbar fractures who underwent anterior corpectomy with stabilization from January 1999 to January 2009. The neurological status of the patient was defined using the American Spinal Injury As

Omega-3 and fish oil supplements do not cause increased bleeding during spinal decompression surgery.

STUDY DESIGN: Retrospective case-control study. OBJECTIVE: The purpose of this study was to assess whether preoperative use of fish oil supplements increases intraoperative blood loss and postoperative bleeding complications during lumbar decompression surgery. SUMMARY OF BACKGROUND DATA: Omega-3 fatty acids (n-3FA) are widely used as over-the-counter supplements because of well-established cardioprotective and antiplatelet effects. Concern over bleeding associated with changes in platelet function have led to prohibiting these supplements before surgery although there are no clinical data a

The Impact of Time-to-Intervention on Mortality and Functional Outcome in Acute Subdural Hematoma: A Systematic Review and Meta-Analysis.

Introduction: Acute subdural hematoma (aSDH) represents one of the most lethal and debilitating forms of traumatic brain injury (TBI) encountered in neurosurgical emergency settings, carrying high mortality rates and severe long-term functional deficits. Historically, the timing of surgical decompression has been considered a critical modifiable prognostic factor. However, contemporary evidence regarding the relationship between time to surgery and clinical outcomes remains inconsistent. This systematic review and meta-analysis evaluates the impact of surgical timing on mortality and functiona

Surgical treatment of craniofacial fibrous dysplasia in adults.

Craniofacial fibrous dysplasia (FD) is a rare disorder that may require neurosurgical expertise for definitive management; however, surgical management of FD in adult patients is uncommon. Although other therapies have been shown to slow progression, the only definitive cure for adult craniofacial FD is complete resection with subsequent reconstruction. The authors review the biological, epidemiologic, clinical, genetic, and radiographic characteristics of adult FD, with an emphasis on surgical management of FD. They present a small series of three adult patients with complex FD that highlight

Surgical complications of microendoscopic procedures for lumbar spinal stenosis.

Microendoscopic discectomy (MED) is one of the minimally invasive endoscopic procedures for treating lumbar disc herniation. We have applied MED techniques to posterior decompression procedures for treating lumbar spinal stenosis (LSS). In the present study, we examined the surgical complications in 114 consecutive patients surgically treated with MED procedures for LSS. Intraoperative complications occurred in 9 patients. Six patients (5.3%) experienced a dural tear, and three (2.6%) had a fracture of an inferior facet. Early postoperative complications occurred in 13 patients. Twelve patient

Osteonecrosis of the humeral head: trends in surgical management of a nationwide sample from 2010 to 2021.

BACKGROUND: The humeral head is the second most common site for osteonecrosis (ON) but its epidemiology is poorly described. This study aimed to better understand its treatment in the United States by 1) evaluating total operative procedures with rates normalized to the annual surgical volume; 2) determining trends of nonjoint preserving (shoulder arthroplasty) vs. joint preserving procedures; and 3) quantifying rates of operative techniques in different aged cohorts (<50 vs. ≥50 year old). METHODS: A nationwide database was queried for patients with humeral ON who underwent surgery between J

Intraneural ganglion cyst of the common peroneal nerve causing foot drop in a 12-year old child.

OBJECTIVES: Intraneural ganglia are benign fluid-filled cysts contained within the subepineurial space of peripheral nerves. The common peroneal nerve at the fibular neck is by far the most frequently involved, although other nerves can be affected as well. Although the differential diagnosis of foot drop in adults and children show some differences, clinical presentation, diagnostic workup, treatment and follow-up of intraneural ganglia are quite similar in both groups. The primary objective was to create an overview of intraneural ganglia in children, with an emphasis on diagnostic workup an

Case Report: Glossopharyngeal neuralgia secondary to syncope: insights into microvascular decompression treatment.

This article reports a case of a 56-year-old male patient with glossopharyngeal-vagal nerve syndrome. The main clinical manifestations were paroxysmal, sharp, knife-like pain in the right root of the tongue and pharynx, which occurred synchronously with syncope without any premonitory dizziness or nausea during pain episodes. Preoperative imaging suggested neurovascular compression, with the right posterior inferior cerebellar artery traversing and compressing the rootlets of both the glossopharyngeal nerve and proximal vagus nerve. The patient underwent a right retrosigmoid craniotomy for mic

Anterior intradural CSF collection causing postoperative neurological deterioration after intramedullary tumor resection and associated syringomyelia: illustrative case.

BACKGROUND: CSF-related complications such as CSF leakage and meningitis are well recognized after intramedullary spinal cord tumor surgery. In contrast, postoperative localized ventral intradural CSF collection causing acute spinal cord compression has not been well described. OBSERVATIONS: A 62-year-old man with an intramedullary spinal cord tumor extending from C6 to T2 and associated extensive syringomyelia underwent tumor resection with posterior decompression and fusion. Gross-total resection was achieved under intraoperative neurophysiological monitoring without immediate neurological

Endoscope-Assisted Microvascular Decompression for Hemifacial Spasm: A Consecutive Series Using a Hybrid Visualization Strategy.

BACKGROUND: Microvascular decompression (MVD) is the most effective treatment for hemifacial spasm (HFS). However, microscopic visualization alone may insufficient for assessing the neurovascular relationship, and the optimal role of endoscopy remains unsettled. The purpose of this study was to evaluate the feasibility, safety, and outcomes of endoscope-assisted MVD (EA-MVD) for HFS. METHODS: A retrospective review was performed of 200 consecutive patients who underwent EA-MVD between January 2019 and June 2021. The endoscope was used to inspect the neurovascular relationship before and after

Acute Pediatric Hand Compartment Syndrome Following Metacarpal Fractures: A Case Report.

Acute compartment syndrome (ACS) of the hand is a rare but potentially devastating condition in children, particularly in young patients, in whom diagnosis may be difficult due to unreliable clinical signs. We report the case of a three-year-old girl admitted following bicycle-wheel trauma to the hand, with fractures of the bases of the second and third metacarpals. Progressive pain, worsening edema, and increasing compartment tension raised suspicion for ACS. Urgent fasciotomy was performed approximately two hours after admission using two dorsal incisions over the second and fourth metacarpa

Intraoperative venous air embolism during endoscopic transsphenoidal surgery: recommendations for management. Illustrative case.

BACKGROUND: Venous air embolism (VAE) is a rare neurosurgical complication that occurs when air is trapped in a large, exposed vein and there is a pressure gradient that drives the air embolus from the surgical site to the right heart. This case highlights the identification and management of intraoperative VAE during transsphenoidal surgery. Additional discussion is provided on pathophysiology and best practices for reducing risk of occurrence. OBSERVATIONS: A 53-year-old female presented with several days of severe headaches and acute visual loss. Initial workup revealed a hemorrhagic supra

Hyperbaric Treatment of Air or Gas Embolism: Current Recommendations.

Gas can enter arteries (arterial gas embolism) due to alveolar-capillary disruption (caused by pulmonary overpressurization, e.g. breath-hold ascent by divers), veins (venous gas embolism, VGE) as a result of tissue bubble formation due to decompression (diving, altitude exposure), or during certain surgical procedures where capillary hydrostatic pressure at the incision site is subatmospheric. Both AGE and VGE can be caused by iatrogenic gas injection. AGE usually produces stroke-like manifestations, such as impaired consciousness, confusion, seizures, and focal neurological deficits. Novel Concepts in Headache Surgery: Case Report Incorporating Interdisciplinary Care for Complex Head and Neck Pain with Cervical Dystonia.

Complex cases of head and neck pain may involve overlapping disorders such as occipital neuralgia, chronic migraine, cervical dystonia, peripheral and central sensitization, and complex regional pain syndrome of the head. In these cases, traditional nerve decompression techniques may not provide adequate symptom relief, and advanced multidisciplinary treatment involving conservative, minimally invasive, and surgical measures may be necessary. This case report illustrates novel concepts in headache surgery that have not been previously described to address complex pain in the head and neck regi
