To determine the cellular and subcellular reactions of keratinocytes at contact dermatitis, transmission electron microscopy was used in combination with energy dispersive X-ray microanalysis. Nickel is the most common cause of allergic contact dermatitis in females. The dermatitis can be maintained both by direct contact and by ingestion of nickel. In 9 out of 17 patients suffering from dermatitis, a diet with a low nickel content has improved their condition. Eleven patients with chronic nickel hand dermatitis were given a daily dosage of 200 to 400 mg of tetraethylthiuramdisulfide (Antabuse). We describe a quantitative method for the grading of contact allergic reactions in guinea pigs. These reactions are characterized by marked cellular infiltration, and the method is based on total and differential counting of cells in the upper corium. A varying and objectively gradable increase in mononuclear and basophil polymorphonuclear cells was found. In naked-eye-positive reactions this increase was highly significant 24, 48, and 72 hours after epicutaneous application of dinitrochlorobenzene (DNCB). Staphylococcus aureus (60 p. 100) and Candida albicans (50 p. 100) were mostly found in 73 cases of diaper dermatitis. They were present together in 16 out of these 73 cases. This study enables us to think this is a matter of opportunistic infection. Staphylococcus aureus (75 p. 100) and beta-hémol. Streptococcus A (28 p. 100) were found in 40 children with impetigo contagiosa. They were present together in 7 out of the 40 cases. Nephritis was never found either in those cases or in other 93 cases of pyodermitis.

A male patient with unusual clinical features of dermatitis herpetiformis is reported. The eruption consisting mainly of erythematous papules developed symmetrically around the mouth and later around the eyes, and has been circumscribed within these areas for 4 years. The dermatitis herpetiformis nature of the lesion was confirmed by direct immunofluorescent testing.

The association between antigens of the HLA system and dermatitis herpetiformis (DH) using in vivo-bound IgA as an added criteria for diagnosis is reexamined. Twenty-eight of 33 (85%) immunologically proven patients with DH were HLA-B8 positive, whereas only 17 of 92 (18%) controls were HLA-B8 positive.

The development of skin lesions in patients with dermatitis herpetiformis after the withdrawal of their dapsone therapy was studied with the electron microscope. In control biopsies from patients prior to cessation of treatment, membrane-bound vacuoles were found beneath the basal lamina of the epidermis as previously described.
