A novel allele of myosin VIIa reveals a critical function for the C-terminal FERM domain for melanosome transport in retinal pigment epithelial cells.

Mutations in the head and tail domains of the motor protein myosin VIIA (MYO7A) cause deaf-blindness (Usher syndrome type 1B, USH1B) and nonsyndromic deafness (DFNB2, DFNA11). The head domain binds to F-actin and serves as the MYO7A motor domain, but little is known about the function of the tail domain. In a genetic screen, we have identified polka mice, which carry a mutation (c.5742 + 5G > A) that affects splicing of the MYO7A transcript and truncates the MYO7A tail domain at the C-terminal FERM domain. In the inner ear, expression of the truncated MYO7A protein is severely reduced, leading

Endocochlear potential depends on Cl- channels: mechanism underlying deafness in Bartter syndrome IV.

Human Bartter syndrome IV is an autosomal recessive disorder characterized by congenital deafness and severe renal salt and fluid loss. It is caused by mutations in BSND, which encodes barttin, a beta-subunit of ClC-Ka and ClC-Kb chloride channels. Inner-ear-specific disruption of Bsnd in mice now reveals that the positive potential, but not the high potassium concentration, of the scala media depends on the presence of these channels in the epithelium of the stria vascularis. The reduced driving force for K(+)-entry through mechanosensitive channels into sensory hair cells entails a profound

Transfer of accelerated presbycusis by transplantation of bone marrow cells from senescence-accelerated mice.

Until now, there has been no effective therapy for chronic sensorineural hearing impairment. This study investigated the role of bone marrow cells (BMCs) in cochlear dysfunction. BALB/c mice (2 months of age), a non-presbycusis-prone mouse strain, were lethally irradiated and then transplanted with BMCs from SAMP1 mice (2 months of age), a presbycusis-prone mouse strain. Acceleration of age-related hearing loss, early degeneration of spiral ganglion cells (SGCs) and impairment of immune function were observed in the recipient mice as well as in the SAMP1 mice. However, no spiral ganglion cells

Mutations in the PDS gene in German families with Pendred's syndrome: V138F is a founder mutation.

Pendred's syndrome, an autosomal-recessive condition characterized by congenital sensorineural hearing loss and goiter, is caused by mutations in the PDS gene. Located on chromosome 7q22-q31, it encodes a chloride-iodide transporter expressed in the thyroid, inner ear, and kidney. We investigated the PDS gene of six affected individuals from four unrelated families with Pendred's syndrome by direct sequencing. PDS mutations were identified in homozygous or compound heterozygous state in all six cases. A homozygous missense mutation leading to the amino acid substitution S133T was detected in a

Long-term vestibulo-ocular reflex changes in patients with vestibular ablation.

Long-term changes in the parameters of the vestibulo-ocular reflex (VOR) were evaluated using recently developed multilevel statistical modelling techniques in a group of patients who had undergone ablative inner ear procedures. Data from 84 patients were available for study. The most dramatic recovery in the VOR occurred during the first 60 postoperative days, however, slow changes continued, levelling off at about 120 days. While frequency was a highly significant predictor of outcomes in all parameters, age showed significant predictive value only to phase measures (p = 0.006) and gender ha

A possible correlation between vertebral artery insufficiency and degenerative changes in the cervical spine.

We studied 130 patients, aged 20 to 81 years, with symptoms of tinnitus, vertigo or dizziness. Radiological examinations revealed degenerative changes in the cervical spines of all patients such as discopathy or osteophytes. Head and neck and neurological examinations ruled out other symptoms apart from vertebrobasilar artery flow insufficiency. The vertebrobasilar arteries were examined by means of a color Doppler ultrasonograph using duplex scanning. The correlation coefficient (CC) defining the relationship between the number of patients with abnormal blood flow and the total number of pati

The cytoskeleton and disease: genetic disorders of intermediate filaments.

Specialized cytoskeletons play many fascinating roles, including mechanical integrity and wound-healing in epidermal cells, cell polarity in simple epithelia, contraction in muscle cells, hearing and balance in the inner ear cells, axonal transport in neurons, and neuromuscular junction formation between muscle cells and motor neurons. These varied functions are dependent upon cytoplasmic networks of actin microfilaments (6 nm), intermediate filaments (10 nm) and microtubules (23 nm), and their many associated proteins. In this chapter, I review what is known about the cytoskeletons of interme

Massive nerve root enlargement in chronic inflammatory demyelinating polyneuropathy.

OBJECTIVE: To report three patients with chronic inflammatory demyelinating polyneuropathy (CIDP) presenting with symptoms suggestive of cervical (one patient) and lumbar root disease. METHODS: Nerve conduction studies, EMG, and nerve biopsy were carried out, having found the nerve roots to be very enlarged on MRI, CT myelography, and at surgery. RESULTS: Clinically, peripheral nerve thickening was slight or absent. Subsequently one patient developed facial nerve hypertrophy. This was mistaken for an inner ear tumour and biopsied, with consequent facial palsy. Neurophysiological tests sugges

Otosclerosis: a measles virus associated inflammatory disease.

The etiology of otosclerosis is still unknown. Immunohistochemical studies have revealed the characteristics of chronic inflammation in the otospongiotic area. Paramyxoviral structures have been identified by electron microscopy and the expression of measles virus antigen has been observed by immunohistochemistry in active otosclerotic tissue. By use of the polymerase chain reaction, measles virus related sequences have been detected in otosclerotic bone tissue but not in control specimens. The aim of our study was i) to detect measles virus genome in affected patients using a more sensitive P

The relationship between vasopressin and endolymphatic hydrops in the guinea pig.

Clinical studies have shown that plasma vasopressin level is significantly elevated in patients with Meniere's disease. Other reports indicated that histamine induced a very quick and high elevation of vasopressin level and caused nystagmus in experimentally produced endolymphatic hydrops. We became interested in further investigating the details of this relationship by studying the effect of experimental endolymphatic hydrops and histamine upon plasma vasopressin level in the guinea pig. The results are as follows: 1) Histamine increased the plasma vasopressin level in normal guinea pigs. 2)

Acute simultaneous bilateral vestibulocochlear impairment in neuro-Behçet's disease: a case report.

The central nervous system (CNS) is often a preferential target of Behçet's disease and diverse neurological manifestations have been described. Isolated hearing impairment or disequilibrium is also known to occur, but the simultaneous bilateral impairment of both vestibular and cochlear functions has been only rarely documented in Behçet's disease. A 52-year-old Japanese woman with neuro-Behçet's disease had meningoencephalitis, a profound thrombocytopenia, and bilateral vestibulocochlear impairment during the acute exacerbation. Although the CNS involvement was apparent, the clinical history

New approach for operative management of vascular lesions of the infratemporal internal carotid artery.

Direct operative management of aneurysms of the internal carotid artery is associated with significant morbidity. Preauricular surgical approaches to this area can result in violation of the inner ear, temporomandibular joint, or mandibular ramus, and cranial nerves V, VII, IX, X, XI, and XII. A new technique for saphenous vein bypass of the infratemporal and petrous internal carotid artery, using a postauricular curvelinear groove drilled in the mastoid cortex, is described. Proximally, the reverse vein graft is anastomosed to either the internal or external carotid artery, whereas the distal

CO2 laser for otosclerosis: safe energy parameters.

Safe energy parameters for each Sharplan CO2 laser model (734, 1040, 1100A) were established in the laboratory. Ultrasensitive pyroelectric detectors analyzed the precise energy package delivered to the operative field with each of these power setting. Subsequently, 0.6-mm stapedotomy and stapedectomy revisions were performed under simulated operating room conditions while measuring temperature changes in the vestibule with a thermocouple. Table I illustrates safe energy settings for each of the Sharplan CO2 models tested. These power settings produced no more than 0.3 degrees C temperature ri

Uni-lateral hearing-loss with acoustic neuroma patients: electrocochleographic findings.

38 patients with proven acoustic neuroma were examined. Intratympanic electrocochleography was performed with alternating clicks of 90 dB (nHL) intensity and a stimulus rate of 20 per second; 500 sweeps were averaged. Widening of the Cochlear Action Potential (CAP)-complex by a mean of 1.6 ms was the most striking finding. Summating potentials (ratio of SP-amplitude to CAP-amplitude) were enlarged to 0.6, compared to 0.2 in healthy persons and 0.8 in patients suffering from Meniere's disease. The most noticeable result was the loss of amplitude using very short interstimulus intervals. With a

Neurotological studies upon intoxication by organic mercury compounds.

In 1965, a strange illness, which afterwards was diagnosed as the intoxication caused by organic mercury compounds, began to affect the inhabitants of the riverside areas of the Aganogawa River, Niigata, Japan. The authors have investigated the neurotological disturbances, which occurred in 144 cases intoxicated with organic mercurials. The results obtained are summarized as follows: (1) Recruitment (in SISI and other tests) was negative in all the cases except for 32 with inner ear deafness, and Békèsy's AUDIOMETRY (TTS) showed type II: unsettled wide amplitude in 17 cases and severe abnormal

Hearing loss associated with CDC42 in mice and humans (Takenouchi-Kosaki syndrome): CDC42 and RHOQ synergistically function in cochlear hair cells.

CDC42 is involved in multiple signaling pathways, including actin organization and polarity. We previously reported progressive sensorineural hearing loss (SNHL) in inner ear hair cell (HC)-specific Cdc42-knockout (Atoh1-Cre+/-;Cdc42flox/flox) mice. However, the phenotype was milder than expected, suggesting possible redundancy with other Rho-family GTPases. Thereafter, Takenouchi-Kosaki syndrome (TKS), caused by de novo CDC42 mutations and manifesting as SNHL, was reported, in which the p.Y64C mutation was speculated to be constitutively active. However, the relationship between CDC42 status

Advances towards biological hearing restoration by genetic reprogramming.

PURPOSE OF REVIEW: Advances in genetic reprogramming and groundbreaking first-in-human gene therapy trials have generated significant momentum for treating sensorineural hearing loss. This review evaluates the expanding potential of biological approaches for hearing restoration. RECENT FINDINGS: The mammalian organ of Corti possesses limited natural regenerative capacity. Recent studies are attempting to restore hearing by genetically reprogramming supporting cells into hair cells. While merely modulating cell signaling pathways in the inner ear is currently insufficient to regenerate hair ce

Traumatic brain injury impact on normal hearing.

Traumatic brain injury (TBI) incidents bring drastic health adversities in multiple systems of the body. The immediate impact on the head and central nervous system is promptly accelerated to the autonomous and peripheral nervous system. It causes an instant variability in intracranial pressure, heart rate, blood pressure, normal function of chemo/baro receptors, and sudden failure in normal communication. These effects promote long-term secondary health adversity, such as nerve schwannoma, gliosis, Wallerian degeneration, mitochondrial dysfunction, change in blood-brain barrier pathology, and
