CASE: Although congenital nonunion of the clavicle is a well-known pathology in children, posttraumatic nonunion is a rare entity. Nonunion after open reduction and internal fixation of a clavicle fracture is exceptional in the pediatric population. We report on a patient presenting posttraumatic pseudarthrosis of the clavicle after surgical treatment. Pseudarthrosis was resected, and the defect was bridged with an iliac crest autograft, supplemented with bone morphogenetic protein, and fixed with an elastic stable intramedullary nail.
CONCLUSION: This procedure offered enough stability for a

Aortic root dilatation is a well-known complication in patients with congenital aortic valve malformation, tetralogy of Fallot, or a double outlet right ventricle. We report two rare patients who underwent composite graft replacement of the aortic root with a mechanical valve, the so-called Bentall-type operation, after Fontan completion. The pathological examination on the resected aortic wall revealed mucoid degeneration in tunica media and elastic fiber fragmentation. Our report emphasizes the need for close observation of these patients over a long-term period.

Annular elastolytic giant cell granuloma (AEGCG) is a rare granulomatous skin disease characterized clinically by annular plaques with elevated borders and atrophic centers found mainly on sun-exposed skin and histologically by diffuse granulomatous infiltrates composed of multinucleated giant cells, histiocytes and lymphocytes in the dermis along with phagocytosis of elastic fibers by multinucleated giant cells. We report a case of AEGCG in a 50-year-old woman and is highlighted for the classical clinical and histological findings of the disease and its rare co-existence with Hashimoto's thyr

Pseudoxanthoma elasticum (PXE) is a disorder characterized by progressive degeneration of elastic fibers and ectopic mineralization. Cutaneous manifestations are a hallmark of this disease and organs that may be affected by the disease process include cardiovascular, central nervous system, ocular and gastrointestinal systems. Oral manifestations of PXE have been previously reported as intramucosal nodules observed on various intraoral surfaces. We present a case of a 46-year-old female with PXE diagnosed with temporomandibular disorder (TMD). To our knowledge, this is the first report of a pa

A multidisciplinary therapy used to treat a patient with a severe mucogingival deformity in the area of maxillary anterior teeth is presented. The simple technique uses an interocclusal appliance and elastic band for orthodontic extrusion to correct the mucogingival deformity and develop an implant site. This approach is designed to increase the amount of keratinized tissue and new alveolar bone, which are necessary for the development of an implant site. Clinical and radiographic examinations at the 5-year follow-up revealed a successful tooth replacement and improved esthetic appearance that

Granuloma annulare (GA) is a noninfectious necrobiotic granuloma, which can present as localized or generalized patterns. We present a case of generalized GA with multiple large open comedones over the lesions in a woman with hypothyroidism and diabetes mellitus. GA is associated with collagen and elastic-fibre damage, which may lead to loss of follicular support and resultant open comedone formation, analogous to solar elastosis or senile comedones. The rapid recovery of our patient after treatment with of oral hypoglycaemics supports the role of insulin resistance as a predisposing factor fo

A newborn male Romagnola calf, who had died a few hours after birth, was submitted for postmortem examination. Necropsy revealed a 23 cm × 22 cm × 5 cm, pale pink, lobulated, elastic, partially fluctuant mass that protruded from the dorsal retroperitoneal space into the abdominal cavity, extending from the diaphragm to the left kidney. The mass consisted of mature pulmonary tissue and was consistent with a pulmonary choristoma. The gross and microscopic appearance of this rare tumor-like congenital lesion and the possible pathogenesis are discussed.

Arytenoid subluxation is a rare laryngeal injury that may follow instrumentation of the airway and present as hoarseness, vocal fatigue, stridor, dysphagia, odynophagia and sore throat. We report the case of an 88-year-old man with type 2 diabetes mellitus who developed this complication during a difficult intubation where a Macintosh laryngoscope and gum elastic bougie were used to facilitate intubation. Previously considered to play a minor role in treatment, voice therapy was used successfully in this patient to correct subluxation of the arytenoid, with prompt resolution of his symptoms.

The article deals with pathophysiology of disturbed nasal breathing, tools of surgical correction of deviated nasal septum, features of the course of wound process. Clinical efficacy of stent fixation of the septum and conventional elastic tamponade was compared in 38 patients who had undergone septoplasty and submucous radiowave disintegration of the lower nasal turbinate bones for deviation of the nasal septum and a neurovegetative form of chronic vasomotor rhinitis. Higher efficacy of intranasal stenting is shown. Tamponadeless management of the patients after surgical interevention on the

There has been progress made in the understanding of 3 Mendelian disorders: pseudoxanthoma elasticum, cutis laxa, and lipoid proteinosis cutis and mucosae. While they are primary connective tissue diseases, their names imply a connection to the skin, and in fact, it is often the dermatologist who makes the diagnosis. It seems rational that defects in various extracellular matrix proteins cause lipoid proteinosis or subtypes of cutis laxa, yet the discovery of a liver- and kidney-based transmembrane transporter as the culprit of pseudoxanthoma elasticum was rather surprising and may shed new li

Solar elastosis is a recognized manifestation of chronic sun exposure affecting mainly Caucasian skin. Recognizable clinical manifestations of solar elastosis include cutis rhomboidalis nuchae, Favre-Racouchot syndrome, actinic comedonal plaque, nodular forms such as elastomas, elastic nodule of the ear and elastotic bands, collagenous plaques of the hand, and colloid milia. We report two patients with an unusual presentation of vesicular lesions clinically and histopathologically situated in zones of severe solar elastosis that has been previously referred to as bullous solar elastosis.

Mandibular widening by distraction osteogenesis has recently been shown to be a very predictable, successful, and stable procedure with limited morbidity. One indication for mandibular widening by distraction osteogenesis is a Brodie bite (scissors or buccal crossbite). This case report demonstrates a technique used to treat a 12-year-old girl with a true unilateral buccal crossbite (no functional shift). It details how distraction osteogenesis can successfully treat unilateral problems with a distractor, a splint, and elastics. Details on this treatment technique have not been previously publ

Elephant tusk removal usually requires costly surgical procedures that are time-consuming and present a significant risk to the animal when performed using general anesthesia. Such techniques require gauges, chisels, and forceps to remove the tusk. This article reports the simple removal of the tusk of an 18-yr-old African elephant (Loxodonta africana) without the use of surgical instruments and anesthesia. Rubber elastics were placed around a tusk, causing loss of alveolar bone with subsequent exfoliation of the tusk within 3 wk. The healing process was uneventful.

Lymphedema are primary (lower limb) or secondary for the upper limb after treatment of breast cancer. Genetic research allows to explain better pathophysiology of lymphedema. Lymphoscintigraphy is the main imaging technique for lymphedema. Venous or compressive etiologies may be first excluded. Treatment of lymphedema includes decongestive physiotherapy: manual lymph drainages, bandages, elastic stockings, exercises, skin care to avoid infections. Other treatments may be associated: weight loss, drugs (procyanidolic oligomers), surgery for genital lymphedema, pneumatic compression pumps for ly

Acrocyanosis is an acrosyndrome frequently found among adolescent or young women. Four clinical symptoms are necessary and sufficient to establish the diagnosis: permanent and painless cyanosis of extremities, local hypothermia, permanent sweatiness, and elastic infiltration of the integument. In practice, only one investigation is useful but not indispensable: capillaroscopy which visualizes capillarovenular stasis. Numerous clinical forms have been described, but they are exceptions, but for supramalleolar erythrocyanosis. Treatment is mainly preventive: protection against cold. Two conditio

Patients with dentoalveolar asymmetries can present some of the most biomechanically challenging situations to the orthodontist. One creative approach for managing dental asymmetries is to extract a combination of teeth that will simplify intra-arch and interarch mechanics. Often this will also reduce the dependency on patient compliance for elastic wear and may even shorten treatment time. Atypical extraction patterns can also be beneficial in the presurgical orthodontic preparation of orthognathic surgery patients. Multiple diagnostic scenarios are discussed, and various treatment plans invo

The case history is presented of a 61 year old man with von Recklinghausen's disease who developed a spontaneous haemothorax. In spite of being asymptomatic for five days after drainage, he died as a result of fatal sudden re-bleeding. The post mortem examination showed dissection and rupture of the left subclavian artery. Microscopically, disarrangement of smooth muscle and decrease of elastic fibre was observed in the ruptured artery. Haemothorax in patients with von Recklinghausen's disease may require thoracotomy, even if the condition of the patient appears to be stable.

Children with Marfan syndrome rarely undergo surgery for annuloaortic ectasia and aortic regurgitation in the first decade. A 7-year-old girl presented with congestive heart failure due to severe aortic regurgitation associated with annuloaortic ectasia (6 cm). She also had funnel chest. She underwent a Bentall operation and sternal turn-over with a satisfactory result. Since the aortic valve cusps had rolled edges, the aortic valve was not spared. Histology of the aortic valve cusps showed myxoid degeneration and fragmentation of elastic fibers.

Granulomatous slack skin disease (GSS) is a rare disorder characterized by bulky cutaneous lesions and epithelioid and giant cell granulomas with destruction of the dermal elastic tissue. We detail the observation of a 29-year-old man with clinical and histological features of GSS. Pendulous skin tumors were associated with typical clinical and immunohistochemical aspects of mycosis fungoides and with clonal rearrangement of the V gamma T-cell receptor gene in lesional skin. This case report supports cutaneous T-cell lymphoma as a cause of GSS.

The present report shows the experience of the authors in the treatment of esophageal variceal veins with a new technique: Endoscopic band ligation of esophageal varices. This new procedure developed recently in the western world has been introduced in Perú, thanks to one of the authors who brought the Stiegmann Goff elastic band ligation device. The results of its use in cirrhotic patients with acute variceal hemorrhage and with large esophageal varices has been very effective, getting hemostasis and obliteration of esophageal varices. Very few complications were observed.
