Aneurysm of the vein of Galen is a rare congenital disease, with a generally poor prognosis. It represents less than 1% of cerebral arteriovenous malformations. Intravascular lymphoma (IVL) is an uncommon neoplastic disorder characterized by monoclonal intravascular expansion of lymphoid cells. Gastric vascular ectasias are rare lesions of the gastric antrum that may be the cause of iron deficiency anemia in the elderly. We report a case of proximal gastric vascular ectasias in a young man which caused rapidly fatal hemorrhage. This case is instructive because of the patient's youth, the location of the lesions, and the rapidly fatal hemorrhage.

A premature infant with fatal zygomycotic necrotizing cellulitis is reported. Zygomycosis occurs as either an indolent or an acute fulminating form. Cutaneous zygomycosis is a fulminant form of the disease and affects mainly immunocompromised patients. Idiopathic retroperitoneal fibrosis (RPF) is a disease entity rarely encountered by the general surgeon. In most cases, ureteral stricture is the underlying problem requiring lysis of fibrotic adhesions. However, infrequently, the gastrointestinal tract may become involved. The following report describes the complications of intestinal obstruction by RPF in one of our patients. The discussion then focuses on the need for early diagnosis and treatment of gastrointestinal invasion by RPF.

Extrapulmonary involvement with Pneumocystis carinii has been described in 0.5%-2.5% of persons with AIDS. One hundred nine patients with AIDS and confirmed extrapulmonary pneumocystosis were identified, and seven of these patients (including our patients) had central nervous system (CNS) pneumocystosis. We report on a young man with fibromuscular dysplasia involving the basilar artery detected at autopsy. He presented with sudden onset of stroke, and the lesion was complicated by thrombosis and dissection of the vessel wall. The organizing thrombotic lesion of the basilar artery was responsible for the ventral pontine infarct that resulted in "locked-in syndrome."

Two previously healthy young adults came to our community hospital with rapidly progressive respiratory failure. Investigation confirmed Blastomyces as the responsible etiologic agent. Despite adequate antifungal chemotherapy and intensive supportive care, both patients died, one within 24 hours and the other after 14 days. Overwhelming infection with Blastomyces dermatitidis can cause acute respiratory failure, possibly the adult respiratory distress syndrome, even in immunocompetent hosts.

Microangiopathic hemolytic anemia and thrombocytopenia have been reported in patients with primary pulmonary hypertension, but not in patients with congenital heart disease even if accompanied with pulmonary hypertension. We present a 7-year-old boy with atrial septal defect and pulmonary hypertension who developed microangiopathic hemolysis and thrombocytopenia. Microangiopathic hemolytic anemia and thrombocytopenia should be remarked as a complication in patients with congenital heart disease.
