Plating of the Pelvis and Acetabulum via the Laparoscopic Technique: An Alternative Method for ORIF.

INTRODUCTION: Open reduction and internal fixation (ORIF) is a standard treatment for pelvic and acetabular fractures, but it is associated with great trauma and a high incidence of complications. The purpose of this retrospective study was to investigate whether laparoscopy can be used as an alternative to standard ORIF in the treatment of pelvic and acetabular fractures. MATERIALS AND METHODS: This study included 15 patients with pelvic and acetabular fractures who underwent laparoscopy. The surgical time, intraoperative blood loss, and surgery-related complications of all the patients were

Numerical Analysis of Postoperative Rehabilitation of Femoral Neck Fracture: Coupling of Vascular Growth, Bone Remodeling, and the Effect of Electrical Stimulation.

Cannulated screws are often used for internal fixation after a femoral neck fracture to ensure the smooth rehabilitation of patients. Patients frequently have various complications, such as avascular necrosis, during the recovery period. Reasonable rehabilitation training after surgery is necessary for patient rehabilitation. Multiple studies have proved a relationship between bone growth repair and blood vessel growth, and the blood supply system provides various oxygen and nutrients for bone remodeling to promote the smooth progress of bone remodeling. As a new therapeutic method, electrical

Case Report: A case of surgical and enzyme replacement therapy for type I Gaucher disease complicating femoral shaft pathological fracture.

Gaucher disease (GD) is an inherited lysosomal storage disorder caused by glucocerebrosidase (GCase) deficiency. A 35-year-old male patient was admitted to our hospital due to left thigh pain and restricted mobility for 10 h. Following comprehensive evaluations, the patient was diagnosed with GD complicated by a pathological fracture of the left femur. He has a known L444P mutation, a suspected pathogenic A170H mutation, and an A271 V mutation of uncertain significance not in GD databases. However, a potential association with the disorder cannot be excluded. We speculate that the patient's ma

Association of hardware removal with secondary osteonecrosis following femoral neck fractures: a systematic review and meta-analysis.

BACKGROUND: It has been controversial that whether hardware removal will increase the risk of osteonecrosis of femoral head (ONFH) in fracture-healed patients who underwent internal fixation for femoral neck fractures (FNFs). This meta-analysis aimed to clarify the association of hardware removal with secondary hardware removal-induced ONFH (HR-ONFH). METHODS: Four electronic databases (PubMed, Embase, Web of Science, Cochrane Library) were searched for eligible studies published up to March 10, 2023. Studies reporting the relative risk of hardware status (i.e., risk rate, odds ratio [OR], or

Herbert screw fixation for scaphoid nonunions. An analysis of factors influencing outcome.

A retrospective review of 160 cases of scaphoid nonunion treated by internal fixation using a Herbert screw with bone grafting was conducted at an average followup of 24 months. Definite radiographic union was achieved in 90% of cases. Based on Cooney's clinical scoring system, 80 cases had an excellent result, 37 had a good result, 33 had a fair result, and 10 had a poor result. Failure of union was related to the existence of avascular changes of the proximal fragment, instability of the fracture fragment, the prolonged delay in surgery, and the location of the fracture site. In the united s

Avulsion fracture of the extensor carpi radialis longus in a rugby player: a case report.

Avulsion fracture of the base of the second metacarpal is an unusual injury, and the cause in the few cases reported in the literature was a fall on a volarly flexed wrist. A case of this rare injury suffered in a sport related accident by a semiprofessional rugby player is reported. It was treated with open reduction and internal fixation after failure of conservative management.

Technical modification of the sagittal split mandibular ramus osteotomy.

The sagittal split mandibular ramus osteotomy is a versatile surgical option for the correction of mandibular prognathism, retrognathism, and asymmetry. At the same time, it enables the use of internal rigid fixation. However, surgical complications including injury to the inferior dental neurovascular bundle and unfavorable fracture can occur. A technical modification and a concept in instrumentation is suggested with the view of minimizing such complications.

Outcome of olecranon osteotomy in the trans-olecranon approach of intra-articular fractures of the distal humerus.

BACKGROUND: The trans-olecranon approach has been suggested to improve the visualization of complex intra-articular distal humerus fractures. Significant osteotomy complications have prompted a search for alternative approaches. The purpose of this series was to study the outcome of the olecranon osteotomy in terms of union and complications and the ultimate outcome of the fracture. METHODS: Ninety-four patients with intra-articular fractures of the distal humerus (type C3) were treated by open reduction and internal fixation using the trans-olecranon approach. The patients were followed from

An unusual combination of acetabular and pelvic fracture: is this a new subtype of acetabular fracture?

INTRODUCTION: Acetabular fractures are a common problem among young males. An acetabular fracture with disruption of the joint surface, if untreated, will rapidly lead to post-traumatic osteoarthritis. Proper reduction and internal fixation depend on accurate classification and the quality of imaging. CASE PRESENTATION: We present an unusual form of acetabular fracture, which is not included in the conventional classification (Judet and Letournel) ; this occurred in a middle-aged male who was operatively treated without any complications. In this case due to posterior extension of the fractur

Nonunion of the so-called acromion: a systematic review with consideration of the terminology.

INTRODUCTION: There is no widely accepted standard for the classification and treatment of traumatic acromion/scapular spine fracture nonunion due to the scarcity of this condition and the confusion of terminology. MATERIALS AND METHODS: PubMed and Scopus were searched using "scapular fracture" and "acromion fracture" or "scapular spine fracture" as search terms. The inclusion criteria were English full-text articles concerning acromion/scapular spine fracture nonunion that described patient characteristics and presented appropriate images. The exclusion criteria were cases without appropriat

Stable fixation of osteoporotic fractures and nonunions in the upper limb - life before the "locking plate".

I have had a wonderful opportunity over the past 30 years to surgically reconstruct many complex fractures and non- unions in the upper limb in the elderly patient with underlying osteoporosis and prior to the development of the "locked plate". This article will present a number of specific techniques using standard LC-DCP and screw in a variety of applications to provide stable internal fixation. These include the use of long plates; creating a "waved plate" initially described by Blatter and Weber; double plating; 3.5 mm intramedullary plate combined with a larger plate on the cortex; custom

Influence of Coronoid fixation on the functional outcome and rate of complications in surgically treated acute complex elbow instability.

INTRODUCTION: Fractures of the coronoid commonly present in elbow fracture-dislocations. Despite the evidence that the coronoid plays an important role in elbow stability, there is still controversy on which fractures should be surgically fixed. The aim of this study is to compare the clinical outcomes and rate of complications of patients with elbow fracture-dislocations in which the coronoid was fixed or left untreated. MATERIALS AND METHODS: Thirty-nine patients with an elbow fracture-dislocation involving a coronoid fracture were prospectively followed for an average of 90 months (range 2

Surgical Technique to Manage Periprosthetic Fractures of the Knee in Patients with Infected Leg Ulcers: A Report of Two Cases.

CASES: We describe 2 cases of nonagenarians with periprosthetic knee fractures that were not amenable to either standard internal fixation nor prosthesis revision because of infected leg ulcers in the same limb. The fractures were internally fixed by percutaneous insertion of medial and lateral plates that spanned the knee. Both patients returned to their baseline level of activity without developing surgical site infections. CONCLUSIONS: Percutaneous bridging plates that span the knee are a useful option for treating these difficult cases.
