Efficacy and Safety of Intravenous Esketamine in Elderly Patients Undergoing Laparoscopic Radical Prostatectomy.

While perioperative esketamine use has grown increasingly prevalent, evidence supporting its efficacy and safety in elderly patients undergoing laparoscopic prostate cancer surgery remains limited. This retrospective study evaluated intravenous esketamine's performance in this population by analyzing 186 elderly patients treated between 2021 and 2024, divided into a control group (n = 91; conventional anesthesia) and an esketamine group (n = 95; conventional anesthesia plus esketamine). Esketamine infusion was discontinued 30 min before surgery completion. Hemodynamic parameters-heart rate (HR

Reimagining falls prevention with insights from systems mapping on the use of millimetre-wave radar for remote health monitoring.

Falls constitute a significant public health concern, demanding innovative solutions that transcend traditional methodologies. Current falls practice focuses on reactive post-fall assessment and management rather than proactive prevention and mitigation. We propose that millimetre-wave radar technology for real-time, continuous falls risk screening at home may address the limitations of current falls practice. To investigate the feasibility of this solution, we interviewed five experts in physiotherapy, falls prevention among older adults, and comprehensive geriatric assessment to identify the

[Malnutrition in older adults].

. Malnutrition in older adults. Malnutrition in older adults is a multifactorial condition with serious health and social consequences. Physiological changes such as reduced appetite, impaired taste and smell, and slower gastrointestinal signaling contribute to the "anorexia of aging." Physical impairments, including poor oral health, chewing and swallowing difficulties, and mobility limitations, further reduce food intake. Cognitive decline, dementia, depression, and delirium also compromise nutrition, while chronic diseases, inflammation, and polypharmacy increase nutritional needs or interf

Diagnostic accuracy of the SARC-CalF in community-dwelling older adults: a systematic review and meta-analysis.

BACKGROUND AND OBJECTIVES: The Sarcopenia and Calf Circumference (SARC-CalF) tool has been proposed to enhance the detection of sarcopenia. A comprehensive systematic review evaluating the diagnostic performance of the SARC-CalF could support its broader adoption as a valid screening instrument. Thus, this study aimed to assess the diagnostic accuracy of the SARC-CalF in detecting sarcopenia risk among older adults. RESEARCH DESIGN AND METHODS: A systemic review was conducted in six electronic databases and Google Scholar. The values of sensitivity, specificity, and area under the curve were

[Dizziness in old age].

Dizziness is a frequent and often multifactorial symptom in older patients that can significantly impair the quality of life. The causes are diverse and differ from younger patients. Polyneuropathy, orthostatic dysregulation, drug-related causes, positional vertigo and bilateral vestibulopathy are more frequent in old age. A detailed and vertigo-specific patient history, including the assessment of risk factors such as frailty, polypharmacy and comorbidities, is essential for the diagnostics. The treatment depends on the underlying cause and includes physiotherapeutic interventions as well as

SOHO State of the Art Updates and Next Questions | Diffuse Large B-Cell Lymphoma in Older Adults: A Comprehensive Review.

Older adults (OA) with DLBCL are a heterogenous population with suboptimal outcomes. In this review, we identify and address the unique challenges encountered in the care of OA with DLBCL. We elaborate on the role and limitations of current geriatric assessment (GA) tools and ways to incorporate fitness in therapeutic decision making. We suggest best practices to implement GA in routine practice and clinical trials. The most widely used tool is simplified GA (sGA) which categorizes patients into fit, unfit and frail groups. Patients who are fit benefit from full dose/curative approach, whereas

Role of Cognitive Frailty in Older Adults With Cardiovascular Disease.

As the older adult population expands, an increasing number of patients affected by geriatric syndromes are seen by cardiovascular clinicians. One such syndrome that has been associated with poor outcomes is cognitive frailty: the simultaneous presence of cognitive impairment, without evidence of dementia, and physical frailty, which results in decreased cognitive reserve. Driven by common pathophysiologic underpinnings (eg, inflammation and neurohormonal dysregulation), cardiovascular disease, cognitive impairment, and frailty also share the following risk factors: hypertension, diabetes, obe

New horizons in hospital-associated deconditioning: a global condition of body and mind.

Hospital-associated deconditioning is a broad term, which refers non-specifically to declines in any function of the body secondary to hospitalisation. Older people, particularly those living with frailty, are known to be at greatest risk. It has historically been most commonly used as a term to describe declines in muscle mass and function (i.e. acute sarcopenia). However, declines in physical function do not occur in isolation, and it is recognised that cognitive deconditioning (defined by delayed mental processing as part of a spectrum with fulminant delirium at one end) is commonly encount

Postgraduate education in healthy and active ageing: a systematic scoping review of learning needs, curricula and learning impact.

To inform a new European-based online master's programme in active aging and age-friendly society, a scoping review was performed to identify previously reported learning needs and learning outcomes on this topic. Four electronic databases (PubMed, EBSCO [Academic Search Complete], Scopus, and ASSIA) were systematically searched, along with gray literature. Dual, independent review of an initial 888 studies led to 33 included papers which underwent independent data extraction and reconciliation. Just 18.2% of studies employed a student survey or similar to determine learning needs, with the ma

ChatGPT vs Google for Queries Related to Dementia and Other Cognitive Decline: Comparison of Results.

BACKGROUND: People living with dementia or other cognitive decline and their caregivers (PLWD) increasingly rely on the web to find information about their condition and available resources and services. The recent advancements in large language models (LLMs), such as ChatGPT, provide a new alternative to the more traditional web search engines, such as Google. OBJECTIVE: This study compared the quality of the results of ChatGPT and Google for a collection of PLWD-related queries. METHODS: A set of 30 informational and 30 service delivery (transactional) PLWD-related queries were selected an

Glutathione and glutathione-dependent enzymes: From biochemistry to gerontology and successful aging.

The tripeptide glutathione (GSH), namely γ-L-glutamyl-L-cysteinyl-glycine, is an ubiquitous low-molecular weight thiol nucleophile and reductant of utmost importance, representing the central redox agent of most aerobic organisms. GSH has vital functions involving also antioxidant protection, detoxification, redox homeostasis, cell signaling, iron metabolism/homeostasis, DNA synthesis, gene expression, cysteine/protein metabolism, and cell proliferation/differentiation or death including apoptosis and ferroptosis. Various functions of GSH are exerted in concert with GSH-dependent enzymes. Inde

High SARC-F score predicts poor survival of patients with cancer receiving palliative care.

PURPOSE: A high score determined by SARC-F, a simple screening questionnaire for sarcopenia, has been reportedly associated with worse medical outcomes. However, information regarding whether high SARC-F scores are associated with a poor prognosis in patients with advanced cancer remains limited. We clarified whether a SARC-F score ≥ 4 predicts poor prognosis in patients with cancer receiving palliative care. METHODS: We conducted a retrospective cohort study of patients with cancer who received palliative care at a university hospital between May 2019 and April 2020. Patient characteristics

The influence of multiple frailty profiles on institutionalization and all-cause mortality in community-living older adults.

BACKGROUND: Frailty in older adults is associated with adverse geriatric outcomes. Physical frailty is often accompanied by problems in the cognitive, psychological, and social domains. This study investigated the ability of physical frailty combined with other health domains to predict institutionalization and mortality. METHODS: A national sample of 9171 Koreans aged 65 years or older were surveyed at baseline in 2008 and 3 year follow-up. Those who were prefrail or frail according to the Fried criteria were conceived to have physical frailty. Psychological frailty, cognitive frailty, and s

Role of sarcopenia in the frailty transitions in older adults: a population-based cohort study.

BACKGROUND: Frailty and sarcopenia are age-associated syndromes that have been associated with the risk of several adverse events, mainly functional decline and death, that usually coexist. However, the potential role of one of them (sarcopenia) in modulating some of those adverse events associated to the other one (frailty) has not been explored. The aim of this work is to assess the role of sarcopenia within the frailty transitions and mortality in older people. METHODS: Data from the Toledo Study of Healthy Aging (TSHA) were used. TSHA is a cohort of community-dwelling older adults ≥65. Fr

Association between multidimensional prognostic index (MPI) and pre-operative delirium in older patients with hip fracture.

Pre-operative delirium may cause delay in surgical intervention in older patients hospitalized for hip fracture. Also it has been associated with higher risk of post-surgical complications and worst functional outcomes. Aim of this retrospective cohort study was to evaluate whether the multidimensional prognostic index (MPI) at hospital admission was associated with pre-operative delirium in older individuals with hip fracture who are deemed to require surgical intervention. Consecutive older patients (≥ 65 years) with hip fracture underwent a comprehensive geriatric assessment to calculate th

[Screening and management of sarcopenia by the general practitioner].

Sarcopenia is an aging syndrome with multiple contributing factors, characterized by a loss of muscle strength, function and mass. It affects a third of the elderly population, increasing morbidity and mortality, as well as health costs. It should be suspected in the event of a decrease in physical capacities reported or observed during the consultation, in a patient with risk factors. Five questions (SARC-F formulary) or the measure of the gait speed makes screening easy to perform ; the diagnosis is confirmed by supplementary examinations in a specialized center. Treatment consists on perfor
