Suture-mediated closure of antegrade femoral arteriotomy following infrainguinal intervention.

Antegrade femoral arterial access has been less commonly adopted for infrainguinal intervention due to increased risk of retroperitoneal hemorrhage secondary to noncompressibility of arteriotomy site. We evaluated the efficacy and safety of suture-mediated closure of antegrade femoral arteriotomy using the Closer device. Twelve consecutive patients undergoing infrainguinal intervention (females, 5; mean body weight, 69 +/- 16 kg; limb threatening ischemia, 50%) underwent repair of the antegrade femoral arteriotomy immediately postprocedure using the Closer. Indications for antegrade access wer

[Non-contact argon gas coagulation in flexible endoscopy of the gastrointestinal tract: in vitro studies and initial clinical experiences].

Diathermocoagulation is indispensable in interventional endoscopy. The argon beam coagulation represents an innovative electrocoagulation method, where high-frequency alternating current is conducted to tissues by ionized argon gas without contact. Before clinical application we performed in vitro studies to evaluate depth and diameter of tissue coagulation of fresh resectations from stomach, small intestine and colon. Power and gas flow were graduated in five steps from 40 to 155 W and from 2 to 7 l/min respectively. Coagulation time (1 s-10 s) and angle of the probe in relation to tissue sur

History of urethral catheters and their balloons: drainage, anchorage, dilation, and hemostasis.

Male urethral catheterization, as practiced in early Greece, was made possible by a sound knowledge of anatomy. It has been preserved by written tradition and advanced by the development of new materials. Balloon dilation, conceived and practiced more than 160 years ago by urologists, has undergone a similar cycle. Some useful ideas, once conceived, have been forgotten and have surfaced again, decades, sometimes centuries, later as new inventions.

[Local treatment of Von Willebrand's disease--use of GRF adhesive].

Among the systemic hemorrhagic we find Von Willebrand's, the most prevalent coagulation related illness. It may be defined as a qualitative platelet abnormality which in most cases is transmitted as a dominant mendelian trait. Clinically there is prolonged bleeding time, epistaxis, gingival hemorrhage and increased bleeding after surgery or dental extractions. The purpose of this project was to test the effectivity of a local element such as GRF glue as the only treatment for these patients. With the purpose to avoid the use of a substitute therapy, mainly taking into consideration all the pro

Comparison of Three TR Band Deflation Protocols Following Radial Coronary Procedures: A Prospective Randomized Trial.

BACKGROUND: Radial artery hemostasis can be achieved through a variety of methods after the excision of the sheath. The risk of radial artery blockage presents a challenge linked to the radial approach, potentially limiting its future application. The patent hemostasis method effectively achieves hemostasis while maintaining the patency of the radial artery, thereby reducing the likelihood of radial artery occlusion. AIMS: The aim of this study was to compare three TR band deflation protocols (early initiation with long intervals, delayed initiation with short intervals, and routine protocol)

Comparative Effectiveness of Vascular Closure Device Strategies in Transcatheter Aortic Valve Replacement: A Network Meta-Analysis.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become the preferred treatment for high-risk patients with severe aortic stenosis. However, the management of vascular complications associated with large-bore femoral arterial access sheaths remains challenging for clinicians. Traditional manual compression exhibits limited efficacy in homeostasis, while the clinical outcomes of vascular closure devices (VCDs) remain controversial. To conduct a systematic evaluation on the differences among various types of VCDs in preventing vascular complications and achieving device success rate

Immediate Leg Mobilization is Feasible After Catheter Ablation of Atrial Fibrillation Using Large Vascular Access Sheaths (Pulsed Field and Cryoballoon Ablation).

BACKGROUND: Vascular recovery from catheter ablation (CA) has traditionally required a period of leg immobilization which can lead to discomfort and prolonged time to discharge. OBJECTIVE: The objective of this study was to compare a strategy of immediate leg mobilization (IM) using suture-mediated closure devices against traditional vascular recovery consisting of figure-of-eight suture and 4 h bed rest (BR) after CA of atrial fibrillation (AF) using large vascular access sheaths for cryoballoon ablation (CBA) and pulsed field ablation (PFA). METHODS: Two hundred subjects were retrospective

The VWF binding aptamer rondoraptivon pegol increases platelet counts and VWF/FVIII in type 2B von Willebrand disease.

Type 2B von Willebrand disease (VWD) is characterized by an increased binding affinity of von Willebrand factor (VWF) to platelet glycoprotein Ib. This can lead to clearance of high-molecular-weight (HMW) multimers and thrombocytopenia with a resulting moderate-severe bleeding phenotype. Rondoraptivon pegol (BT200) is a pegylated aptamer binding to the A1 domain of VWF with a novel mechanism of action: it enhances VWF/factor VIII (FVIII) levels by decreasing their clearance. To study the potential benefit of rondoraptivon pegol in patients with type 2B VWD, we conducted a prospective phase 2 t

Comparing Gelfoam vs fat as a sealing material in stapedotomy: A prospective double-blind randomised clinical trial.

OBJECTIVES: One research aspect of stapes surgery is various materials that are used to seal the oval window. Several materials are used to seal the oval window, for example adipose tissue, perichondrium, vein graft, gelatin sponge (Gelfoam), blood clot and soft connective tissue. Up to now, there has been no randomised clinical trial that has evaluated the effects of different types of sealing material on hearing outcomes after stapedotomy. Hence, the present study aimed to find out which of these materials; fat or Gelfoam was associated with better hearing outcome, when used as a sealing mat

Advances in the treatment of portal hypertension in cirrhosis.

Non-selective beta-blockers and handling of esophageal varices has been key elements in the treatment of portal hypertension in recent decades. Liver vein catheterization has been essential in diagnosis and monitoring of portal hypertension, but ongoing needs for noninvasive tools has led to research in areas of both biomarkers, and transient elastography, which displays promising results in discerning clinically significant portal hypertension. Novel research into the areas of hepatic stellate cell function and the dynamic components of portal hypertension has revealed promising areas of trea

Access site complications following transfemoral coronary procedures: comparison between traditional compression and angioseal vascular closure devices for haemostasis.

BACKGROUND: Vascular closure devices such as angioseal are used as alternatives to traditional compression haemostasis. Although the safety and efficacy of angioseal are confirmed, their use remains controversial because of the potential complications of these devices compared with those of traditional compression haemostasis. The aim of this study was to compare the access site complication rate, the predictive factors for these complications, and patient comfort levels after coronary procedures with traditional compression or angioseal haemostasis. METHODS: Data were collected from a cardia
