Evaluation of adenosine triphosphate test for cleaning assessment of gastroscopes and the effect on workload in a busy endoscopy center.

OBJECTIVE: Using adenosine triphosphate (ATP) tests to assess manual cleaning of gastroscopes and to determine the associated workload in a busy endoscopy unit. METHODS: Patient-used gastroscopes were sampled before and after cleaning to assess ATP levels, bioburden, and protein. Samples were collected by flushing 20 mL of sterile water through the biopsy port to the distal end. Time spent for reprocessing and performing the ATP test was recorded. RESULTS: Twenty-four samples were collected from 10 gastroscopes. After manual cleaning, 14/24 (58.3%) samples had no microbial growth (mean, 21 c

An appraisal of the neglected tropical diseases control program in Cameroon: the case of the national program against onchocerciasis.

BACKGROUND: Onchocerciasis is a severe parasitic infestation which causes disabling skin and subcutaneous tissue changes. Current global estimates suggest that it accounts for 1135.7 disability adjusted life years (DALYs) per 100,000 population. The disease is endemic in many African countries including Cameroon, probably suggesting that the current health policies are inadequate to achieve eradication of the disease. We aimed to appraise the current Onchocerciasis control program in Cameroon in the context of existing literature. METHODS: We carried out a MEDLINE search via PubMed to source

Parental Presence During Treatment of Ebola or Other Highly Consequential Infection.

This clinical report offers guidance to health care providers and hospitals on options to consider regarding parental presence at the bedside while caring for a child with suspected or proven Ebola virus disease (Ebola) or other highly consequential infection. Options are presented to help meet the needs of the patient and the family while also posing the least risk to providers and health care organizations. The optimal way to minimize risk is to limit contact between the person under investigation or treatment and family members/caregivers whenever possible while working to meet the emotiona

Disposable surgical face masks for preventing surgical wound infection in clean surgery.

BACKGROUND: Surgical face masks were originally developed to contain and filter droplets containing microorganisms expelled from the mouth and nasopharynx of healthcare workers during surgery, thereby providing protection for the patient. However, there are several ways in which surgical face masks could potentially contribute to contamination of the surgical wound, e.g. by incorrect wear or by leaking air from the side of the mask due to poor string tension. OBJECTIVES: To determine whether disposable surgical face masks worn by the surgical team during clean surgery prevent postoperative su

The current situation of meningococcal disease in Latin America and recommendations for a new case definition from the Global Meningococcal Initiative.

The Global Meningococcal Initiative (GMI) is an international group of scientists and clinicians with expertise in meningococcal disease (MD). It promotes MD prevention through education and research. Given geographic differences in disease epidemiology, prevention strategies (e.g., vaccination) should be country-specific to ensure local needs are met. However, regional policies/recommendations and standardized disease diagnostic criteria should be implemented to improve surveillance and control strategies, and allow for more robust data comparisons. Consequently, the GMI convened a meeting wi

Patient-days used for isolation in a community hospital.

BACKGROUND: Isolation of patients with known or suspected infection strains the resources of hospitals, but little information exists on the actual utilization of isolation beds. METHODS: The infection control team of a community hospital keeps a log of hospital days used for isolation. We obtained information from this log for August 2009 through August 2010 on hospital days for isolation due to methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, Clostridium difficile, other multidrug-resistant organisms, and disorders necessitating droplet or airborne isolation, s…

Rapid development and use of a nationwide training program for cholera management, Haiti, 2010.

When epidemic cholera appeared in Haiti in October 2010, the medical community there had virtually no experience with the disease and needed rapid training as the epidemic spread throughout the country. We developed a set of training materials specific to Haiti and launched a cascading training effort. Through a training-of-trainers course in November 14-15, 2010, and department-level training conducted in French and Creole over the following 3 weeks, 521 persons were trained and equipped to further train staff at the institutions where they worked. After the training, the hospitalized cholera

Antimony toxicity.

Antimony toxicity occurs either due to occupational exposure or during therapy. Occupational exposure may cause respiratory irritation, pneumoconiosis, antimony spots on the skin and gastrointestinal symptoms. In addition antimony trioxide is possibly carcinogenic to humans. Improvements in working conditions have remarkably decreased the incidence of antimony toxicity in the workplace. As a therapeutic, antimony has been mostly used for the treatment of leishmaniasis and schistosomiasis. The major toxic side-effects of antimonials as a result of therapy are cardiotoxicity (~9% of patients) an

Tetracyclines: what a rheumatologist needs to know?

Tetracyclines are broad-spectrum antibiotics that act at the ribosomal level. They were first introduced in 1948 and were widely prescribed by dermatologists in the early 1950s for treatment of acne. More recently, biologic actions of tetracyclines affecting inflammation, angiogenesis, and bone metabolism have been researched. The therapeutic effects of tetracycline and its analogues in rheumatic diseases have also been investigated. This article will review the rheumatological use of tetracycline and its analogues.

Legionella species colonization in cooling towers: risk factors and assessment of control measures.

BACKGROUND: Cooling towers can be colonized by Legionella spp, and inhalation of aerosols generated by their operation may cause Legionnaires' disease in susceptible hosts. Environmental investigations of Legionnaires' disease outbreaks linked with cooling towers have revealed poorly maintained systems, lack of control measures, and failure of system equipment. The purpose of this study was to identify Legionella-contaminated cooling towers, identify risk factors for contamination, and assess the effectiveness of control measures. METHODS: A total of 96 cooling towers of public buildings were

Preventive effects of various types of footwear and cleaning methods on dermatophyte adhesion.

Tinea pedis is contagious and typically spreads from infected to non-infected persons. The purpose of this study was to evaluate the efficacy of footwear in preventing tinea pedis adhesion. Using the stamp culture method, we investigated the effectiveness of preventing dermatophyte passage by the wearing of stockings made of nylon, socks made of cotton and tabi (Japanese socks), as well as the effect of removing dermatophytes from these items by washing with soap, cold water and cold water after turning inside-out. For sandals, sneakers and boots, we also investigated the effect of dermatophyt

Modeling the role of bacteriophage in the control of cholera outbreaks.

Cholera is a waterborne diarrheal disease that continues to plague the developing world. Individuals become infected by consuming water from reservoirs contaminated by virulent strains of the bacterium Vibrio cholerae. Epidemiological and environmental observations of a cholera outbreak in Dhaka, Bangladesh, suggest that lytic bacteriophage specific for V. cholerae may limit the severity of cholera outbreaks by killing bacteria present in the reservoir and in infected individuals. To quantify this idea and generate testable hypotheses, we analyzed a mathematical model that combines the epidemi

Fighting infection: an ongoing challenge, part 3-antimycobacterials, antifungals, and antivirals.

The first part of this series provided a brief overview of how antimicrobials, the "silver bullets" of modern medicine, are designed to target specific agents of infection. The second part addressed several classes of antibacterials: bacterial cell wall inhibitors (penicillins, cephalosporins), protein synthesis inhibitors (macrolides, tetracyclines, aminoglycosides), and nucleic acid inhibitors (sulfonamides and quinolones). This third section focus on those drugs used to treat mycobacterial infections (Mycobacterium tuberculosis), fungal infections (Candida species, Aspergillus species), and

Antibiotics for preventing infection in open limb fractures.

BACKGROUND: Wound and bone infections are frequently associated with open fractures of the extremities and may add significantly to the resulting morbidity. The administration of antibiotics is routinely used in developed countries as an adjunct to a comprehensive management protocol that also includes irrigation, surgical debridement and stabilisation when indicated, and is thought to reduce the frequency of infections. OBJECTIVES: To quantify the evidence for the effectiveness of antibiotics in the initial treatment of open fractures of the limbs. SEARCH STRATEGY: We searched the Cochrane
