Interpretable Alzheimer's Diagnosis via Multimodal Fusion of Regional Brain Experts.

Accurate and early diagnosis of Alzheimer's disease (AD) is critical for effective intervention and requires integrating complementary information from multimodal neuroimaging data. However, conventional fusion approaches often rely on simple concatenation of features, which cannot adaptively balance the contributions of biomarkers such as amyloid PET and MRI across brain regions. In this work, we propose MREF-AD, a Multimodal Regional Expert Fusion model for AD diagnosis. It is a Mixture-of-Experts (MoE) framework that models mesoscopic brain regions within each modality as independent expert

Multi-site analysis of COVID-19 and new-onset diabetes reveals need for improved sensitivity of EHR-based COVID-19 phenotypes-a DiCAYA Network analysis.

OBJECTIVE: We discuss implications of potential ascertainment biases for studies examining diabetes risk following SARS-CoV-2 infection using electronic health records (EHRs). We quantitatively explore sensitivity of results to misclassification of COVID-19 status using data from the U.S.-based Diabetes in Children, Adolescents and Young Adults (DiCAYA) Network on children (≤17 years) and young adults (18-44 years). MATERIALS AND METHODS: In our retrospective case study from the DiCAYA Network, SARS-CoV-2 was identified using labs and diagnoses from June 1, 2020 to December 31, 2021. Patients

Biases in an artificial intelligence image-generator's depictions of healthy aging and Alzheimer's.

OBJECTIVE: This content analysis study investigates potential biases in image generation by 2 artificial intelligence (AI) tools, DALL-E 3 and Midjourney, in portraying older adults and individuals living with dementia. Despite widespread use of generative AI in various sectors, there is limited research on how these models might perpetuate stereotypes and stigmatization through their images. MATERIALS AND METHODS: 1056 images were generated using specified prompts categorized into 3 groups: general older adults, dementia-related, and control. Each prompt began with "photorealistic portrait"

A real-time machine learning framework for improved intraoperative risk predictions in cardiac surgery patients.

BACKGROUND AND OBJECTIVE: Cardiac surgeries are associated with a range of serious postoperative complications, and current predictive models often rely on static summaries of preoperative and intraoperative data compiled after the case. Such development limits their utility in real-time clinical settings. To address this challenge, we developed TE-Transformer, an end-to-end deep learning framework that continuously ingests heterogeneous electronic health record (EHR) data-including preoperative variables, high-frequency vital signs, medication administration records, and brief surgical notes

Improving Survival Prediction of Head and Neck Cancer Patients by Modeling Long-term Symptom Burden.

Accurate survival prediction in head and neck cancer (HNC) is essential for effective treatment planning and management. Patient-reported outcomes (PROs) provide a complementary source of prognostic information by capturing longitudinal symptom trajectories that reflect patients' health status and treatment response, but their integration into survival modeling remains challenging due to missing data and limited availability at the desired decision points. We propose an end-to-end approach that models longitudinal symptom trajectories using a bidirectional long short-term memory (Bi-LSTM) netw

Comparing patient-reported symptoms and structured clinician documentation in electronic health records.

OBJECTIVES: Real-world data (RWD) analyses primarily rely on structured clinical documentation collected through routine clinical care or driven by medical billing requirements. Patient-reported outcome measures (PROMs), integrated into electronic health records (EHRs), are an additional data source that could offer valuable insights into a patient's perspective and contribute to a more comprehensive understanding of health outcomes in RWD studies. This study aims to characterize agreement between PROMs symptoms and structured clinical documentation of these symptoms by clinicians in EHRs. MA

Negative descriptors in electronic health records of patients with diabetes.

BACKGROUND: Negative descriptors in electronic health records (EHR) contribute to worse health outcomes; studies show they are also more prevalent in EHRs of women and racial minorities and affect downstream research biases. Similar and unique patterns of negative descriptors may also exist in the records of blind patients, including those with diabetic retinopathy. Diabetic retinopathy is a preventable but leading cause of blindness in the US that is disproportionally high among women and racial and ethnic minorities. METHODS: Using EHR from a large medical center, we created "matched" cohor

Beyond electronic health record data: leveraging natural language processing and machine learning to uncover cognitive insights from patient-nurse verbal communications.

BACKGROUND: Mild cognitive impairment and early-stage dementia significantly impact healthcare utilization and costs, yet more than half of affected patients remain underdiagnosed. This study leverages audio-recorded patient-nurse verbal communication in home healthcare settings to develop an artificial intelligence-based screening tool for early detection of cognitive decline. OBJECTIVE: To develop a speech processing algorithm using routine patient-nurse verbal communication and evaluate its performance when combined with electronic health record (EHR) data in detecting early signs of cogni

Enhancing patient representation learning with inferred family pedigrees improves disease risk prediction.

BACKGROUND: Machine learning and deep learning are powerful tools for analyzing electronic health records (EHRs) in healthcare research. Although family health history has been recognized as a major predictor for a wide spectrum of diseases, research has so far adopted a limited view of family relations, essentially treating patients as independent samples in the analysis. METHODS: To address this gap, we present ALIGATEHR, which models inferred family relations in a graph attention network augmented with an attention-based medical ontology representation, thus accounting for the complex infl

Real-world federated learning in radiology: hurdles to overcome and benefits to gain.

OBJECTIVE: Federated Learning (FL) enables collaborative model training while keeping data locally. Currently, most FL studies in radiology are conducted in simulated environments due to numerous hurdles impeding its translation into practice. The few existing real-world FL initiatives rarely communicate specific measures taken to overcome these hurdles. To bridge this significant knowledge gap, we propose a comprehensive guide for real-world FL in radiology. Minding efforts to implement real-world FL, there is a lack of comprehensive assessments comparing FL to less complex alternatives in ch

Beyond Phecodes: leveraging PheMAP to identify patients lacking diagnosis codes in electronic health records.

OBJECTIVE: Diagnosis codes documented in electronic health records (EHR) are often relied upon to clinically phenotype patients for biomedical research. However, these diagnoses can be incomplete and inaccurate, leading to false negatives when searching for patients with phenotypes of interest. This study aims to determine whether PheMAP, a comprehensive knowledgebase integrating multiple clinical terminologies beyond diagnosis to capture phenotypes, can effectively identify patients lacking relevant EHR diagnosis codes. Long-term care plan recommendation for older adults with disabilities: a bipartite graph transformer and self-supervised approach.

BACKGROUND: With the global population aging and advancements in the medical system, long-term care in healthcare institutions and home settings has become essential for older adults with disabilities. However, the diverse and scattered care requirements of these individuals make developing effective long-term care plans heavily reliant on professional nursing staff, and even experienced caregivers may make mistakes or face confusion during the care plan development process. Consequently, there is a rigid demand for intelligent systems that can recommend comprehensive long-term care plans for
