Sucrose analgesia for venepuncture in neonates.

RATIONALE: Sucrose is, in general, safe and effective for analgesia during venepuncture in hospitalised neonates. However, there is a lack of evidence on its analgesic effects. OBJECTIVES: To evaluate the benefits and harms of orally administered sucrose for pain relief from venepuncture in preterm and term neonates compared to no intervention, standard care, and other types of analgesic interventions. SEARCH METHODS: We searched CENTRAL, MEDLINE, Ovid Embase, and trial registries in July 2025, and the China National Knowledge Infrastructure, VIP Chinese Science and Technology Periodicals, a

Digital decision aid for prenatal counseling in imminent extreme premature labor: development and pilot testing.

BACKGROUND: In case of extreme premature delivery at 24 weeks of gestation, both early intensive care and palliative comfort care for the neonate are considered treatment options. Prenatal counseling, preferably using shared decision making, is needed to agree on the treatment option in case labor progresses. This article described the development of a digital decision aid (DA) to support pregnant women, partners and clinicians in prenatal counseling for imminent extreme premature labor. METHODS: This DA is developed following the International Patient Decision Aid Standards. The Dutch treatm

Perinatal outcomes associated with magnesium sulfate for preeclampsia without severe features.

OBJECTIVE: To evaluate neonatal outcomes in patients with preeclampsia without severe features who received magnesium for seizure prophylaxis. METHODS: Retrospective cohort study of patients with preeclampsia without severe features delivering ≥32 weeks' gestation at our institution (November 2021-May 2023). The primary outcome was a composite of adverse neonatal outcomes (neonatal intensive care unit [NICU] admission, umbilical artery pH < 7.1, umbilical artery base deficit ≥12, 5-min Apgar <3, neonatal death, culture proven sepsis, and mechanical ventilation). Secondary outcomes included th

Neonatal reference intervals for salivary steroids: focus on prematurity, stressful events and perinatal betamethasone.

CONTEXT: Painlessly obtained samples can improve diagnostics and care in paediatric conditions with affected steroidogenesis. OBJECTIVE: To establish neonatal reference intervals (RIs) and identify sources of biological variation for steroid hormones in saliva. METHODS: Steroid concentrations were determined using LC-MS/MS in 412 saliva samples from 174 infants aged 0-4 weeks, born at gestational age 26-42 weeks, recruited from three neonatal intensive care units and one maternity ward. We used six approaches to construct RIs with partitioning based on results from linear mixed models. RESU

The applications, complications, and management of neonatal peripherally inserted central catheters (n-PICCs): a narrative review.

The neonatal peripherally inserted central catheters (n-PICCs) is a commonly used vascular access device in neonatal intensive care, yet its associated complications significantly affect patient outcomes. This narrative review aims to summarize current status of n-PICCs, major complications and risk factors, and evidence-based preventive strategies. A literature search was conducted in PubMed, Web of Science, and Embase for articles published between January 2000 and February 2026, and relevant studies were synthesized and discussed. The findings demonstrate that the incidence of n-PICCs-relat

Clinical Effectiveness and Tolerability of Parenteral Iron Sucrose in the Second- and Third-Trimester Anemia: A Retrospective Record-Based Analysis.

Background Iron-deficiency anemia (IDA) is a leading cause of adverse maternal and neonatal outcomes, particularly in resource-limited settings where nutritional deficits and late antenatal presentation are common. When diagnosed in late gestation, oral iron therapy is often insufficient for timely hematologic correction. Methods This retrospective observational study reviewed medical records of 75 pregnant women between 26 and 32 weeks of gestation with hemoglobin <9 g/dL who had received intravenous iron sucrose as part of routine clinical care. Baseline and six-week post-treatment hematolog

Laser acupuncture versus oral glucose administration for pain prevention in term neonates: an observer-blinded non-inferiority randomized controlled clinical trial.

BACKGROUND: Neonates undergoing intensive care are routinely submitted to minor painful procedures such as heel lances. Pharmacological treatment is limited in neonatal care and not recommended for minor painful interventions. Hence, non-pharmacological acute pain management is an important subject. Acupuncture could be a new strategy for neonatal pain prevention. Accordingly, the aim of the present study was to investigate whether laser acupuncture is non-inferior to oral sweet solutions in pain prevention. METHODS: In total, 96 healthy term neonates were randomly assigned to receive either

Maternal Heart Failure.

Heart failure (HF) remains the most common major cardiovascular complication arising in pregnancy and the postpartum period. Mothers who develop HF have been shown to experience an increased risk of death as well as a variety of adverse cardiac and obstetric outcomes. Recent studies have demonstrated that the risk to neonates is significant, with increased risks in perinatal morbidity and mortality, low Apgar scores, and prolonged neonatal intensive care unit stays. Information on the causal factors of HF can be used to predict risk and understand timing of onset, mortality, and morbidity. A v

Improved outcome of preterm infants when delivered in tertiary care centers.

OBJECTIVE: Previous studies that compared outcomes of infants born outside tertiary care centers (outborn) with those born in tertiary care centers (inborn) did not account for admission illness severity and perinatal risks. The objective of this study was to examine whether outborn status is associated with higher mortality and morbidity, after adjustment for perinatal risks and admission illness severity (using the Score for Neonatal Acute Physiology, Version II [SNAP-II]) among preterm infants who were admitted to Neonatal Intensive Care Units (NICUs). METHODS: Logistic regression analysis

Evaluating the maternal and perinatal sequelae of severe gestational hypertension.

BACKGROUND: Hypertensive disorders of pregnancy are widespread and have long-standing implications for women's health. Historically, the management of "severe gestational hypertension," or the presence of severely elevated blood pressures without any other signs or symptoms of end-organ damage meeting the criteria for preeclampsia, has been unclear. The new American College of Obstetricians and Gynecologists guidelines based on expert opinion recommend that severe gestational hypertension be treated similarly to preeclampsia with severe features, but data regarding outcomes for women with this

Exogenous surfactant therapy in newborns.

The introduction of surfactant replacement therapy for the management of respiratory distress syndrome (RDS) in the premature infant is one of the major advances in neonatal intensive care. After almost 50 years of intense research, the concerted efforts of basic scientists and clinicians have been rewarded. Despite the remarkable effects of surfactant therapy of RDS, the impact on the incidence of chronic lung disease has been unclear. With the recognition of the role of surfactant inactivation in the pathogenesis of other respiratory disorders of newborns (i.e., meconium aspiration syndrome
