Rheumatic diseases are complex systemic disorders characterized by multi-organ involvement and diverse laboratory abnormalities. The diagnostic approach involves a comprehensive evaluation of clinical and serological markers and the exclusion of conditions with overlapping presentations, such as other rheumatic diseases and systemic infections. Visceral leishmaniasis (VL) is a protozoan infection that primarily affects the reticuloendothelial system. Direct effects of parasite infiltration of the reticuloendothelial system, combined with the subsequent host immune response, produce clinical an

Brucellosis is an endemic zoonotic infection in Turkey, capable of involving multiple organs and systems. While systemic symptoms are common, focal organ involvement may also occur. Isolated splenic infarction is a rare complication. We report a 27-year-old man with a one-week history of left upper abdominal pain, without fever or weight loss. Physical examination revealed closed Traube's space and left upper quadrant tenderness. Laboratory tests showed leukocytosis, thrombocytosis, and elevated transaminases. Brucella tube agglutination test was positive at 1/640. Abdominal CT demonstrated mu

Elizabethkingia meningoseptica, a Gram-negative pathogen once deemed clinically insignificant, tends to cause infections among low-birth-weight infants and immunocompromised patients. Previously, vancomycin was reported to cure several patients with bacteraemia caused by E. meningoseptica. Nevertheless, some laboratory investigations also showed considerable discordance between in vitro vancomycin susceptibility results obtained by the disk diffusion and broth microdilution methods against clinical E. meningoseptica isolates as determined using the criteria for staphylococci recommended by the

The filoviruses, Ebola virus (EBOV), and Marburg virus (MARV), are among the most pathogenic viruses known to man and the causative agents of viral hemorrhagic fever outbreaks in Africa with case fatality rates of up to 90%. Nearly 30,000 infections were observed in the latest EBOV epidemic in West Africa; previous outbreaks were much smaller, typically only affecting less than a few hundred people. Compared to other diseases such as AIDS or Malaria with millions of cases annually, filovirus hemorrhagic fever (FHF) is one of the neglected infectious diseases. There are no licensed vaccines or

CASE PRESENTATION: Here, we report on a case of VL in an HIV-infected patient from the Republic of Georgia who had moved to Germany 14 years before and who had travelled several times to southern Europe in between. After presenting with typical Pneumocystis jiroveci pneumonia, which was treated appropriately, the patient was started on antiretroviral therapy. Shortly thereafter, however, he developed fever of unknown origin. All laboratory assays for the diagnosis of various infectious agents including serological assays and polymerase chain reaction testing of bone marrow aspirate to diagnose

Cryptococcus gattii, a pathogenic fungus historically appreciated to be endemic to tropical regions, was recognized to emerge in a more temperate zone of North America in the 1990s. Early reports focused on an outbreak that was first apparent on Vancouver Island (BC, Canada), involving both the veterinary and human population. More recently, it has been recognized that this organism is endemic to a wider geography in western North America, with recognized disease caused by unique molecular subtypes in both healthy and immunosuppressed human hosts and a variety of domestic and wild animals. PURPOSE OF REVIEW: This review provides clinicians with an overview of the clinical features of the atypical pneumonias. Atypical community-acquired pneumonia pathogens cause systemic infections with pneumonia. The key to the clinical diagnosis of atypical pneumonias depends on recognizing the characteristic pattern of extrapulmonary organ involvement different for each pathogen. As Legionella is likely to present as severe pneumonia and does not respond to beta-lactams, it is important to presumptively diagnose Legionnaires' disease clinically so that Legionella coverage is included in empiri

C. Larralde et al. (1990, Aoff. Pathol. Lab. Med., 114:926-928) demonstrated that heterologous antigen from the laboratory-adapted murine Taenia crassiceps metacestode may substitute those from Taenia solium in the immunodiagnosis of human cysticercosis by the indirect enzyme-linked immunosorbent assay (IE). This antigen is easily obtained at a laboratory level and solves the problem of T. solium cysticerci collection from naturally or experimentally infected swine. In this study an IE employing a heterologous antigen from the T. crassiceps metacestode was evaluated for the immunodiagnosis of

BACKGROUND: A retrospective study was carried out to confirm the clinical impression that, in Lothian, non-tuberculous mycobacterial infections are as common as pulmonary tuberculosis.
METHODS: All pulmonary isolates of Mycobacterium tuberculosis/bovis and non-tuberculous mycobacteria in Scotland from April 1990 to March 1993, and the notes of all patients with M malmoense isolates in Lothian, were reviewed. Information on mycobacterial culture procedures in Scottish laboratories was obtained as part of an audit project.
RESULTS: Of all pulmonary isolates of mycobacteria in Lothian 53% (108/

Mycoplasma pneumoniae (Mpn) is a common respiratory tract pathogen known to cause cyclic epidemics every 3-4 years. For 4 years (2020-2023), a low positivity rate (0.64%) for Mpn laboratory testing at Public Health Ontario suggested minimal circulation of Mpn in Ontario, Canada, likely owing, in part, to COVID-19-related non-pharmaceutical interventions. In 2024, we observed a dramatic re-emergence of Mpn, characterized by large increases in the number of laboratory-confirmed cases (n = 1,122) and the positivity rate (19.1%) of laboratory detections. Patient age distribution of Mpn infections
