Basic fibroblast growth factor increased regional myocardial blood flow and limited infarct size of acutely infarcted myocardium in dogs.

Basic fibroblast growth factor (bFGF), a growth factor potent in promoting angiogenesis, has been shown to reduce infarct size in experimentally induced acute myocardial infarction. However, the effect of bFGF on regional myocardial blood flow (Qm) in the acutely infarcted myocardium has not been well clarified. In 20 open-chest dogs, the left anterior descending (LAD) coronary artery was occluded and animals were maintained in this condition for 4 weeks. In eight of these dogs, bFGF (300 microg) was injected into the myocardium supplied by the LAD and the artery was ligated (bFGF group), and …

Application of thrombolysis in myocardial infarction risk index in the prediction of long-term outcomes for patients with ST-elevation myocardial infarction and multiple vessel disease: A single-center prospective observational cohort study.

The thrombolysis in myocardial infarction (TIMI) risk index has been indicated to be a simple and useful tool for risk stratification of patients with ST-elevation myocardial infarction (STEMI). However, the predictive value of the TIMI risk index regarding the long-term outcome for patients with STEMI with multiple vessel disease has remained to be determined. In the present study, a total of 369 patients diagnosed with STEMI who received emergency percutaneous coronary intervention treatment were analyzed. A five-year follow-up was performed to record the primary endpoint of all-cause mortal…

Comparison of the prognostic effect of left versus right versus no bundle branch block on presenting electrocardiogram in acute myocardial infarction patients treated with primary angioplasty in the primary angioplasty in myocardial infarction trials.

The presence of bundle branch block (BBB) has been associated with poor outcomes in patients who have acute myocardial infarction. Whether this is true in the angioplasty era is not known. We sought to evaluate the outcome of patients with acute myocardial infarction and BBB who were treated with primary angioplasty. We evaluated 3,053 patients who underwent emergency catheterization in the PAMI trials. Patients who had left BBB (n = 48, 1.6%) on presenting electrocardiogram were compared with patients who had right BBB (n = 95, 3.1%) or no BBB (n = 2,910, 95.3%). Patients who had BBB were old…

Immediate administration of mineralocorticoid receptor antagonist spironolactone prevents post-infarct left ventricular remodeling associated with suppression of a marker of myocardial collagen synthesis in patients with first anterior acute myocardial infarction.

BACKGROUND: Aldosterone (ALD) has been shown to stimulate cardiac collagen synthesis and fibroblast proliferation via activation of local mineralocorticoid receptors. In patients with acute myocardial infarction, we demonstrated that ALD was extracted through the infarct heart and extracting ALD-stimulated post-infarct left ventricular (LV) remodeling. METHODS AND RESULTS: To evaluate the effect of mineralocorticoid receptor antagonist (MRA) spironolactone on post-infarct LV remodeling, 134 patients with first anterior acute myocardial infarction were randomly divided into the MRA (n=65) or n…

Targeting NLRP3 Inflammasome Signaling After Myocardial Infarction: Mechanistic Rationale, Regulatory Networks, and Translational Perspectives.

Myocardial infarction (MI) remains a major cause of death and heart failure despite advances in reperfusion and secondary prevention. Sterile inflammation triggered by ischemic myocardial injury is increasingly recognized as a determinant of infarct expansion, adverse ventricular remodeling, and subsequent clinical outcomes. The NOD-like receptor protein 3 (NLRP3) inflammasome integrates danger-associated molecular patterns, mitochondrial dysfunction, ionic disturbance, metabolic stress, and innate immune signaling. Following activation, caspase-1 processes interleukin-1β and interleukin-18 an…

Percutaneous Coronary Intervention Facility Access and Clinical Outcomes Among Patients With Acute Myocardial Infarction.

IMPORTANCE: The proliferation of percutaneous coronary intervention (PCI) facilities across the US has raised questions about whether expanding capacity improves outcomes in areas already served vs those without prior access. OBJECTIVE: To evaluate the association between the opening of PCI-capable facilities and outcomes following acute myocardial infarction (AMI) in communities with and without baseline PCI access. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used California data from inpatient, emergency department, and ambulatory surgery encounters from January 1, 2011, to Decemb…

Engineered ATP-loaded extracellular vesicles: a dual-functional strategy for improving myocardial infarction therapy.

Myocardial infarction (MI) represents a major component of cardiovascular disease, primarily due to severe energy depletion in ischemic tissue. Extracellular vesicles (EVs) have recently emerged as promising cell-free nanocarriers capable of targeted delivery and intercellular communication. Leveraging these advantages, engineered EVs were investigated in this study as a direct ATP-delivery platform to cardiomyocytes. EVs were functionalized with an anti-myosin antibody to form targeted extracellular vesicles (T-EVs) and subsequently loaded with ATP, generating T-ATP-EVs for selective energy t…

Atypical Posterior Myocardial Infarction With Minimal Symptoms: Diagnostic Impact of Point-of-Care Ultrasound.

Posterior myocardial infarction (PMI) is frequently under-recognised due to its non-classical electrocardiographic (ECG) presentation and variable symptom profile. We report a case of a 39-year-old male patient presenting with minimal chest pain and subtle ECG changes, in whom bedside point-of-care ultrasound (POCUS) played a pivotal role in diagnosis. Despite an initial low-risk clinical impression and non-diagnostic ECG findings, POCUS revealed regional wall motion abnormalities, prompting urgent cardiology reassessment. Subsequent transthoracic echocardiography confirmed inferior wall hypok…

ESC quality indicators for post-myocardial infarction care: transition and chronic coronary syndrome phases.

AIMS: We aimed to develop the European Society of Cardiology (ESC) quality indicators (QIs) for myocardial infarction (MI), from 1 year after hospital discharge, corresponding to transition to the chronic coronary syndrome phases. METHODS AND RESULTS: We collaborated with the European Association of Preventive Cardiology (EAPC) and developed QIs for the long-term management of patients following MI. We applied the ESC methodology for QI development by (i) determining key domains of post-MI care; (ii) developing candidate QIs by performing a systematic review of the literature, and (iii) selec…

Sex-specific impact of glycemic variability on long-term outcomes after acute myocardial infarction.

Acute myocardial infarction (AMI) remains a major cause of global morbidity and mortality. Glycemic variability (GV), reflecting fluctuations in blood glucose levels, has emerged as a prognostic marker in critically ill patients. However, evidence on the sex-specific impact of GV on long-term outcomes after AMI is limited. To evaluate the association between GV and short- and long-term mortality after AMI, with emphasis on sex-based differences. Data of 1666 AMI patients were extracted from a public critical care database. GV was calculated as the coefficient of variation of plasma glucose lev…

Non-ST-Elevation Myocardial Infarction: A Heterogeneous Syndrome with Evolving Management-A Narrative Review.

Non-ST-segment elevation myocardial infarction (NSTEMI) has become the predominant form of acute coronary syndrome (ACS) and is frequently associated with multivessel coronary artery disease (MVD). Patients presenting with NSTEMI and MVD represent a particularly high-risk population characterized by advanced age, comorbidities, and an increased atherosclerotic burden. Although advances in pharmacological therapy and early invasive management have improved prognosis, the optimal revascularization strategy in this setting remains uncertain. In contrast to ST-segment elevation myocardial infarcti…

Integrated left ventricular geometry-function phenotypes and long-term outcomes after acute myocardial infarction.

BACKGROUND/AIMS: Left ventricular (LV) remodeling after acute myocardial infarction (AMI) is a key determinant of long-term outcomes. However, it remains unclear how LV geometry and systolic function interact to influence prognosis in real-world populations. We investigated the combined prognostic impact of LV dilatation and systolic dysfunction in patients with AMI. METHODS: We analyzed 19,664 patients with AMI from nationwide Korean multicenter registries. LV geometry-function phenotypes were defined according to LV end-diastolic dimension (LVEDD ≥53 mm for dilatation) and LV ejection fract…

Macrophage USP7 aggravates acute myocardial infarction by inhibiting STING degradation through the ESCRT pathway.

Macrophage-driven inflammation plays a critical role in the progression of acute myocardial infarction (AMI). This study revealed that USP7 expression was elevated in peripheral blood mononuclear cells (PBMCs) from patients with coronary artery disease (CAD). Furthermore, in a mouse model of myocardial infarction (MI), after AMI cardiac inflammation was suppressed by macrophage-specific USP7 knockout. Mechanistically, USP7 directly interacted with STING and suppressed its K63-linked ubiquitination at residue K289, thereby preventing STING degradation via the endosomal sorting complex required …

Occupational noise exposure and risk of myocardial infarction: results from a prospective Swedish cohort.

OBJECTIVES: To explore the association between occupational noise exposure the preceding year and myocardial infarction (MI) and cumulatively. METHODS: Data came from the Swedish National Cohort on Work and Health, a cohort of all Swedish individuals born between 1930 and 1990. We included individuals with at least one occupational code between 1985 and 2013, which was matched to a job-exposure matrix for occupational noise in five categories (LAeq8h): <70, 70-74, 75-79, 80-84, ≥85 dB(A). First-time MI was ascertained from national patient registers. We used a discrete-time hazards model to a…
