Monitoring of the posterior cricoarytenoid muscle represents another option for neural monitoring during thyroid surgery: Normative vagal and recurrent laryngeal nerve posterior cricoarytenoid muscle electromyographic data.

OBJECTIVE: Intraoperative neural monitoring (IONM) of laryngeal nerves using electromyography (EMG) is routinely performed using endotracheal tube surface electrodes adjacent to the vocalis muscles. Other laryngeal muscles such as the posterior cricoarytenoid muscle (PCA) are indirectly monitored. The PCA may be directly and reliably monitored through an electrode placed in the postcricoid region. Herein, we describe the method and normative data for IONM using PCA EMG. STUDY DESIGN: Retrospective review. METHODS: Data were reviewed retrospectively for thyroid and parathyroid surgery patient

Sustained response of HER2-positive metastatic salivary adenocarcinoma, not otherwise specified, treated with trastuzumab.

OBJECTIVE: Adenocarcinoma-not otherwise specified (AD-NOS) is an aggressive salivary gland carcinoma subtype with poor prognosis. Her2/neu-targeted therapy may be beneficial in cases of overexpression (20% of AD-NOS). Here, a case of metastatic AD-NOS of the submandibular gland showing sustained complete response to trastuzumab is reported, and the existing literature is reviewed. STUDY DESIGN AND RESULTS: A 68-year-old male with poorly differentiated AD-NOS of the submandibular gland with multiple metastases (T3 N2 bM1) underwent radical surgery and adjuvant radiation. The primary lesion dem

Risk factors of thrombosis in a single method of microsurgical head and neck reconstruction: A multi-institutional study of 773 reconstructions with a free jejunal graft after total pharyngolaryngoesophagectomy for hypopharyngeal cancer.

BACKGROUND: The risk factors for thrombus formation in anastomotic vessels in free-flap head and neck reconstruction have been previously reported. However, the evidence is inconsistent. METHODS: In total, 773 patients who underwent free jejunal graft reconstruction after pharyngolaryngoesophagectomy for hypopharyngeal cancer were enrolled at 12 institutions in Japan from 1995 to 2006. Both the resection area and the applied reconstruction method were constrained to overcome the limitations of previous studies. After the exclusion of recurrent cases, odds ratios (ORs) and 95% confidence inter

Primary Surgery vs Chemoradiation Treatment of Advanced-Stage Hypopharyngeal Squamous Cell Carcinoma.

IMPORTANCE: There is no consensus whether primary surgery followed by either adjuvant radiotherapy (RT) or adjuvant chemoradiation (CRT) or definitive CRT should be the standard treatment approach to advanced-stage hypopharyngeal squamous cell carcinoma (HP SCC). OBJECTIVES: To determine survival outcomes for patients with advanced-stage HP SCC treated at a single institution with either primary surgery plus RT or CRT or definitive CRT. EVIDENCE AND DATA ACQUISITION: We conducted a retrospective analysis of prospectively collected medical records in an institutional database for patients wit

Sleep apnoea in patients after treatment of head neck cancer.

CONCLUSIONS: The prevalence of sleep apnoea among patients following treatment of head and neck carcinomas seems to be slightly higher than in the normal population. The possible importance of tumour treatment features, especially transient tracheostoma, needs further assessment. OBJECTIVES: The epidemiology of sleep apnoea in patients following the treatment of head and neck cancer remains unclear. This study was undertaken to assess the prevalence of sleep apnoea in head and neck cancer patients by characterizing their clinical, anatomical and tumour treatment features. PATIENTS AND METHOD

Cardiovascular computed tomography and MRI in clinical practice: aortopathy.

The aorta serves as a lifeline for arterial supply throughout the body, the site of potentially fatal mechanical complications, and a mechanistic partner in hypertension and numerous cardiovascular diseases, thus meriting our careful attention. The following clinical vignette illustrates a case of aortic disease, paying homage to the aorta as we begin a series on cardiovascular imaging in the Journal of Cardiovascular Medicine. This series seeks to present contemporary approaches to diagnosis and management in cardiovascular medicine that include state-of-the-art imaging techniques guided by b

[Carcinosarcoma of the larynx and hypopharynx].

INTRODUCTION: A carcinosarcoma is an uncommon neoplasm with both malignant epithelial and malignant mesenchymal components. The tumor entity has been described in many epithelial organs. Manifestation in the larynx and the hypopharynx is rare. PATIENTS: This report recounts two patients, one with a carcinosarcoma of the vocal cords and the other with a hypopharynx tumour. On the background of the literature we discuss the peculiarities of histology, pathogenesis, clinical features and therapy. DISCUSSION: Carcinosarcomas of the larynx and hypopharynx are typically described in elderly men. S

[Hypopharyngeal carcinomas and radical operation--can one refrain from total laryngeal excision?].

BACKGROUND: Patients with hypopharyngeal carcinoma have a poor prognosis. In reviewing patients treated at our clinic from 1977 to 1990, we … ither by excision of the primary tumor combined with laryngectomy, or by irradiation alone, without neck dissection. The respective 5-year survival rates were 19% for all patients, 11% for the group of radiotherapy alone, and 33% for patients undergoing additional surgery. CONCLUSION: In our opinion less radical surgical procedures are justified for the treatmen …

Results of surgery in primary hyperparathyroidism.

In a prospective study 92 consecutive patients with biochemically proved primary hyperparathyroidism underwent initial neck exploration at Oulu University Hospital. The incidence of multiglandular disease was 34%. 23 patients (25%) underwent a simultaneous thyroidectomy. The cure rate after initial exploration was 91.3%, ectopic parathyroid glands and multiglandular disease being the most common causes of failure. Simultaneous thyroidectomy increased somewhat but not significantly the complication risk. There was a slight tendency for serum calcium concentrations to increase during the mean fo

Glottic cancer. Surgical salvage for radiation failure.

Radiation therapy is a well-established treatment for early squamous cell carcinoma of the glottic larynx. However, the management of patients suspected to have persistent or recurrent disease may present both diagnostic and therapeutic dilemmas, particularly when laryngeal edema persists months after conclusion of radiation therapy. We reviewed the outcome of 212 patients with T1s, T1, and T2NOMO carcinoma of the glottic larynx who had been treated for cure with radiation therapy, with surgical salvage reserved for radiation failures. Twenty-four patients (11%) had recurrences after radiation

A rare case report of vascular Ehlers-Danlos syndrome as a pulsatile neck mass with an internal carotid artery aneurysm.

Vascular Ehlers-Danlos syndrome, also known as Ehlers-Danlos syndrome type IV, is a rare inherited connective tissue disorder caused by autosomal-dominant pathogenic variants in the COL3A1 gene. Defective type III collagen results in marked fragility of arteries and hollow organs, predisposing affected individuals to spontaneous aneurysms, dissections, and rupture. We report the case of a 35-year-old Bangladeshi woman who presented with a gradually enlarging right-sided pulsatile neck mass accompanied by intermittent headache. Physical examination revealed a soft pulsatile cervical swelling wi

Murine Model of Cerebral Venous Outflow Occlusion Through Bilateral Ligation of Jugular Veins.

Lymphatic vessels play a key role in maintaining fluid homeostasis and immune surveillance within tissues. At the brain's border, the dural meninges contain lymphatic vessels closely neighboring venous sinuses. In humans, idiopathic intracranial hypertension (IIH) is associated with stenosis of dural venous sinuses, suggesting that venous outflow disturbances may affect brain fluid regulation and meningeal lymphatic drainage. To explore this relationship in a controlled setting, we developed a mouse model of cerebral venous outflow occlusion via bilateral ligation of both internal and external

Bilateral Vertebral Artery Dissections Associated With Postpartum Preeclampsia: A Case Report.

Postpartum vertebral artery dissections (VADs) are infrequently reported; although data are limited, several case reports describe VAD occurring in the setting of postpartum preeclampsia. We present a 39-year-old G3P3003 (P1 term stillbirth) who developed bilateral VADs in the setting of postpartum preeclampsia and a persistent, nonfocal occipital headache. Six days after an uncomplicated cesarean delivery, the patient developed an unrelenting occipital headache accompanied by severe-range blood pressures. She met diagnostic criteria for postpartum preeclampsia. Despite treatment with magnesiu

A Three-Step Submandibular Retropharyngeal Approach to the Craniovertebral Junction: Is Less Always More?

Background: Accessing the craniovertebral junction poses unique challenges due to its anatomical complexity and proximity to critical structures, such as the cord-brainstem junction, great vessels of the neck, cranial nerves, oropharynx, and rhinopharynx. Among the approaches that have been developed over the years, the submandibular retropharyngeal approach offers good antero-lateral access without the need of transgressing mucosal layers. In its traditional form, however, this approach involves multiple sequential steps and requires intricate dissection, extensive retraction, and meticulous
