An update on calcium metabolism alterations and cardiovascular risk in patients with chronic kidney disease: questions, myths and facts.

In 2006, the Kidney Disease Improving Global Outcomes (KDIGO) guidelines introduced, for the first time, the definition and diagnostic and therapeutic criteria for a systemic complication of the mineral metabolism dysfunction, such as vascular calcification, caused by chronic renal insufficiency. Abdominal x-ray and echocardiography rather than the more complex CT scan is suggested to make the diagnosis. This condition is associated with high cardiovascular risk and consequent poor prognosis. An alteration in total body calcium (Ca) content is one of the key factors in the cardiovascular compl

The impact of an "acute dialysis start" on the mortality attributed to the use of central venous catheters: a retrospective cohort study.

BackgroundCentral venous catheters (CVCs) are associated with early mortality in dialysis patients. However, some patients progress to end stage renal disease after an acute illness, prior to reaching an estimated glomerular filtration rate (eGFR) at which one would expect to establish alternative access (fistula/peritoneal dialysis catheter). The purpose of this study was to determine if exclusion of this "acute start" patient group alters the association between CVCs and mortality.MethodsWe conducted a retrospective cohort study of 406 incident dialysis patients from 1 Jan 2006 to 31 Dec 200

Urinary proteome in a snapshot: normal urine and glomerulonephritis.

The analysis of the urinary proteome is a potential source of information regarding the kidney's physiopathology. A clear knowledge of the protein composition of normal urine is an essential prerequisite to looking at its pathology. Technological evolution in the field of proteomics (2-dimensional [2D] electrophoresis, equalization, mass spectrometry and exosomes) has greatly expanded the power of analysis, allowing the detection of almost 2,000 spots in normal urine, only a minor part of which has been characterized as isoforms of known proteins. The identity of most spots (80%) remains to be

Role of iron deficiency and anemia in cardio-renal syndromes.

Chronic heart failure is a common disorder associated with unacceptably high mortality rates. Chronic renal disease and anemia are two important comorbidities that significantly influence morbidity and mortality in patients with chronic heart failure (CHF). Progress in CHF again may cause worsening of kidney function and anemia. To describe this vicious cycle, the term cardio-renal anemia syndrome has been suggested. Iron deficiency is part of the pathophysiology of anemia in both CHF and chronic kidney disease, which makes it an interesting target for treatment of anemia in cardio-renal anemi

Metabolic syndrome and chronic kidney disease in high-risk Italian hypertensive patients: the I-DEMAND study.

BACKGROUND: Metabolic syndrome (MS) and chronic kidney disease (CKD) are well-known, independent predictors of increased cardiovascular risk. Both conditions are fairly prevalent in the general population. The aim of this study was to assess the relationship between MS or its individual components and CKD in an Italian population of hypertensive patients with normal or mildly to moderately impaired renal function under specialist care. METHODS: A total of 2,916 patients (mean age 62 ± 11 years) among those enrolled in the I-DEMAND study were taken into consideration for this analysis. MS was

Expression of transcription factor Snai1 and tubulointerstitial fibrosis in progressive nephropathy.

BACKGROUND: Tubulointerstitial fibrosis (TIF) is seen as the final stage of progressive nephropathy, and the degree of TIF is reported to be a major determinant in renal outcomes. In recent years, epithelial-mesenchymal transition (EMT) and the zinc-finger transcription factor snail homolog 1 (Snai1) have each been implicated in the mechanism of TIF. The relationship between EMT and these transcription factors is unclear, however, so in this study we attempted to elucidate the correlation between the expression of Snai1 and clinical markers. METHODS: We performed immunohistochemical staining

VEGF and podocytes in diabetic nephropathy.

Vascular endothelial growth factor-A (VEGF-A) is a protein secreted by podocytes that is necessary for survival of endothelial cells, podocytes, and mesangial cells. VEGF-A regulates slit-diaphragm signaling and podocyte shape via VEGF-receptor 2-nephrin-nck-actin interactions. Chronic hyperglycemia-induced excess podocyte VEGF-A and low endothelial nitric oxide drive the development and the progression of diabetic nephropathy. The abnormal cross-talk between VEGF-A and nitric oxide pathways is fueled by the diabetic milieu, resulting in increased oxidative stress. Recent findings on these pat

Increased apoptosis and expression of FasL, Bax and caspase-3 in human lupus nephritis class II and IV.

BACKGROUND: Apoptosis is involved in glomerular injuries leading to glomerulonephritis. However, the role of renal cellular apoptosis in the pathogenesis and progression of human lupus nephritis (LN) is controversial, and studies on the expression of apoptosis-related proteins, such as FasL, Bax and caspase-3, in different classifications of human LN renal tissues are limited. METHODS: Thirty-two samples of LN tissues, including 10 cases of class II and 22 cases of class IV LN, and 5 cases of human normal renal tissues were obtained. Expression of FasL, Bax and caspase-3 proteins in LN tissue

The added-up albumin enhances the diuretic effect of furosemide in patients with hypoalbuminemic chronic kidney disease: a randomized controlled study.

BACKGROUND: Chronic kidney disease (CKD) with edema is a common clinical problem resulting from defects in water and solute excretion. Furosemide is the drug of choice for treatment. In theory, good perfusion and albumin are required for the furosemide to be secreted at the tubular lumen. Thus, in the situation of low glomerular filtration rate (GFR) and hypoalbuminemia, the efficacy of furosemide alone might be limited. There has been no study to validate the effectiveness of the combination of furosemide and albumin in this condition. METHODS: We conducted a randomized controlled crossover

Acceleration of crescent formation by L1 retrotransposon in mouse BSA-induced nephritis.

BACKGROUND: While crescentic glomerulonephritis (GN) is frequently associated with a rapid clinical course and poor prognosis, its pathogenesis has not been elucidated. Bovine serum albumin -induced nephritis (BSA-N), an established mouse model of crescentic GN, is a favorable model for investigation of chronologic gene expression because it is characterized by synchronized progression of crescentic GN. METHODS: Specific genes expressed in glomeruli of crescentic GN were screened using representational difference analysis before and after crescent formation. The obtained LINE-1 (L1) elements

Impact of extrahepatic complications (diabetes and glomerulonephritis) associated with hepatitis C virus infection after renal transplantation.

Hepatitis C virus (HCV) infection is associated with some extrahepatic complications, like diabetes, cryoglobulinemia, and membranoproliferative glomerulonephritis. Each of these complications has important implications in the renal allograft recipient. Hence, HCV infection is strongly associated with a lower patient and graft survival in renal transplantation. The increased risk of death in HCV-infected renal allograft recipients has been attributed to cardiovascular mortality, linked someway to the insulin resistance and high risk of new-onset diabetes, rather than to the progression of HCV-

Pancreas transplant as treatment to arrest renal function decline in patients with type 1 diabetes and proteinuria.

Recent findings from the Diabetes Control and Complications Trial and Epidemiology of Diabetes Interventions and Complications study showed that long-term improved glycemic control in patients with type 1 diabetes with normal renal function and normoalbuminuria can delay development of impaired renal function by at least 6.5 years, although the reduction in the relative risk of end-stage renal disease (ESRD) was not significant. The unanswered question is: can improvement of glycemic control delay the onset of ESRD in patients with established diabetic nephropathy? In this context, pancreas tr

Suboptimal medical care of patients with ST-elevation myocardial infarction and renal insufficiency: results from the Korea Acute Myocardial Infarction Registry.

BACKGROUND: The clinical outcomes of ST-segment elevation myocardial infarction (STEMI) are poor in patients with renal insufficiency. This study investigated changes in the likelihood that patients received optimal medical care throughout the entire process of myocardial infarction management, on the basis of their glomerular filtration rate (GFR). METHODS: This study analyzed 7,679 patients (age, 63 ± 13 years; men 73.6%) who had STEMI and were enrolled in the Korea Acute Myocardial Infarction Registry (KAMIR) from November 2005 to August 2008. The fate of bone marrow-derived cells carrying a Polycystic Kidney Disease mutation in the genetically normal kidney.

BACKGROUND: Polycystic Kidney Disease (PKD) is a genetic condition in which dedifferentiated and highly proliferative epithelial cells form renal cysts and is frequently treated by renal transplantation. Studies have reported that bone marrow-derived cells give rise to renal epithelial cells, particularly following renal injury as often occurs during transplantation. This raises the possibility that bone marrow-derived cells from a PKD-afflicted recipient could populate a transplanted kidney and express a disease phenotype. However, for reasons that are not clear the reoccurrence of PKD has no

Epidemiology and mortality in dialysis patients with and without polycystic kidney disease: a national study in Taiwan.

BACKGROUND: Polycystic kidney disease (PCKD) is one of the most common inherited disorders in end-stage renal disease patients. It is generally thought that the survival of PCKD patients undergoing dialysis surpasses that of general dialysis patients, but the cause of this improved survival is not clear. METHODS: Using Taiwan's national health insurance claims data, we performed a longitudinal cohort study to investigate the survival and impact of comorbidities on mortality in dialysis patients with and without PCKD. We excluded patients without diabetes mellitus (DM) in a further analysis. A

Rationale and design of the RESOLVE trial: lanreotide as a volume reducing treatment for polycystic livers in patients with autosomal dominant polycystic kidney disease.

BACKGROUND: A large proportion of patients with autosomal dominant polycystic kidney disease (ADPKD) suffers from polycystic liver disease. Symptoms arise when liver volume increases. The somatostatin analogue lanreotide has proven to reduce liver volume in patients with polycystic liver disease. However, this study also included patients with isolated polycystic liver disease (PCLD). The RESOLVE trial aims to assess the efficacy of lanreotide treatment in ADPKD patients with symptomatic polycystic livers. In this study we present the design of the RESOLVE trial. METHODS/DESIGN: This open-lab
