Current guidance on the management of acute coronary syndrome.

Acute coronary syndrome (ACS) describes a spectrum of symptoms arising from the development of atherosclerosis. The degree of myocardial ischaemia depends on plaque stability and the extent of vessel occlusion. This article examines underlying pathophysiological processes and reviews current guidance and principles of managing ACS through symptom control, reducing mortality and maximizing secondary prevention. Nurses have a vital role in all aspects of delivering this care and meeting National Service Framework Standards for Coronary Heart Diseases (CHD) (2000). A clear understanding of the pa

Nurses' knowledge about the risk of light cigarettes and other tobacco "harm reduction" strategies.

Light and ultra-light cigarettes are associated with little or no risk reduction versus regular cigarettes, but the majority of smokers believe they are safer. We evaluated whether nurses believe that certain products (light and ultra-light cigarettes, reduced-tar cigarettes, nicotine replacement), cigarette design features (filters, no additives), and smoking behaviors (smoking reduction) reduce health risks among their patients. Nurses (N = 178; 93% female, 10% smokers) completed questionnaires before mandatory training in smoking cessation counseling. Among respondents, 25% believed that li

Exploring Screening Practices for Child Sexual Abuse in School Settings: An Integrative Review.

Child sexual abuse (CSA) is a pervasive public health problem. If left undetected, CSA can result in immediate and long-term health problems, which can be mitigated through early identification. Schools are an ideal environment to implement screening measures, and school nurses (SN) are uniquely poised to intervene and respond early. The aim of this review was to systematically examine and synthesize the international evidence related to screening for early identification of CSA in schools. Themes emerging from the analysis were SN behaviors relative to screening, potential instruments or appr

Virginity Testing.

Virginity testing is a human rights violation involving an examination of the female genitalia, often by a healthcare professional, intended to correlate the appearance of the hymen in women or girls with previous sexual intercourse. Virginity testing examinations are conducted under a variety of circumstances and settings: before marriage to establish virginity, to disprove or prove an allegation of sexual violence, to legally assess the allegation of a woman or girl not being a virgin in the absence of bleeding on her wedding night, and to intimidate political activists and detainees. Despit

Myasthenia gravis.

Myasthenia gravis (MG) is a chronic neuromuscular disorder characterized by fluctuating skeletal muscle weakness and fatigue. This rare autoimmune condition can present a diagnostic challenge for the advanced practice nurse (APN). Because of its low incidence in clinical practice and frequently unrecognized symptoms, a delay of 1 to 2 years prior to diagnosis is not uncommon. Recognition of the clinical features is a key component of prompt diagnosis. To avoid delay in treatment, APNs should be aware of variations in MG symptoms and the tools used to confirm a suspected diagnosis. Because pati

Breastfeeding.

The benefits of breastfeeding for the infant and nursing mother are well established. The World Health Organisation (WHO) recommends exclusive breastfeeding up to 6 mo and continued breastfeeding up to 2 y. Exclusive breastfeeding is the most effective intervention to reduce infant mortality and is estimated to prevent 13 % of under 5 child mortality in low income countries. However, rate of exclusive breastfeeding is alarmingly low in our country. Mothers who face problems in breastfeeding immediately resort to top feeds. It is the responsibility of the health professionals to identify and ma
