Serratia marcescens as a cause of postoperative infection in a total joint implant.

Serratia marcescens is a rare cause of musculoskeletal infections and osteomyelitis occurring most frequently in nosocomial infections, debilitated patients, drug addicts, and traumatic open wounds. It is rarely a cause of postoperative infection in elective surgery with only a few such cases being reported in the United States and European literature. The authors present a case of Serratia marcescens infection of a total joint implant from an apparent perioperative source and review the literature concerning this organism as a cause of infection in man.

The periphysis and its effect on the metaphysis. II. Application to rickets and other abnormalities.

The metaphyseal collar due to the periphysis is lost (in 98% of 49 subjects) as an early sign of rickets and returns with recovery. The term "pseudophysis" refers to that unossified metaphyseal osteoid and cartilage as in rickets. The collar is intact in metaphyseal chondrodysplasias (a helpful sign in differential diagnosis from rickets), is shortened in achondroplasia, and frequently is spared in osteomyelitis, luetic bone disease, and Caffey infantile cortical hyperostosis. The periphyseal bone bark is thinned, but often apparent, in rickets, and is not thinned in cases of tyrosinosis and p…

Ticarcillin-induced neutropenia corroborated by in vitro CFU-C toxicity.

A patient developed a drug rash and neutropenia while receiving tobramycin, ticarcillin and flucloxacillin intravenously for osteomyelitis. Incorporation of these antibiotics into in vitro cultures of bone marrow granulocyte macrophage precursors (CFU-C) showed no inhibition of the patient's marrow or normal marrow by tobramycin. The patient's marrow was more sensitive to ticarcillin than were control cultures, and all cultures incorporating flucloxacillin failed to show growth of CFU-C. Drugs of the penicillin group appear to be responsible for the patient's neutropenia.

[Preventive removal of displaced teeth].

Using the data acquired from our catamnestic survey, we were able to establish a broad catalogue of complications involved with impacted and displaced teeth. Given these complications, it is not medically responsible to leave impacted and displaced teeth in the jaw, even if they are asymptomatic. The risks increase considerably with age. Late operative removal of such teeth can be a dangerous intervention due to local complications such as abscesses, cysts, osteomyelitis, tumors, systemic diseases, and age-related changes in the cardiovascular system.

Pediatric pneumococcal bone and joint infections. The Pediatric Multicenter Pneumococcal Surveillance Study Group (PMPSSG).

OBJECTIVE: To describe the clinical and microbiological characteristics of infants and children with bone and joint infections caused by pen … f children. The mean (+/- standard deviation) duration of total antibiotic therapy in children with osteomyelitis was 57.5 +/- 48.6 days (range, 23-196 days) and 29.2 +/- 11.8 days (range, 12-67 days) for arthritis. Late sequelae (long-term destructive changes of the bone or joint) were documented in 5 (12%) children, 4 with osteomyelitis, and 1 with arthritis. …

Application of vancomycin-impregnated calcium sulfate hemihydrate/nanohydroxyapatite/carboxymethyl chitosan injectable hydrogels combined with BMSC sheets for the treatment of infected bone defects in a rabbit model.

BACKGROUND: The choice of bone substitutes for the treatment of infected bone defects (IBDs) has attracted the attention of surgeons for yea … d and cultured into cell sheets. After the successful establishment of an animal model with chronic osteomyelitis, 48 New Zealand white rabbits were randomly divided into 4 groups. Animals in Group A were treated with thorough debridement as a control. Group B was treated with BMSC sheets. CSH/n-HA/CMCS hydrogels were implanted in the treatment of Group C, and …

Hybrid bone SPECT/CT reveals spleen calcification in sickle cell mutation and beta-thalassemia.

We present a case of a 65 years-old male with sickle cell mutation and beta-thalassemia (Hb S/β-Thal), who had whole-body bone scan evaluation for osteomyelitis. The examination revealed high radiopharmaceutical uptake in the left abdomen. Further evaluation with hybrid single photon emission computed tomography/computed tomography (SPECT/CT) showed calcification of approximately the entire spleen, in the context of sickle cell anemia. This report highlights the role of SPECT/CT in such cases.

First recorded outbreak of Veronaea botryosa in North American amphibians: Clinicopathologic features of a rare cause of phaeohyphomycosis in captive White's tree frogs (Litoria caerulea).

We describe fatal phaeohyphomycosis due to Veronaea botryosa in captive White's tree frogs (Litoria caerulea), the first confirmed report in amphibians in North America. Over 15 months, six frogs developed ulcerative dermatitis on distal extremities/ventrum, which in one animal progressed to vasculitis and necrotizing osteomyelitis. All six frogs died. Clinicopathologic findings, diagnostic challenges, and control are discussed. Emerging fungi such as V. botryosa pose serious concerns for zoonosis and potential spread through the pet trade.

Imaging of Pediatric Bone Tumors and Tumor-like Lesions.

Bone lesions are commonly seen when reporting pediatric skeletal imaging. Distinguishing aggressive from nonaggressive lesions is essential in making the diagnosis. Not all aggressive lesions are neoplastic; indeed, osteomyelitis frequently presents with aggressive appearances and is far more commonly seen in the pediatric population than neoplastic lesions. In this article, we discuss an approach for the diagnosis of pediatric bone tumors and tumor-like conditions. The most common pediatric benign and malignant bone tumors are discussed in more detail.

Congenital insensitivity to pain in one family.

Congenital insensitivity to pain (CIP) is a rare autosomal recessive genetic disease caused by mutation in several different genes. The diagnosis requires the combined skills and cooperation of pediatricians, neurologists, radiologists, pathologists, and orthopedic surgeons. Orthopedic manifestations of CIP include delayed diagnosis of fractures, nonunions, Charcot arthropathy, avascular necrosis, osteomyelitis, joint dislocations, and heterotopic ossifications. We present case reports of two brothers with CIP with various orthopedic manifestations and methods of surgical treatment with 10 yea…

Whole-body MRI: non-oncological applications in paediatrics.

Whole-body magnetic resonance imaging (WBMRI) is a fast and accurate method for detecting and monitoring of diseases throughout the entire body without exposure to ionizing radiation. Among emerging non-oncological potential applications of WBMRI, rheumatological diseases play an important role. Rheumatological WBMRI applications include the evaluation of chronic multifocal recurrent osteomyelitis, dermatomyositis, fever of unknown origin, arthritis, and connective tissue diseases. Aim of this review is to give an overview of the use of WBMRI in rheumatological field.

Prosthetic aspects in adult osteopetrosis.

Osteopetrosis (OP) is a rare condition characterized by skeletal sclerosis caused by dysfunctional osteoclasts. Though many reports have described severe infantile-malignant autosomal recessive OP, few have described the prosthetic management of adult OP. This report discusses the prosthetic treatment of adult OP. Although prosthodontists should try to reconstruct occlusal function as much as possible, a conservative prosthodontic approach may be a reasonable and recommended treatment option for minimizing the risk of further osteomyelitis and osteonecrosis.

Community-acquired disseminated methicillin-resistant Staphylococcus aureus infection: case report and clinical implications.

A 6-year-old girl with community-acquired disseminated infection caused by methicillin-resistant Staphylococcus aureus (MRSA) is described. She had sepsis, meningo-encephalitis, pyomyositis, osteomyelitis, pericarditis and pulmonary embolisation caused by a multi-resistant strain of MRSA. Vancomycin is not routinely recommended as the first-line antimicrobial agent for suspected Staphylococcus aureus infection; however, it should be considered pending susceptibility results in patients presenting with severe sepsis in areas where the prevalence of MRSA is high.

Severe hepatitis associated with oxacillin therapy.

A 6-year-old girl had fever, abdominal pain, and severe anicteric hepatitis during intravenous oxacillin therapy for staphylococcal osteomyelitis. She had greatly elevated liver enzymes, prolonged prothrombin time, leukopenia, and eosinophilia. Clinical symptoms resolved and laboratory data returned to normal after withdrawing oxacillin and substituting cefazolin. This hepatotoxicity appears to be specific to oxacillin and not to other beta-lactams. Monitoring liver function tests during oxacillin therapy, especially in patients receiving prolonged treatment, may be warranted.

Assessing the limping child with skeletal scintigraphy.

Skeletal scintigraphy is frequently used in the clinical investigation of young children who present with limping as their only or predominant symptom. This article reviews techniques used for pediatric skeletal scintigraphy, skeletal tracer distribution in the immature skeleton and scintigraphic manifestations of relatively common conditions that can produce limping in children 1-6 yr old. Acute osteomyelitis, vertebral infections, transient synovitis, septic arthritis, Legg-Calvé-Perthes disease, lower extremity injuries in toddlers and osteoid osteoma are emphasized.

Hyperbaric oxygen therapy. Promoting healing in difficult cases.

Inhalation of pressurized 100% oxygen is a helpful adjunctive treatment for certain patients, because the increased oxygen carried by the blood to the tissue enhances new growth of microcirculation and, thus, healing. Patients with tissue breakdown after radiation therapy, refractory osteomyelitis, gas gangrene, soft-tissue infection with necrosis from mixed aerobic and anaerobic organisms, crush injuries resulting in acute ischemia, and compromised skin grafts or non-healing wounds are likely to benefit from hyperbaric oxygen therapy.

Imaging of acute musculoskeletal infections in children and their differential diagnoses.

Musculoskeletal complaints are a frequent cause of emergency department visits in pediatric patients. While many of these visits are due to acute trauma, infection is also common and can cause substantial morbidity. This article provides a review of acute pediatric musculoskeletal infections with an additional emphasis on important differential diagnoses to consider. The scope includes cellulitis, abscess, necrotizing soft tissue infections, pyomyositis, infectious tenosynovitis, septic bursitis, septic arthritis, and osteomyelitis. Important differential considerations for these musculoskelet…

Nanozyme-Based Strategies against Bone Infection.

Nanozymes are a class of nanomaterials that exhibit catalytic functions analogous to those of natural enzymes. They demonstrate considerable promise in the biomedical field, particularly in the treatment of bone infections, due to their distinctive physicochemical properties and adjustable catalytic activities. Bone infections (e.g., periprosthetic infections and osteomyelitis) are infections that are challenging to treat clinically. Traditional treatments often encounter issues related to drug resistance and suboptimal anti-infection outcomes. The advent of nanozymes has brought with it a new…
