Neuroinflammation and Mental Health in Multiple Sclerosis and Autoimmune Encephalitis: Bridging Biological Mechanisms and Psychosocial Factors.

Autoimmune neuroinflammatory diseases, including multiple sclerosis (MS) and autoimmune encephalitis (AE), are characterized by dysregulated immune activity within the central nervous system that leads to both neurological and psychiatric symptoms. While MS is typically a chronic condition indicated by demyelination and progressive neurodegeneration, AE often presents more acutely through antibody-mediated disruption of synaptic function. Despite differences in disease course and mechanisms, both conditions highlight the significant impact of immune-mediated processes on cognition, mood, and b

Minimal intervention approach for a lesion suggestive of congenital granular cell epulis: a case report.

Congenital granular cell epulis (CGCE), also known as newborn epulis and Neumann tumor, is a rare benign tumor characterized by neoplastic connective tissue proliferation. It typically occurs as a single mass in the maxillary anterior region of the alveolar ridge and seems to have a predilection for females. This case report describes a minimal intervention approach to CGCE that focused on the biopsychosocial well-being of the infant and family. Clinical examination of a 3-month-old male infant with gingival swelling that had been present since birth revealed a reddish, multilobulated, sessile

Taking a spiritual history in the oral oncology setting.

BACKGROUND: Spirituality is recognized as a dimension of health and is increasingly incorporated into supportive cancer care settings. Patients with oral cancer frequently experience profound functional, psychosocial, and existential challenges associated with their diagnosis and treatment, often leading to unmet spiritual needs. Taking a spiritual history provides an approach to identify beliefs, values, and coping resources that influence quality of life and clinical decision-making. OBJECTIVE: To synthesize evidence-based strategies for assessing patients' spirituality within oral oncology

Australian immigration detention health study protocol: a prospective, mixed-methods cohort study examining the physical and mental health of refugees and asylum seekers.

INTRODUCTION: Globally, studies have consistently demonstrated the harmful mental and physical health impacts of immigration detention, with high levels of distress documented among detained asylum seekers and refugees (ASR). However, the consequences of immigration detention over time on the psychological and physical health of ASR are unclear and poorly quantified. METHODS AND ANALYSIS: This prospective, mixed-methods cohort study will recurrently assess and describe the health profiles of adult ASR with an experience of Australian Government-sponsored immigration detention greater than 28

Bidirectional associations between parental negativity and child externalising problems: Social support and neighbourhood cohesion as moderators.

BACKGROUND: Externalising behaviours are among the most common childhood mental health problems and have been linked to numerous adverse psychosocial outcomes including antisocial behaviour and depression. Parental negativity (PNeg) and child behaviours have been shown to mutually influence each other, leading to coercive cycles of negative behaviour over time. Interrupting these negative cycles is a common target for clinical intervention but little is known about what factors moderate these cycles over time in the general population. METHOD: Using data on 9943 families from The Avon Longitu

Brain activation and heart rate variability as markers of autonomic function under stress.

Efficient brain-heart interactions, mediated by the central autonomic network (CAN), are crucial in regulating physiological and psychological stress. The ability of the autonomic nervous system to adapt to stress predicts resilience to cardiovascular, anxiety, and mood disorders. Since the neural dynamics underlying brain-heart interactions remain poorly understood, this study investigated brain activation and heart rate variability (HRV) during stress and relaxation. Functional magnetic resonance imaging (fMRI) and peripheral heart rate assessment were used to assess brain-heart coupling dur

Lifelong Management of Neurofibromatosis 1 Patients.

Neurofibromatosis type 1 (NF1) is a prevalent genetic disorder characterized by a wide spectrum of clinical manifestations, including cutaneous, neurological, and oncological complications. The disease results from mutations in the NF1 gene, which encodes neurofibromin, a tumor suppressor that regulates the RAS/mitogen-activated protein kinase (MAPK) pathway. The loss of neurofibromin function predisposes individuals to both benign and malignant neoplasms, including malignant peripheral nerve sheath tumors, optic pathway gliomas, and gastrointestinal stromal tumors. Additionally, women with NF

[Integrated youth mental health services - Low-threshold, integrated programs for young people in mental health crises : International experiences and the current situation in Germany].

BACKGROUND: Young people in mental health crises have severely limited access to clinical care and show the lowest adherence to conventional clinical services. Low-threshold and integrated services (integrated youth mental health services, IYMHS) are recommended to overcome these barriers and provide early detection, early intervention and psychosocial care appropriate for young people. OBJECTIVE: The aim is to provide an overview of low-threshold IYMHS globally and in Germany and to report on international experiences with these services and evidence of their effectiveness. METHODS: The nar

Symptom and Medication Burden in Home-Based Palliative Care Patients.

Home-based palliative care (HBPC) improves quality of life by addressing distressing physical, psychosocial, and spiritual symptoms. However, symptom and medication burdens remain prevalent challenges. This study sought to characterize symptom prevalence and severity, the associated medication burden, and determine opportunities for deprescribing in the setting of HBPC. Patients referred to a home-based palliative care practice over a two-year period were included; 109 patients met inclusion criteria. Mean age was 78 years (± 11.8), and average Charlson Comorbidity Index (CCI) was 6.4 (± 2.7).

Respiratory Manifestations and Their Physical, Psychological, and Social Impacts in Ehlers-Danlos Syndromes and Generalized Hypermobility Spectrum Disorders: A Narrative Review.

Ehlers-Danlos Syndromes (EDS) and Generalized Hypermobility Spectrum disorders (G-HSD) are a group of genetic, connective multi-systemic disorders that can affect the musculoskeletal, gastrointestinal, cardiovascular, and respiratory systems. Respiratory manifestations in EDS/G-HSD can contribute to decrements in health-related quality of life (HRQL); however, these relationships have not been previously characterized. We aimed to review the association of respiratory manifestations with the physical, psychological, and social domains of HRQL in EDS/G-HSD. A comprehensive search was conducted

From Hopelessness to Hope: Addressing Demoralization in Parkinson's Disease.

BACKGROUND: Demoralization is a pervasive yet under-recognized syndrome that manifests as profound hopelessness, helplessness, loss of motivation and purpose, and impaired coping capacity. Affecting over one-third of individuals in medical and neurological settings and 20-30% of the general population, demoralization is frequently misclassified as depression. However, its distinct phenomenology necessitates precise differentiation, particularly in Parkinson's disease (PD), where it can profoundly impact quality of life (QoL), social engagement, and treatment adherence, potentially contributing
