We are reporting 4 cases of unnoticed sacral fractures in osteoporotic women. Their mean age was 73 years. Only in one case the fracture was seen on a standard X-Ray. On the other hand, hyperfixation with isotopes on the sacrum formed a very characteristic H-shaped image. In two of these patients, the sacral fracture was confirmed by tomodensitometry, but this examination was unavailing in the third patient. In the 4th case, hyperfixation of the isotope was not significant. Only a guided bone biopsy and the favorable evolution of the disease enabled the diagnosis of sacral fracture.

Many complications can occur following insertion of silicone elastomer implants into the foot. Postoperative infection may be difficult to distinguish from other conditions such as dislodgment, fracture, ectopic and heterotopic new bone formation, synovitis, and bursitis. White blood cell scintigraphy, in conjunction with the clinical scenario, may prove to be an invaluable tool in the diagnosis of postoperative osteomyelitis, subsequent to implant arthroplasties.

Tuberculosis of the liver in association with generalized miliary tuberculosis is not an uncommon clinical entity. A solitary tuberculous liver abscess, however, is rare. Two such cases, in which there was no clinical evidence of extrahepatic tuberculosis, are reported. The similarities between this condition and other causes of hepatic lesions are discussed, with particular emphasis placed on the imaging patterns of various diagnostic imaging procedures.

Eight children and adolescents, seven female and one male, aged 7.1 to 15.0 years, referred over a 12-year period for a solitary mass in an otherwise normal thyroid gland, exhibited a hyperfunctioning nodule on thyroid scintiscan. Tracer uptake in the surrounding thyroid tissue was reduced or completely suppressed, but could be restored after TSH stimulation. Only one patient had mild clinical hyperthyroidism with normal T4 but increased T3 serum levels and blunted TSH responsiveness to TRH. Iodine-123-4-amino-3-iodobenzylguanidine ([123I]AIBG), an analog of 123I metaiodobenzylguanidine ([123I]MIBG), has an advantage in having a more rapid and simple synthesis. 99mTc-iminodiacetic acid (IDA) compounds are concentrated by hepatocytes and excreted into the biliary system. The utility of 99mTc-IDA imaging in 116 patients with suspected acute cholecystitis was studied retrospectively over a 2-year period. Visualization of the gallbladder ruled out obstructed cystic duct and acute cholecystitis with a high degree of accuracy.

Using the method of equilibrium radionuclide ventriculography (RNV), the right ventricular ejection fraction (RVEF) at rest and at a standard workload of 250 kpm per min was determined in 25 control subjects and in 30 patients with pulmonary hypertension (8 patients with chronic obstructive bronchopulmonary disease, 12 with recurrent pulmonary embolism and 10 with pure mitral stenosis). A prospective study of 11 patients who had undergone two sequential hepatobiliary scans prior to surgery demonstrated that initially positive scans could be due to acute cholecystitis, or chronic cholecystitis with exacerbation. A second hepatobiliary scan performed four to five days later differentiated acute from chronic cholecystitis. All five patients with surgery-proven acute cholecystitis remained hepatobiliary-positive, while patients with surgery-proven chronic cholecystitis reverted back to negative hepatobiliary scans if the second hepatobiliary scan was done properly.

Gamma-camera imaging of the abdomen after injection of autologous 111In-labelled platelets was applied for localization of gastrointestinal bleeding in a study of 22 patients. In 15 studies showing scintigraphic signs of bleeding, the clinical presentation included occult bleeding in 6, melaena in 4, and bloody stools in 5. Technetium-99m-Sn-phosphate colloid was prepared in the presence of polyvinylpyrollidone(PVP), molecular weight 44,000, for bone marrow imaging. Size of the colloidal particles, as determined by coulter counter, microphotographic and electron microscopic studies was 15-35 nm. Ninety-nine patients with alcohol related problems underwent a liver scintiscan and biopsy. The scan changes were graded and compared with the histological changes found from percutaneous liver biopsy. The overall correlation between scan and biopsy changes was poor. However, scintiscanning was found to be highly sensitive (90%) in detecting cirrhosis and for this reason is useful as a screening test in selecting alcoholic patients for biopsy.

In a review of 60 patients with fatty infiltration of the liver documented by Xe-133 imaging, 43% had normal radiocolloid liver images, and 57% had abnormal images with various combinations of hepatomegaly, mottling, splenomegaly, and splenic shift of radioactivity. Endothelial injury associated with cyclosporine (CSA) therapy in the absence of rejection has resulted in irreversible intrarenal allograft thrombosis and transplant loss. Emphasis is placed on the existence of two types of dilatation of the upper excretory tract: obstructive and non-obstructive lesions. Endometriosis is a common disorder, affecting women in the child-bearing years. While urologic involvement is rare, the bladder is more often affected than the ureter. The authors describe the case of a 30-year-old woman who had unilateral ureteral obstruction secondary to an isolated periureteral retroperitoneal area of endometriosis. The theories of histogenesis are discussed. Current treatment consists of surgery or hormonal manipulation or a combination of the two.
