Adjunctive Traditional Chinese Medicine for Osteonecrosis of the Femoral Head: A Systematic Review and Meta-Analysis of Randomized Trials.

Osteonecrosis of the femoral head (ONFH) is a progressive orthopedic disorder that frequently results in femoral head collapse and substantial functional impairment. Hip-preserving surgical procedures remain the primary treatment strategy for early- and mid-stage disease; however, therapeutic outcomes remain variable. Traditional Chinese medicine (TCM) has increasingly been investigated as an adjunctive therapy because of its potential effects on microcirculation, bone remodeling, and inflammatory regulation. This systematic review and meta-analysis evaluated whether adjunctive Chinese herbal

Effectiveness of Interactive Remote Rehabilitation After Total Knee Arthroplasty: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Remote rehabilitation has become increasingly relevant after total knee arthroplasty (TKA). However, existing reviews have often pooled interventions with markedly different levels of clinician involvement, making it difficult to determine whether interactive, feedback-enabled models provide distinct clinical value. OBJECTIVE: This systematic review and meta-analysis evaluated the effectiveness of interactive remote rehabilitation (IRR), defined as technology-enabled rehabilitation involving bidirectional communication between patients and health care providers, compared with conv

The Effect of Medical Therapies for Subthreshold Abdominal Aortic Aneurysm Growth and Mortality: A Network Meta-Analysis of Randomized Controlled Trials.

OBJECTIVES: Abdominal aortic aneurysm (AAA) is often fatal when ruptured, and current guidelines suggest surgical management at suprathreshold sizes (50 mm for women or 55 mm for men) or with rapid expansion (>5 mm/year). Many medical therapies have been assessed for reducing subthreshold AAA expansion, though the evidence remains inconclusive. This network meta-analysis (NMA) compares AAA growth and mortality amongst medical treatments for AAAs. METHODS: MEDLINE (via PubMed), Scopus, Web of Science, EBSCO, and Cochrane Library databases were searched for relevant randomized controlled trials

Placebo or active comparator in chronic migraine trials: Balancing evidence and practicality.

This debate addresses the choice of placebo versus active comparator in randomized controlled trials (RCTs) for chronic migraine (CM), a disabling condition with high global burden. Placebo-controlled designs have traditionally been considered the gold standard for new treatments, allowing quantification of placebo and pharmacological effects, ensuring internal validity, smaller sample sizes, and regulatory acceptance. Yet, ethical concerns arise from the possibility that participants may be denied effective treatments. The emergence of migraine-specific therapies targeting the calcitonin gene

Ceiling effects, mechanistic mismatch, and the need for standardized reporting in autologous concentrates for temporomandibular joint disorders.

This letter addresses methodological considerations in a recent randomized controlled trial comparing platelet-rich plasma (PRP) and hyaluronic acid (HA) after temporomandibular joint (TMJ) arthrocentesis. We highlight a ceiling effect in which the control group achieved near-complete symptom resolution, leaving no room for adjunctive treatments to demonstrate superiority. We discuss the mechanistic mismatch between regenerative (PRP) and biomechanical (HA) approaches and emphasize the critical need for standardized PRP reporting. A systematic review found that only 10% of PRP clinical trials

Tackling Anxiety- and Stress-Related Freezing of Gait in People With Parkinson's Disease (TACKLING-FOG): Study Protocol for a Randomized Controlled Trial.

Freezing of gait (FOG) is a common and disabling symptom in people with Parkinson's Disease (PD), characterized by paroxysmal episodes where there is an inability to step effectively, despite attempting to do so. Anxiety and stress exacerbate FOG, particularly in situations where people with FOG anticipate not being in control of their movements. Some people with PD use compensatory strategies that target anxiety and stress to improve FOG. However, tailored strategies to ameliorate anxiety- and stress-related FOG have never been evaluated in a systematic manner. We describe the study protocol
