Tympanometric evaluation of Eustachian tube function in Polish scuba divers.

BACKGROUND: Scuba divers are subjected to relatively high ambient pressures while descending. Equalizing maneuvers (e.g., Valsalva) are necessary to open the Eustachian tube (ET) and allow air into the middle ear (ME) cavity. Insufficient opening of the ET leads to ME barotrauma, which is the most common injury related to scuba diving. The study aims were to assess the incidence of ME barotrauma and to compare tympanometric parameters and stapedial reflexes in scuba divers and non-diving individuals. MATERIAL AND METHODS:: 60 scuba divers participated in the study; control consisted of 90 non…

Vagal Control of Breathing Pattern after Midcervical Contusion in Rats.

The present study was designed to establish a midcervical contusion model that can simulate long-term respiratory deficits, and investigate the breathing pattern during vagal-mediated respiratory reflexes following midcervical contusion. Moderate and severe (impactor height: 6.25 or 12.5 mm) contusion was induced at midline C3-4 spinal cord in adult Sprague-Dawley rats. The ventilatory behaviors of unanesthetized were evaluated by whole body plethysmography at 1 day and 1, 2, 4, and 8 weeks post-injury. The tidal volume was decreased and frequency was increased in contused animals compared wit…

Latah: an Indonesian startle syndrome.

The nature of culture-specific startles syndromes such as "Latah" in Indonesia and Malaysia is ill understood. Hypotheses concerning their origin include sociocultural behavior, psychiatric disorders, and neurological syndromes. The various disorders show striking similarities despite occurring in diverse cultural settings and genetically distant populations. They are characterized clinically by exaggerated startle responses and involuntary vocalizations, echolalia, and echopraxia. Quantifying startle reflexes may help define Latah within the 3 groups of startle syndromes: (1) hyperekplexia, (…

Resveratrol ameliorates hypoxia/ischemia-induced behavioral deficits and brain injury in the neonatal rat brain.

Hypoxia-ischemia (HI) induced injury of the neonatal brain accounts for behavioral deficits concerning mainly neurological reflexes, sensorimotor functions and learning/memory disabilities that may evolve throughout development. The positive biological effects of resveratrol, a natural compound with anti-oxidant/anti-inflammatory properties found mainly in red wine have been indicated recently. Aim of this study was to investigate the delayed outcome of early administration of resveratrol in an experimental model of hypoxic-ischemic encephalopathy, by means of behavioral analysis and late neur…

Successful management of a dog that had severe rhabdomyolysis with myocardial and respiratory failure.

CASE DESCRIPTION - A 6-year-old castrated male Llewelyn Setter was evaluated because of an acute onset of myalgia and respiratory distress. CLINICAL FINDINGS - Physical examination revealed a stiff stilted gait, swollen muscles that appeared to cause signs of pain, panting, and ptyalism. The dog had a decrease in palpebral reflexes bilaterally and a decrease in myotatic reflexes in all 4 limbs. The panniculus reflex was considered normal, and all other cranial nerve reflexes were intact. Serum biochemical analysis revealed markedly high cardiac troponin-I concentration and creatine kinase and …

A neurologic basis for the overactive bladder.

OBJECTIVES: The functions of the lower urinary tract (LUT) to store and periodically release urine are dependent on neural circuits in the b … g to the neurogenic overactive bladder. RESULTS: Normal storage of urine is dependent on 1) spinal reflex mechanisms that activate sympathetic and somatic pathways to the urethral outlet and 2) tonic inhibitory systems in the brain that suppress the parasympathetic excitatory outflow to the urinary bladder. Voiding is mediated by inhibition of sympathet …

Spasticity following spinal cord injury.

Spasticity is a hyper-excitable state of the reflex arcs in the spinal cord below the level of injury. Not only is the skeletal motor system affected, but bladder, bowel, blood pressure, and erection reflex mechanisms are also involved. Spasticity gradually emerges from the initial phase of spinal shock one to two months after the injury, and usually reaches a plateau of a mild to moderate degree in 3 to 4 months. Neurophysiological mechanisms indicate an increase in the alpha and gamma reflex systems and that central excitability through the interneurons is also involved in these systems. Exc…

Mitochondrial-neuroimmune interfaces in post-stroke spasticity: from acute brain injury to chronic motor phenotypes.

Post-stroke spasticity is a common and clinically consequential manifestation of the upper motor neuron syndrome, yet its mechanisms are incompletely explained by stretch reflex hyperexcitability alone. Established models emphasize corticospinal and corticoreticulospinal injury, altered brainstem descending drive, spinal reflex amplification, impaired inhibitory control, and secondary changes in skeletal muscle and connective tissue. In parallel, stroke induces profound mitochondrial stress and neuroimmune activation, including bioenergetic failure, mitochondrial reactive oxygen species produc…

Use of transcutaneous spinal cord stimulation to explore inhibitory and facilitatory circuits in muscles of the human lower limb.

The aim of this study was to explore the primary afferent depolarization mechanism, to determine whether the soleus transspinal evoked potential (TEP), elicited through transcutaneous spinal cord stimulation over the L1-L2 level, is modulated by presynaptic inhibition and heteronymous facilitation, similar to the Hoffmann (H) reflex, elicited by posterior tibial nerve stimulation. Twenty subjects participated in two experiments. Experiment 1 assessed D1 and D2 inhibition by conditioning the H reflex and TEP with peroneal nerve stimulation at different interstimulus intervals (ISIs; ranging fro…

Cervical and thoracolumbar transspinal stimulation at 0.3 Hz and 30 Hz facilitates neuronal excitability in healthy humans: a randomized cross-over study.

Effective neuromodulation protocols for upper motor neuron lesions are in great need. Based on the significant role of intraspinal neuronal pathways in motor control, we investigated here the effects of multisite cervical and thoracolumbar transspinal stimulation on arm and leg corticospinal and spinal reflex excitability. This was a randomized cross-over study in which 10 healthy subjects received either 0.3–30 Hz multisite transspinal stimulation on different days. Stimulation at either frequency was delivered at alternating subthreshold and suprathreshold intensities in blocks of 10 min for…
