Case Report: Successful Use of Biliary Stent for Iatrogenic Esophageal Perforation Following Balloon Dilation in a 7-Month-Old Infant.

Esophageal perforation is a rare but critical emergency that requires early detection and prompt management. In the pediatric population, iatrogenic injury is the most common etiology of esophageal perforation, and the majority of cases come from stricture dilation. Treatment options include medical management, endoscopic therapy, and surgery. Usually, conservative treatment is appropriate in most carefully selected patients, especially in the setting of early diagnosis and with the absence of severe sepsis. A surgical approach is reserved for a large tear with mediastinum contamination, or cl

ACOG Practice Bulletin No. 210: Fecal Incontinence.

Fecal incontinence, or the involuntary leakage of solid or loose stool, is estimated to affect 7-15% of community-dwelling women (1). It is associated with reduced quality of life, negative psychologic effects, and social stigma (2), yet many women do not report their symptoms or seek treatment. Less than 3% of women who do self-report fecal incontinence will have this diagnosis recorded in their medical record (3). Obstetrician-gynecologists are in a unique position to identify women with fecal incontinence because pregnancy, childbirth, obstetric anal sphincter injuries (OASIS), and pelvic f

Managing acute postoperative pain: is 3 hours too long?

This case study discusses a 64-year-old opioid-tolerant patient who underwent amputation below the left knee and received pain management in the PACU. The patient's self-reported pain level remained 9 on a 0 to 10 scale despite having received a total of 62 mg of morphine sulfate (MSO 4 ) and 60 mg of ketorolac in just less than 3 hours. The patient's facial expression corresponded to a score of 4 to 5 based on the Wong-Baker Faces Pain Scale. This case study illustrates that it is crucial to promptly involve the pain management service when an opioid-tolerant patient requires aggressive pain

Equine self-mutilation syndrome (57 cases).

Owners who believed they had a horse with self-mutilating behavior were asked to complete a questionnaire to obtain information on the signalment, clinical history, clinical signs, management, and treatment of the condition. Fifty-two owners of 59 horses responded; 2 cases were omitted because an organic cause was determined. Owners also were asked to complete information for unaffected horses on the same farm. Various breeds were affected, and of the 57 horses, 20 were stallions, 31 were geldings, and 6 were mares. Affected horses tended to develop the condition prior to sexual maturity. Clin

Racial and ethnic disparities in diabetes-related healthcare service use.

BACKGROUND: Compared to non-Hispanic White adults, racial/ethnic minority adults have a higher risk of developing diabetes and its complications. Proper use of healthcare services is important for reducing the prevalence of diabetes and the risk of complications. However, minority populations tend to receive a lower quality of healthcare service and have greater barriers to diabetes self-management. This study aims to determine racial/ethnic disparities in the utilization of diabetes-related healthcare services and estimate factors that are associated with the utilization. METHODS: This study…

BACKGROUND: Diabetes complicates ≤7% of pregnancies in the United States. Although medical nutrition therapy is the mainstay of diabetes treatment, many barriers exist to the successful implementation of dietary modifications. Home-delivered medically tailored meals (MTMs) are promising to overcome such barriers. OBJECTIVE: The objective of this study was to evaluate the feasibility and acceptability of home-delivered MTM in pregnant patients with diabetes. METHODS: We performed a prospective cohort study of home-delivered MTM for pregnant patients with diabetes using a mixed-methods approac

Endovascular Repair of an Ascending Aorta Dissection After Self-expanding TAVR by Suprasternal Approach.

Management of rare ascending aortic complications after transcatheter aortic valve replacement (TAVR) remains a poorly defined area of knowledge. Furthermore, because most patients undergoing TAVR are at intermediate to high surgical risk, the treatment of such complications often requires endovascular approaches. Herein, we present a novel technique of endovascular ascending aorta stent graft delivery from the innominate artery for the treatment of a type A aortic dissection after self-expandable TAVR implantation, specifically addressing the landing of the aortic stent within the crown of th
