Treatment for Symptomatic SLAP Tears in Middle-Aged Patients Comparing Repair, Biceps Tenodesis, and Nonoperative Approaches: A Cost-Effectiveness Analysis.

PURPOSE: To evaluate the cost-effectiveness of nonoperative management, primary SLAP repair, and primary biceps tenodesis for the treatment of symptomatic isolated type II SLAP tear. METHODS: A microsimulation Markov model was constructed to compare 3 strategies for middle-aged patients with symptomatic type II SLAP tears: SLAP repair, biceps tenodesis, or nonoperative management. A failed 6-month trial of nonoperative treatment was assumed. The principal outcome measure was the incremental cost-effectiveness ratio in 2017 U.S. dollars using a societal perspective over a 10-year time horizon.

Diagnostic Accuracy of Clinical Examination and Imaging Findings for Identifying Subacromial Pain.

BACKGROUND: The diagnosis of subacromial pathology is limited by the poor accuracy of clinical tests for specific pathologies. The aim of this study was to estimate the diagnostic accuracy of clinical examination and imaging features for identifying subacromial pain (SAP) defined by a positive response to diagnostic injection, and to evaluate the influence of imaging findings on the clinical diagnosis of SAP. METHODS AND FINDINGS: In a prospective, diagnostic accuracy design, 208 consecutive patients presenting to their primary healthcare practitioner for the first time with a new episode of

Incidence of risk in shoulder surgery : medical-legal repercussions.

It is generally recognised that any medical-surgical activity entails the risk of failure, placing the physician at risk of malpractice claims. In order to protect both the physician and the patient, it is important to identify the risk factors relating to the various pathological situations and assess both the incidence and significance of such risks. Hence, a "Potential Failure Rating" could be devised already during the preliminary stages for every type of medical process. Such a rating would be useful both in clinical and medicolegal settings. This article considers the most common and imp

Posterior capsulolabral reconstruction in posterior shoulder instability: deltoid saving.

To determine the efficacy of an open posterior capsulolabral reconstruction (PCLR) with a posterior deltoid-saving approach, 30 PCLRs in 29 patients with unidirectional posterior instability were reviewed retrospectively. The mean follow-up period was 30 months (range, 25-59 months). Posterior capsular redundancy was observed in all cases, but a posteroinferior labral tear was found in only 5. Posterior capsular thinning developed in 6 patients. According to the Rowe scale, 24 shoulders were rated as excellent, 1 as good, 2 as fair, and 3 as poor. The modified American Shoulder and Elbow Surge

Painful shoulder in hemiplegia.

Shoulder pain is probably the most frequent complication of hemiplegia. In this study 219 hemiplegia patients were regularly followed up after their cerebrovascular accident (CVA) for one year (166 men, 53 women, with a mean age of 47 years). Criteria and parameters for evaluation of these shoulders were established at the outset. Distinction was made between flaccid and spastic hemiplegia. Other influencing factors were subluxation reflex sympathetic dystrophy syndrome (RSD), isolated tendon lesion cuff rotator tear or association of some of these. Roentgen examinations were done for each pat

Joint pain and osteoarthritis in former recreational and elite cricketers.

BACKGROUND: Sport participants are at increased risk of joint pain and osteoarthritis. A better understanding of factors associated with joint pain and osteoarthritis in this population could inform the development of strategies to optimise their long-term joint health. The purpose of the study was to describe the prevalence of joint pain and osteoarthritis in former cricketers, and determine whether playing position, playing standard (i.e. elite or recreational standard) and length-of-play are associated with region-specific joint pain. METHODS: The data were from the Cricket Health and Well

Return to sport after arthroscopic double endo-button fixation for acute acromioclavicular dislocation: One-year functional outcomes.

INTRODUCTION: Acromioclavicular joint dislocation (ACJD) is a common injury among athletes. Few studies have investigated clinical outcomes following arthroscopic surgery for high-grade acute ACJD. This study aims to determine whether arthroscopic stabilization using a double-button fixation system enables an optimal return to sports in terms of timing and level, with a minimum follow-up of one year postoperatively. MATERIALS AND METHODS: This prospective, single-center study included patients who underwent arthroscopic stabilization with a double-button fixation system for severe acute ACJD

Advancing glenohumeral dysplasia treatment in brachial plexus birth injury: the end-to-side spinal accessory to suprascapular nerve transfer technique.

PURPOSE: Brachial plexus birth injury (BPBI) is a common injury with the spectrum of disease prognosis ranging from spontaneous recovery to lifelong debilitating disability. A common sequela of BPBI is glenohumeral dysplasia (GHD) which, if not addressed early on, can lead to shoulder dysfunction as the child matures. However, there are no clear criteria for when to employ various surgical procedures for the correction of GHD. METHODS: We describe our approach to correcting GDH in infants with BPBIs using a reverse end-to-side (ETS) transfer from the spinal accessory to the suprascapular nerv

Results of latissimus dorsi tendon transfer for irreparable cuff tears.

Rupture of the supraspinatus and infraspinatus tendon (and teres minor) can cause loss of active external rotation (ER), entailing severe functional disability in daily activities. Latissimus dorsi tendon transfer (LDTT), proposed by Gerber in 1988, appears to be the best adapted solution in these cases of irreparable posterior and superior cuff tears. Between 2001 and 2004, 30 patients were operated on by the technique described by Gerber, with the transfer fixed anteriorly to the subscapularis tendon and laterally to the greater tuberosity by transosseous suture. One patient, subsequently re

Return to contact criteria and assessment methods following traumatic anterior glenohumeral joint dislocations in rugby union players: a Delphi consensus study.

OBJECTIVES: Traumatic anterior glenohumeral joint dislocations are associated with prolonged time loss and high rates of reinjury in rugby. This is particularly common during contact events. There is a lack of clear guidance to facilitate return to contact decisions following these injuries in rugby. The aim of the study was to identify and reach consensus (≥70% agreement) regarding return to contact criteria and assessment methods following traumatic anterior glenohumeral joint dislocations in rugby union players. METHODS: A three-round Delphi consensus study was conducted. RESULTS: Thirty-

Editorial Commentary: Improved Suture Configurations Can Enhance Shoulder Rotator Cuff Healing: The Cuff Doesn't Always Heal-But We Can Nudge It in the Right Direction.

The success of rotator cuff repair is often measured by patient-reported outcomes or structural healing on magnetic resonance imaging. Ideally, we can achieve both a satisfied patient and a healed tendon. Various technical modifications and stitch patterns have been presented in an effort to achieve the perfect rotator cuff repair. It appears the independently popular suture bridge technique and Mason-Allen stitch may do their best work when combined together. First, double-row repair "anchors down" the tendon to its footprint, and suture bridge adds to this concept with double-row compressive
