Advanced-Stage Gonadal Dysgerminoma in a Patient With a Previous Diagnosis of Familial Swyer Syndrome: A Very Rare Genetic Entity.

INTRODUCTION: Swyer syndrome is a genetic abnormality characterized by a 46,XY karyotype in a phenotypically female individual. Affected ind … the significant risk of malignant gonadal tumors that may arise at an early age. Prepubertal female siblings of patients diagnosed with Swyer syndrome should be screened. Early diagnosis and prompt prophylactic gonadectomy can allow for a conservative treatment plan that may preserve fertility and improve patient survival.

Novel Homozygous Pathogenic Variant in DNAJC6 Causes Rapid Progressive Parkinsonism.

BACKGROUND: Biallelic variants in DNAJC6 cause PARK-DNAJC6 (historically referred to as PARK19), a rare autosomal recessive juvenile-onset dystonia-parkinsonism syndrome. Neurodevelopmental delay or regression, epilepsy, movement disorders, and psychiatric symptoms are also present. Progression is fast with frequent bulbar, gastrointestinal involvement and early loss of ambulation. CASES: We report two siblings (17 and 14 years) harboring a novel homozygous nonsense variant in DNAJC6, p.Cys325*, not previously described in the medical literature and absent from population databases. The varia…

Impact of lipase and lipoxygenase activity on the aroma profile of whole wheat bread.

Whole wheat flour is prone to lipid oxidation during storage and during the breadmaking process that can negatively impact the flavor profile. The impact of flour enzymatic lipid oxidation on aroma formation in whole wheat bread was investigated. Bread was made from two types of flour, a lipase/lipoxygenase knock-out and the corresponding wild-sibling control, stored at -40 °C (nonaged) and 37 °C (aged) for 8 weeks. Descriptive sensory analysis identified five key aroma attributes of the bread samples: fermented/yeasty, cardboard, fruity, roasty, and bran. A comprehensive gas chromatography-ol

Discovering the pathogenesis of a VUS variant in CDH23 associated with sensorineural hearing loss in an Iranian family.

BACKGROUND: Hearing loss (HL) is a highly heterogeneous condition with concerning statistics, particularly given the significant occurrence of consanguineous marriages in Iran. Despite the presence of variants of uncertain significance (VUS), high-yield prenatal screening remains unattainable. This article aims to discuss a VUS mutation as a potential cause of HL in a sibling, with the goal of reclassifying this variant. METHODS: This research examined a sibling with congenital HL. Their parents were first cousins, and no non-genetic factors for the disease were identified. Whole exome sequen

[Acquired severe aplastic anemia in emerging countries: Management from allogeneic hematopoietic cell transplantation indication until post-transplant follow-up SFGM-TC].

Management of acquired aplastic anemia (AA) in emerging countries depends on the means of prognostic stratification, treatment and logistics available. During the 13th annual harmonization workshop of the francophone Society of bone marrow transplantation and cellular therapy (SFGM-TC), a designated working group reviewed the literature in order to elaborate unified guidelines for allogeneic hematopoietic cell transplantation (Allo-HCT) in this disease. In terms of practice, the conclusions are as follows; The use of anti-tymocyte globuline (ATG) is mainly from rabbit and very little from hors

Birth order and sickness absence: Register-based evidence from Finland.

BACKGROUND: In working ages, sickness absence is strongly related to persons' health condition. We studied how birth order was associated with receipt of sickness allowance, distinguishing between mental disorders, musculoskeletal disorders and injuries. METHODS: A follow-up study based on the entire Finnish population was conducted for sibling groups born 1969-1982, in which each sibling was observed from age 35 years in the period 2004-2018. Focus was on within-family variation in first-time sickness allowance receipt. RESULTS: Results of stratified Cox regressions revealed that each incre

A novel mutation in GJC2 associated with hypomyelinating leukodystrophy type 2 disorder.

Hypomyelinating leukodystrophy type 2 (HLD2), is an inherited genetic disease of the central nervous system caused by recessive mutations in the gap junction protein gamma 2 (GJC2/GJA12). HLD2 is characterized by nystagmus, developmental delay, motor impairments, ataxia, severe speech problem, and hypomyelination in the brain. The GJC2 sequence encodes connexin 47 protein (Cx47). Connexins are a group of membrane proteins that oligomerize to construct gap junctions protein. In the present study, a novel missense mutation gene c.760G>A (p.Val254Met) was identified in a patient with HLD2 by perf

White matter abnormalities and iron deposition in prenatal mucolipidosis IV- fetal imaging and pathology.

Mucolipidosis type IV (MLIV; OMIM 252,650) is an autosomal recessive lysosomal disorder caused by mutations in MCOLN1. MLIV causes psychomotor impairment and progressive vision loss. The major hallmarks of postnatal brain MRI are hypomyelination and thin corpus callosum. Human brain pathology data is scarce and demonstrates storage of various inclusion bodies in all neuronal cell types. The current study describes novel fetal brain MRI and neuropathology findings in a fetus with MLIV. Fetal MRI was performed at 32 and 35 weeks of gestation due to an older sibling with spastic quadriparesis, vi

A novel FBXO7-R345P mutation in a Chinese family with autosomal recessive parkinsonian-pyramidal syndrome.

BACKGROUND: Mutations in the F-box protein 7 (FBXO7) gene is one of the genetic causes of early-onset Parkinson's disease, which usually presents as autosomal recessive early-onset parkinsonian-pyramidal syndrome (PPS). Herein, we report a Chinese PPS family with a novel FBXO7 homozygous mutation. METHODS: Clinical data of the proband and his affected sister manifesting as early-onset parkinsonism combined with pyramidal signs were collected. DNAs of the two affected siblings, an unaffected sibling and their unaffected mother were isolated. Whole-exome sequencing (WES) was performed for the p…

A Novel Pathogenic Variant in CARMIL2 (RLTPR) Causing CARMIL2 Deficiency and EBV-Associated Smooth Muscle Tumors.

CARMIL2 deficiency is a rare combined immunodeficiency (CID) characterized by defective CD28-mediated T cell co-stimulation, altered cytoskeletal dynamics, and susceptibility to Epstein Barr Virus smooth muscle tumors (EBV-SMTs). Case reports associated with EBV-SMTs are limited. We describe herein a novel homozygous CARMIL2 variant (c.1364_1393del) in two Saudi Arabian male siblings born to consanguineous parents who developed EBV-SMTs. CARMIL2 protein expression was significantly reduced in CD4+ T cells and CD8+ T cells. T cell proliferation on stimulation with soluble (s) anti-CD3 or (s) an…

Risk score to predict event-free survival after hematopoietic cell transplant for sickle cell disease.

We developed a risk score to predict event-free survival (EFS) after allogeneic hematopoietic cell transplantation for sickle cell disease. The study population (n = 1425) was randomly split into training (n = 1070) and validation (n = 355) cohorts. Risk factors were identified and validated via Cox regression models. Two risk factors of 9 evaluated were predictive for EFS: age at transplantation and donor type. On the basis of the training cohort, patients age 12 years or younger with an HLA-matched sibling donor were at the lowest risk with a 3-year EFS of 92% (score, 0). Patients age 13 yea

Vaccination management in an asymptomatic child with a novel SCN1A variant and family history of status epilepticus following vaccination: A case report on a potential new direction in personalised medicine.

PURPOSE: SCN1A variants cause a spectrum of epilepsy syndromes from Dravet Syndrome, a severe epileptic encephalopathy of early infancy to the milder disorder of genetic epilepsy with febrile seizures plus (GEFS+). These genetic epilepsies are associated with increased risk of poor outcome including complications of status epilepticus and early mortality. Individualised management of young children known to be at increased risk should be considered, such as around vaccination management. METHODS: We describe two siblings with a novel pathogenic SCN1A variant, their management and clinical out…

First Molecular Diagnosis of Clinical Cases of Gastric Anisakiosis in Spain.

Anisakiosis is a fish-borne disease with gastrointestinal and/or allergic symptoms caused by the consumption of raw or undercooked fish parasitized with nematode larvae of the genus Anisakis. In Europe, Anisakis pegreffii has been detected as the causative agent, although the sibling species Anisakis simplex sensu stricto (s.s.) is also known to cause the disease in other parts of the world, and discrepancies exist regarding their respective pathogenic potential. In Spain a high number of cases has been recorded, with marinated anchovies being the main source of infection, although no specific
