Effect of maxillary anteroposterior position on profile esthetics in headgear-treated patients.

INTRODUCTION: Headgears have been used to treat Class II malocclusions for over a century. The purpose of this retrospective study was to investigate the profile esthetic changes resulting from headgear use in growing Class II patients with protrusive, normal, and retrusive maxillae. METHODS: Profile silhouettes were created from pretreatment and posttreatment lateral cephalometric tracings of growing Class II patients treated with headgear followed by conventional fixed appliances. Ten patients had an initially protrusive maxilla (FH:NA, >92°), 10 had an initially normally positioned maxilla

Burst fractures of the thoracolumbar spine: changes of the spinal canal during operative treatment and follow-up.

Although multiple studies have concluded operative decompression of a traumatically narrowed spinal canal is not indicated because of spontaneous remodeling, instrumental decompression is frequently used as part of the operative treatment of spinal fractures. To investigate the process of remodeling, we studied the diameter of the spinal canal in 95 patients with burst fractures at the thoracolumbar junction (T9-L2). To measure and compare the spinal canal's diameter we used either computed tomography (CT) scans or radiographs, made preoperatively, postoperatively, after 9 months and after 24

Callus distraction of the midface in the severely atrophied maxilla--a case report.

OBJECTIVE: This report introduces the possibilities of callus distraction in the extremely atrophied, edentulous midface in a cleft lip and palate patient. INTERVENTION: After a subtotal Le Fort II osteotomy, tension wires were fixed to the zygomatic buttresses and frontal sinus walls by way of titanium miniplates and mesh and connected to a rigid external distractor. Then distraction of the whole midface (1 mm/d) was performed. RESULTS: Even in severe atrophy a distraction of the maxilla of 20 mm was possible. Stability has been shown for more than 5 months. CONCLUSIONS: Rigid external mid…

Cervical radiculopathy.

The history, pathoanatomy and pathophysiology, clinical picture, differential diagnosis, diagnostic evaluation, and treatment of cervical radiculopathy are reviewed. The review is based on a 10-year Medline literature search, review of bibliographies in textbooks, and bibliographies in articles obtained through the search. Cervical radiculopathy, although recognized early in the 20th century, was first associated with disc pathology in the mid-1930s. It is most commonly caused by disc herniation or cervical spondylosis. History and physical examination using pain location, manual muscle testin

Hangman's fracture in Singapore (1975-1988).

A retrospective study of 33 patients with fracture of the ring of the axis (hangman's fracture) admitted to the Spinal Unit of the Department of Rehabilitation Medicine in the Tan Tock Seng Hospital between 1975 and 1988 was carried out. The aims were to establish the causes, mechanism and outcome of injuries that lead to Hangman's fracture in Singapore. The majority were males (27) and their ages ranged from 16 to 82 with a mean age of 33.7 years. 63.6% (21 cases) were due to road traffic accidents of whom 33% (11 cases) were motorcyclists or pillion riders and 30.3% (10 cases) were drivers o

Prognostic factors in congenital dislocation of the hip treated with closed reduction. The importance of arthrographic evaluation.

The conservative management of Bennett's fracture-dislocation: a 26-year follow-up.

Seventeen patients who had suffered a Bennett's fracture-dislocation have been reviewed with an average follow up of 26 years. All were treated conservatively. At review, only seven patients had symptoms, but all had a decreased range of movement and grip strength. Twelve had a characteristic deformity in the hand. Radiographs showed persistent subluxation of the first carpo-metacarpal joint and marked degenerative changes. We suggest that, in the light of the poor long-term outcome, this injury should not be managed conservatively but by some operative means.

Derotation varus osteotomy in Perthes disease.

Two groups of patients with severe forms of Perthes disease were studied. Containment was obtained by performing a derotation varus osteotomy in 32 hips, and by casts and splints in 12 hips. The clinical results were better in the operated group than in the nonoperated group. Seventy-five percent of the hips in the operated group had good to fair radiological results, compared with 41% fair results only in the nonoperated group. The various radiological methods of assessment are outlined. It is concluded that derotation varus osteotomy has advantages over nonoperative treatment in severe Perth

Acetabular development after reduction in congenital dislocation of the hip.

A study of acetabular development in 148 patients with 185 congenitally dislocated hips treated by closed reduction from 1940 to 1968 showed that eight years after reduction the acetabular index was normal (less than 20 degrees) in 49.2 per cent, satisfactory (from 20 to 24 degrees) in 34 per cent, and poor (more than 24 degrees) in 16.7 per cent. Early treatment led to the best acetabular development. Significant and continuing improvement of the acetabular index was observed between two and eight years after reduction when the femoral head remained concentrically reduced. Failure to maintain

[Injuries of the cervical spine].

In connection with 44 patients, who suffered injuries of the cervical spine, the types of the fracture and dislocation of the cervical spine are discussed. The injuries of the cervical spine--with or without injury of the spinal marrow--signify hard tast in respect of therapy. The conservative (extension) treatment of surgical intervention are even nowadays much debated themes in the neurosurgery. The introduction of Crutchfield tongs was a definite advance in the latter years. With this treatment method the restitution of the form, continuity and stability of the cervical spine is possible. Operator-Driven Active Craniospinal Tensioning (ACT) Device for Non-ambulatory Application.

Active craniospinal tensioning (ACT) is an upright-posture axial spinal traction maneuver hypothesized to couple two mechanisms: dural pull-recoil, a cerebrospinal fluid (CSF) pressure-gradient effect shared with the supine technique pelvis-stabilized axial spinal traction (PSAST), and suboccipital venous occlusion-rebound, proposed to promote central nervous system (CNS) venous and lymphatic drainage, and as a distinct modality for cerebral venous preconditioning (CVPC). The CVPC mechanism depends on upright posture. In the upright position, cerebral venous return preferentially routes throug…
