Study of the effect of Occlutech Atrial Flow Regulator on symptoms, hemodynamics, and echocardiographic parameters in advanced pulmonary arterial hypertension.

Optimal sized balloon atrial septostomy improves hemodynamics in advanced pulmonary arterial hypertension. Occlutech Atrial Flow Regulator is designed to provide an atrial septal fenestration diameter titrated according to the age and right atrial pressures. This observational study analyzed symptoms, exercise distance, oxygen saturations, hemodynamics and echocardiographic parameters after Atrial Flow Regulator implantation in patients with syncope or right-heart failure. Patients with high-risk predictors of mortality during septostomy were scrutinized. Thirty-nine patients (9 children) with

Fetal surgical interventions and the development of the heart in congenital diaphragmatic hernia.

Selective left ventricular hypoplasia is a recognized feature of congenital diaphragmatic hernia (CDH). It is speculated that the herniated viscera act as a space occupying lesion that inhibit normal cardiac development. The purpose of this study was to determine the effect of two separate fetal surgical interventions on subsequent cardiac development in CDH. CDH lambs were created at 80 days gestation and underwent either tracheal ligation (CDH + TL) or diaphragmatic repair (CDH + repair) at 110 days gestation. At term (141 days gestation) the hearts were harvested and fixed in 4% paraformald

Reduced cardiac function is associated with cardiac injury and mortality risk in hospitalized COVID-19 Patients.

BACKGROUND: Cardiac injury is common in COVID-19 patients and is associated with increased mortality. However, it remains unclear if reduced cardiac function is associated with cardiac injury, and additionally if mortality risk is increased among those with reduced cardiac function in COVID-19 patients. HYPOTHESIS: The aim of this study was to assess cardiac function among COVID-19 patients with and without biomarkers of cardiac injury and to determine the mortality risk associated with reduced cardiac function. METHODS/RESULTS: This retrospective cohort study analyzed 143 consecutive COVID-

Beyond the murmur: unmasking a rare sinus venosus ASD in an adolescent with chronic respiratory infections: A case report.

RATIONALE: Overall, 5% to 10% of all atrial septal defects (ASDs) are of the sinus venosus type, a rare congenital disease. This defect must be identified and treated immediately since it might cause significant left atrial dilatation and dyspnea. PATIENT CONCERNS: A 15-year-old male presented with previously undetected sinus venosus type ASD. The patient arrived with a high-grade heart murmur and a history of recurring respiratory illnesses. DIAGNOSES: A clinical examination identified a 3/6 systolic ejection murmur in the second left intercostal gap and a fixed split second heart sound. El

Contemporary outcomes of continuous-flow left ventricular assist devices-a systematic review.

BACKGROUND: End stage heart failure is a major cause of morbidity and mortality, and its prevalence is expected to rise with the ageing population. For suitable patients, orthotopic heart transplantation remains the gold standard therapy, however, a paucity of donor organs has led to the development of left ventricular assist devices (LVAD). These devices can be utilized as either a bridge-to-transplant (BTT) or as an alternative to heart transplantation. While these devices can prolong life and improve quality of life, they are associated with a significant number of adverse events. We aim to

Effects of Qili Qiangxin granules in combination with Entresto on quantitative parameters of myocardial motion in patients with cardiomyopathy in end-stage renal disease.

BACKGROUND: Cardiomyopathy is a devastating complication of end-stage renal disease (ESRD), characterized by complex remodeling and high cardiovascular mortality. While sacubitril/valsartan (Sac/Val) and Qili Qiangxin (QLQX) granules have shown individual benefits in heart failure, their combined efficacy in ESRD patients remains poorly defined. OBJECTIVE: To evaluate the impact of adjunctive QLQX therapy with Sac/Val on quantitative myocardial motion, cardiac function, and hemodynamic parameters in patients with ESRD. METHODS: In this retrospective cohort study, 109 ESRD patients with heart

Long-term follow-up of selective and non-selective His bundle pacing leads in patients with atrioventricular block.

BACKGROUND: His bundle pacing (HBP) is a novel treatment with limited knowledge on long-term outcome. We aimed to assess long-term safety and effectiveness of HBP in patients with atrioventricular block treated with HBP and a back-up right ventricular pacing (RVP) lead. METHODS: We included 38 patients from a completed single-center, randomized controlled cross-over trial designed to compare left ventricular (LV) function after 12 months of HBP vs. RVP conducted between September 2007 and August 2011. Lead performance beyond the 2-year study period was assessed based on a retrospective review

Evaluation of right ventricular myocardial deformation properties in fetal hypoplastic left heart by two-dimensional speckle tracking echocardiography.

PURPOSE: Right ventricular (RV) function influences the outcome of hypoplastic left heart (HLH) patients. This study aimed to confirm the assumption of prenatal RV remodeling and possible influencing factors of myocardial restructuring using two-dimensional speckle tracking echocardiography (2D STE). METHODS: This is a retrospective cross-sectional cohort study including HLH fetuses and gestational age-matched controls. Based on a four-chamber view, cine loops were stored with 60 frames per second. Global longitudinal peak systolic strain (GLPSS) of the RV was retrospectively determined and c

Detection of Congestive Heart Failure and Myocardial Dysfunction in Cats With Cardiomyopathy by Using Two-Dimensional Speckle-Tracking Echocardiography.

Congestive heart failure (CHF) is a life-threatening condition in cats with cardiomyopathy. We hypothesized that myocardial dysfunction may induce progression to CHF pathophysiology in cats with cardiomyopathy. However, no previous studies have evaluated the involvement of myocardial dysfunction in cats with CHF. In this study, we aimed to evaluate the relationship between CHF and myocardial function assessed using two-dimensional speckle-tracking echocardiography (2D-STE). Sixteen client-owned healthy cats and 32 cats with cardiomyopathy were enrolled in this study. Cats were classified into

Restoration of ventricular septal hypoperfusion by cardiac resynchronization therapy in patients with permanent right ventricular pacing.

BACKGROUND: Pacing from the right ventricular apex (RVA) is associated with cardiac dysfunction and shows electrophysiological features similar to left bundle branch block in which left ventricular (LV) mechanical dyssynchrony impairs septal coronary artery perfusion. METHODS: A total of 62 non-ischemic patients with an implanted pacemaker at the RVA with a pacing rate of >95% were studied. LV septal coronary perfusion as indicated by the LV septal perfusion index was measured by electrocardiography (ECG)-gated single-photon emission computed tomography for all patients at baseline and for pa

Loss of vagal tone aggravates systemic inflammation and cardiac impairment in endotoxemic rats.

BACKGROUND: During the course of sepsis, often myocardial depression with hemodynamic impairment occurs. Acetylcholine, the main transmitter of the parasympathetic Nervus vagus, has been shown to be of importance for the transmission of signals within the immune system and also for a variety of other functions throughout the organism. Hypothesizing a potential correlation between this dysfunction and hemodynamic impairment, we wanted to assess the impact of vagal stimulation on myocardial inflammation and function in a rat model of lipopolysaccharide (LPS)-induced septic shock. As the myocardi

Mineralocorticoid receptor antagonism attenuates experimental pulmonary hypertension.

Mineralocorticoid receptor (MR) activation stimulates systemic vascular and left ventricular remodeling. We hypothesized that MR contributes to pulmonary vascular and right ventricular (RV) remodeling of pulmonary hypertension (PH). We evaluated the efficacy of MR antagonism by spironolactone in two experimental PH models; mouse chronic hypoxia-induced PH (prevention model) and rat monocrotaline-induced PH (prevention and treatment models). Last, the biological function of the MR was analyzed in cultured distal pulmonary artery smooth muscle cells (PASMCs). In hypoxic PH mice, spironolactone a
